On Thursday, March 7 th , 2013, Diversified Geophysics, Inc. (DGI) performed a geophysical investigation on the property known as the Montrose VA Hospital, located at 2094 Albany Post Road, in Montrose, NY, 10548. DGI personnel met with STV personnel at the site, and followed your directions throughout the investigation.
The purpose of this investigation was to locate, trace and mark the presence of any subsurface utilities, documented or undocumented structures and/or anomalies within the areas of investigation (AOI’s), in advance of the boring program. Six (6) proposed sampling locations were investigated for obstructions and cleared for drilling.
The geophysical investigation was comprised of a series of Ground Penetrating Radar (GPR) traverses and site-wide radio frequency/electromagnetic (RF/EM) scans. GPR surveys collect anomaly data which can be interpreted in real-time. The geometry of the anomalies located can assist in determining the type of subsurface object producing the anomaly. Linear anomalies can be interpreted to be buried utilities, conduits, trolley tracks, etc., if the return data has the appropriate signature. RF/EM scans are made by using instrumentation that has a transmitter and receiver. The transmitter can be used to send electromagnetic waves of known frequencies along buried metallic conduits housing utilities, or through the utilities themselves. The receiver is used to trace the signal (audibly and/or graphically) and locate the direction of travel, in some cases, the depth of the utility below the surface. Receivers can also be used in stand-alone mode to detect frequencies in the range of 60Hz, often emitted by active electrical services in the United States. In addition, some RF/EM instruments can detect frequencies emitted by gas and water services. These utilities absorb and radiate VLF radio waves. Both technologies, GPR and RF/EM complement one another to provide the most useful data set necessary for the locating, tracing and characterization of subsurface anomalies.
Ground Penetrating Radar (GPR) data was collected with Sensors and Software 250 MHz Noggin
Plus SmartCart
The average penetration depth for this investigation was set from zero (0) to ten (10) feet. In some areas, this was increased from zero (0) to twenty (20) feet.
During this investigation, Noggin
Plus GPR traverse lines were made in sets that ran parallel and perpendicular to the footprints of the individual AOI’s, as illustrated on Figure 2. of this narrative (west and south of building 20; east, west and south of building 23; and within the landfill between the east and north gate-lines to a point 40 feet west of the storm structure, and south to the materials and equipment line). In addition, areas of the east parking area located to the south of building 19 were investigated and all accessible manhole covers were opened to evaluate the disposition of the plumbing –sanitary sewer or storm sewer. GPR scans represent anomalies graphically as hyperbolas, their crests indicating position and depth below the surface to the target. In general, when scanning for linear targets such as utilities, single-line scans are designed to traverse possible targets in directions perpendicular to the long-axis of the target. When a series of parallel scans detect anomalies at similar positions and depths, these results are interpreted to be evidence of a utility or conduit.
Single line GPR traverses in both the X and Y direction were collected throughout all accessible areas of the property.
Subsurface Geophysical Investigations 150 Nassau Terminal Road, New Hyde Park, NY 11040
A radio frequency/electromagnetic (RF/EM) survey was conducted throughout the entire area of investigation. The receiver of a Ridgid
SeekTech (SR-60) Pipe and Cable Locator was used in a stand-alone mode (without the transmitter) to locate utilities emitting frequencies in the 60Hz range (active electric). The Ridgid receiver was also used with a transmitter box (ST-33Q) in an attempt to trace water and drainage services.
Results:
All drainage conduits were traced and marked on the surface in the AOI to the west of buildings 20 and 23. Storm drainage was traced through the east fence of the former landfill area, where it entered a concrete vault located in the northeastern corner of the landfill property. Visual examination of the vault revealed that the plumbing exited the west wall of the enclosure, and may have continued through to a second concrete vault located approximately 60 feet to the southwest. Access issues prevented the tracing of this section of the storm drainage with the GPR.
Attempts were made to determine the direction of travel of storm drainage conduits entering and exiting a grated concrete structure located centrally in the former landfill area (approximately 160 feet west of the entry gate).
Anecdotal information indicated that a communication existed between the two drainage vaults to the northeast;
and that a 40 foot length of drainage pipe exited the structure on a southwest trend. GPR data sets collected throughout the AOI did not reveal any linear target returns that could be interpreted as indication of the presence of storm drainage pipe. The extremely disturbed, heterogeneous nature of the substrate was a factor in the lack of interpretable data. An additional complication, flagging and paint markings from a former geophysical study indicated a drainage conduit paralleling the north fence-line of the landfill from the second interior vault (from the east fence) to a point due north of the central storm structure. GPR traverses across this area to the north of the central drainage structure did not reveal linear anomalies that could be interpreted to be drainage conduit.
RF technology was used throughout the former landfill in an attempt to trace storm drainage. RF technology could not be used to induce a signal through a steel tape placed into the storm drainage conduit, due to the presence of water in the piping –(which would result in the loss of a traceable return signal). Signals were transmitted both inductively (by placing the transmitter box above a known or suspected section of drainage pipe), or conductively by placing the transmitter box directly on an accessible, exposed section of drainage pipe. Weak signal returns in the area to the west of the central drainage structure did appear to reveal a preferred direction to the southwest;
and similar results were encountered along the line of travel marked during the former geophysical survey (which resulted in finding the paint marks and flagging). Due to the amount of metallic debris buried in the landfill, tracing the RF signal was compromised.

CASE NARRATIVE

This case narrative is in the form of an exception report, where only the anomalies related to this report, method specific performance and/or QA/QC issues are discussed. If there are no issues to report, this narrative will include a statement that documents that there are no relevant data issues.
It should be noted that samples with elevated Reporting Limits (RLs) as a result of a dilution may not be able to satisfy customer reporting limits in some cases. Such increases in the RLs are unavoidable but acceptable consequence of sample dilution that enables quantification of target analytes or interferences which exceed the calibration range of the instrument.
Calculations are performed before rounding to avoid round-off errors in calculated results.
All holding times were met and proper preservation noted for the methods performed on these samples, unless otherwise detailed in the individual sections below.
RECEIPT
The samples were received on 06/25/2013; the samples arrived in good condition, properly preserved and on ice. The temperature of the coolers at receipt was 2.5° C.
Note: All samples which require thermal preservation are considered acceptable if the arrival temperature is within 2C of the required temperature or method specified range. For samples with a specified temperature of 4C, samples with a temperature ranging from just above freezing temperature of water to 6C shall be acceptable. Samples that are hand delivered immediately following collection may not meet these criteria, however they will be deemed acceptable according to NELAC standards, if there is evidence that the chilling process has begun, such as arrival on ice, etc.
IGNITABILITY
Samples 460-58470-3, 460-58470-6, 460-58470-9, 460-58470-12, 460-58470-15, 460-58470-18 and 460-58470-19 were analyzed for
Ignitability in accordance with EPA SW-846 Method 1030. The samples were analyzed on 06/28/2013.
No difficulties were encountered during the burn rate analyses.
All quality control parameters were within the acceptance limits.
TCLP METALS
Samples 460-58470-3, 460-58470-6, 460-58470-9, 460-58470-12, 460-58470-15, 460-58470-18 and 460-58470-19 were analyzed for
TCLP metals in accordance with EPA SW-846 Methods 1311/ 6010B. The samples were leached on 06/26/2013 and 06/27/2013, and prepared and analyzed on 06/28/2013.
As a standard practice all TCLP samples are diluted 5X prior to analysis. Further dilutions may be required dependent upon analyte levels in the samples. Refer to the analytical results forms for dilutions.
No difficulties were encountered during the TCLP metals analyses.
All quality control parameters were within the acceptance limits.
TOTAL METALS
Samples 460-58470-1, 460-58470-2, 460-58470-4, 460-58470-5, 460-58470-7, 460-58470-8, 460-58470-10, 460-58470-11, 460-58470-13, 460-58470-14, 460-58470-16, 460-58470-17 and 460-58470-19 were analyzed for total metals in accordance with EPA
SW-846 Method 6010B. The samples were prepared and analyzed on 06/26/2013 and 06/27/2013.
Samples 460-58470-1(4X), 460-58470-2(4X), 460-58470-4(4X), 460-58470-5(4X), 460-58470-7(4X), 460-58470-8(4X), 460-58470-10(4X), 460-58470-11(4X), 460-58470-13(4X), 460-58470-14(4X), 460-58470-16(4X), 460-58470-17(4X) and
460-58470-19(4X) required dilution prior to analysis. The reporting limits have been adjusted accordingly.
As a standard practice all soil samples and related QC samples (i.e., MB, LCS, Dup, MS, SD) are diluted 2X-4X prior to analysis. Further dilutions may be required dependent upon analyte levels in the samples. Refer to the analytical results forms for dilutions.

TCLP mercury in accordance with EPA SW-846 Methods 1311/7470A. The samples were leached on 06/26/2013 and 06/27/2013, and prepared and analyzed on 06/28/2013.
No difficulties were encountered during the TCLP mercury analyses.
All quality control parameters were within the acceptance limits.
TOTAL MERCURY
Samples 460-58470-1, 460-58470-2, 460-58470-4, 460-58470-5, 460-58470-7, 460-58470-8, 460-58470-10, 460-58470-11, 460-58470-13, 460-58470-14, 460-58470-16, 460-58470-17 and 460-58470-19 were analyzed for total mercury in accordance with EPA
SW-846 Method 7471A. The samples were prepared and analyzed on 06/26/2013 and 06/27/2013.

ORGANOCHLORINE PESTICIDES
Samples 460-58470-1, 460-58470-2, 460-58470-4, 460-58470-5, 460-58470-7, 460-58470-8, 460-58470-10, 460-58470-11, 460-58470-13, 460-58470-14, 460-58470-16, 460-58470-17 and 460-58470-19 were analyzed for organochlorine pesticides in accordance with EPA SW-846 Method 8081A. The samples were prepared on 06/26/2013 and 06/27/2013 and analyzed on 06/27/2013 and 07/01/2013.
DCB Decachlorobiphenyl failed the surrogate recovery criteria high for MB 460-167885/1-A. DCB Decachlorobiphenyl failed the surrogate recovery criteria high for 460-58552-A-6-F MS. DCB Decachlorobiphenyl failed the surrogate recovery criteria high for
460-58463-E-1-H MSD. Refer to the QC report for details.
No other difficulties were encountered during the pesticides analyses.
All other quality control parameters were within the acceptance limits.
POLYCHLORINATED BIPHENYLS
Samples 460-58470-1, 460-58470-2, 460-58470-4, 460-58470-5, 460-58470-7, 460-58470-8, 460-58470-10, 460-58470-11, 460-58470-13, 460-58470-14, 460-58470-16, 460-58470-17 and 460-58470-19 were analyzed for polychlorinated biphenyls in accordance with EPA SW-846 Method 8082. The samples were prepared and analyzed on 06/26/2013 and 06/27/2013.
The closing calibration verification (CCV) for Aroclor 1260 associated with batch 167906 recovered above the upper control limit on the secondary column. The samples associated with this CCV were non-detects for the affected analytes; therefore, the data have been reported.
No other difficulties were encountered during the PCBs analyses.
All other quality control parameters were within the acceptance limits.
CHLORINATED HERBICIDES
Samples 460-58470-1, 460-58470-2, 460-58470-4, 460-58470-5, 460-58470-7, 460-58470-8, 460-58470-10, 460-58470-11, 460-58470-13, 460-58470-14, 460-58470-16, 460-58470-17 and 460-58470-19 were analyzed for chlorinated herbicides in accordance with EPA SW-846 Method 8151A. The samples were prepared on 06/26/2013 and analyzed on 06/27/2013 and 06/28/2013.
The closing calibration verification (CCV) for analytical batch 168029 recovered outside control limits for 2,4,5-TP. The data have been qualified and reported.
Refer to the QC report for details.
No other difficulties were encountered during the herbicides analyses.
All other quality control parameters were within the acceptance limits.
VOLATILE ORGANIC COMPOUNDS (GC-MS)
Samples 460-58470-1, 460-58470-2, 460-58470-4, 460-58470-5, 460-58470-7, 460-58470-8, 460-58470-10, 460-58470-11, 460-58470-13, 460-58470-14, 460-58470-16, 460-58470-17 and 460-58470-19 were analyzed for volatile organic compounds (GC-MS) in accordance with EPA SW-846 Method 8260B. The samples were prepared on 06/27/2013 and analyzed on 07/04/2013, 07/05/2013 and
07/06/2013.
The following sample was diluted to bring the concentration of target analytes within the calibration range: SB-3A (2-4') (460-58470-7).

SEMIVOLATILE ORGANIC COMPOUNDS (GC-MS)
Samples 460-58470-1, 460-58470-2, 460-58470-4, 460-58470-5, 460-58470-7, 460-58470-8, 460-58470-10, 460-58470-11, 460-58470-13, 460-58470-14, 460-58470-16, 460-58470-17 and 460-58470-19 were analyzed for semivolatile organic compounds
(GC-MS) in accordance with EPA SW-846 Method 8270C. The samples were prepared on 06/25/2013 and 06/26/2013 and analyzed on
06/29/2013, 07/01/2013 and 07/02/2013.
1,2,4,5-Tetrachlorobenzene and Hexachlorocyclopentadiene failed the recovery criteria low for LCS 460-167865/2-A.
4-Chloro-3-methylphenol and 4-Nitroaniline failed the recovery criteria high. Refer to the QC report for details.
Samples 460-58470-1(5X), 460-58470-2(5X), 460-58470-4(2X), 460-58470-5(2X) and 460-58470-13(2X) required dilution prior to analysis. The reporting limits have been adjusted accordingly.
No other difficulties were encountered during the semivolatiles analyses.
All other quality control parameters were within the acceptance limits.
REACTIVE CYANIDE
Samples 460-58470-3, 460-58470-6, 460-58470-9, 460-58470-12, 460-58470-15, 460-58470-18 and 460-58470-19 were analyzed for reactive cyanide in accordance with EPA SW-846 Method 7.3.3/9014. The samples were prepared and analyzed on 06/27/2013.
No difficulties were encountered during the reactive cyanide analyses.
All quality control parameters were within the acceptance limits.
REACTIVE SULFIDE
Samples 460-58470-3, 460-58470-6, 460-58470-9, 460-58470-12, 460-58470-15, 460-58470-18 and 460-58470-19 were analyzed for reactive sulfide in accordance with EPA SW-846 Method 7.3.4/9034. The samples were prepared and analyzed on 06/27/2013.
No difficulties were encountered during the reactive sulfide analyses.
All quality control parameters were within the acceptance limits.
CORROSIVITY (PH)
Samples 460-58470-3, 460-58470-6, 460-58470-9, 460-58470-12, 460-58470-15, 460-58470-18 and 460-58470-19 were analyzed for corrosivity (pH) in accordance with EPA SW-846 Method 9045C. The samples were analyzed on 06/27/2013.
This analysis is normally performed in the field and has a method-defined holding time of 15 minutes. The following sample(s) has been qualified with the "HF" flag to indicate analysis was performed in the laboratory outside the 15 minute timeframe: REP062413

CLPISM01.2 (Exhibit D). The samples were analyzed on 06/26/2013.
