Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
A new Rochester Outpatient Clinic is scheduled to fully open in April 2019, and has a requirement to procure new equipment for the Dental X-ray Department. The complete requirement is as follows:
Project Room 1903 Quantity 1 Cone Beam Computed Tomography Unit with 3D Face and model scan.
Model Planmeca Promax 3D Mid with 3D Sensor & SP.
Unit offers 3D imaging, 2D panoramic and cephalometric imaging capabilities.
Complies with ALARA (As Low As Reasonably Achievable) Principle for patient does levels.
Open patient positioning with fine adjustment using positioning laser and a joystick.
Unit offers easy wheelchair accommodation with side-entry access.
The ROI (Region of Interest) reconstruction function generates a small voxel volume from the image data of a previously taken large voxel volume; provides precise diagnosis without the need for an additional radiation dose for the patient.

Optimised imaging modes for different needs •Low dose mode takes the image with a minimal dose of radiation. Suitable for child patients in orthodontic studies. Voxel size 400 µm •Normal mode is the best choice for most common imaging needs. Voxel size 200 µm •High definition mode is designed for imaging of small objects, such as ear bones. Voxel size 150 µm •High resolution gives more detail, when necessary. Voxel size 100 µm •Endodontic mode offers the best resolution with the smallest size. Voxel size 75 µm
Precise, free-flowing, computer-controlled SCARA (Selectively Compliant Articulated Robot Arm) arm construction can produce any movement pattern required. This enables accurate and reliable volume positioning and volume diameter adjustment, reducing the amount of radiation your patients are exposed to.

Project Room 1903 Quantity 1 Planmeca Correction Algorithm for Latent Movement Required Algorithm to reduce patient retakes and guards patients from unnecessary exposures.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
The equipment listed in this justification is being purchased for a newly constructed (and soon to be opened) VA Community Based Outpatient Clinic (CBOC) located in Rochester, NY.
The Canandaigua and Bath VA Medical Centers are two of five VA medical centers that comprise the VA Upstate New York Healthcare System. These two facilities have integrated to create what will be known as the VA Finger Lakes Health Care System. With the integration, the expectation is that medical providers may work at any of the facilities within the region to include Bath, Canandaigua and their respective community based outpatient clinics (Rochester, Elmira, and Wellsville). To ensure continuity in care, as well as compatibility of equipment at all facilities that will comprise the Finger Lakes Healthcare System, all facilities within the region must standardize medical equipment.
Must be Veterans Integrated System Technology Architecture “VISTA” Imaging, Digital Imaging and Communications in Medicine “DICOM” compliant, and be able to connect to Hospital Information System “HIS”, Radiology Information System “RIS”, and Picture Archiving and Communication System “PACS” for efficient data transfer. Integration must be capable to the Electronic Health Record “EHR”
4. Description of market research conducted and results or statement why it was not conducted:
A comprehensive search on FPDS-NG indicates that there are at least two (2) or more Service Disabled Veteran Owned Small Businesses that have recently been awarded VA contracts for Planmeca imaging equipment, in addition to several purchases by VA facilities directly from the manufacturer.

A search of Vendor Information Pages (VIP), www.vetbiz.gov using the search word “Planmeca” resulted in at least one match for a Service Disabled Veteran Owned Small Business Vendor.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. .
