PURCHASE ORDER FOR THE PUCHASE OF Zeiss HFA3 Model 860
THIS IS A SOURCES SOUGHT NOTICE ONLY FOR Togus VAMC PURCHASE REQUISITION: 608-21-2-100-0035: This announcement is seeking information from the industry, which will be used for preliminary planning purposes for an open market procurement. No proposals are being requested or accepted with this synopsis. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS SYNOPSIS. THIS IS STRICTLY MARKET RESEARCH.
The purpose of this synopsis is to gain knowledge of interest, capabilities and qualifications of various members of industry, who have the capability to supply Zeiss HFA3 Model 860 as specified in the table below.
Supplying this request includes the supply/delivery of the Zeiss HFA3 Model 860.
VAMC Togus: Zeiss HFA3 Model 860
This Sources Sought is Brand Name Specific. The work shall include product order and delivery. The standard warranty will be provided at no additional cost. The vendor must also demonstrate they are competent and certified by the OEM to work on the listed equipment.
All contractors with interest meeting the requirements identified in this notice should submit the attached area of consideration with their response: The eventual vendor must satisfy the following criteria: delivery on or during the week as agreed upon with the customer after the final selection is made, the technical requirement of supplying the brand name equipment as specified, and providing a quote at a fair and reasonable price—and that also affords best value. Prior Government contracts are not required to submit a response under this Sources Sought synopsis.
The intent of the Sources Sought is to identify vendors in the open market who can fulfill the requirements as stated in the above chart. This notice is published to conduct market research to determine if there is a sufficient number of vendors in the open market who can supply this order in aggregate, while affording competition to the fullest extent possible.
All interested firms must be registered in SAM (System for Award Management) to be eligible for award of Government contracts. Interested parties responding to this Sources Sought request shall submit the following information at a minimum:
(1) Company name and address, point of contact with phone number, and DUNS number;
(2) Documentation relating to capability of performance including installation in a hospital environment, training, and a current point of contact and phone number);
(3) Socio-economic status to identify business class as being a large business, SDVOSB, VOSB, small business, Woman Owned, etc.
(4) Service Area
(5) Any other pertinent company documentation, prior to submission of offers to a solicitation.
Any SD/VOSB firms responding to this Sources Sought Notice must also provide proof of registration in the VA VetBiz, prior to submission of offers to a solicitation.

This market research is for informational and planning purposes. This is not a solicitation and does not obligate the Government to issue a solicitation. Please be advised the Government will not pay any costs for responses and information submitted. All submissions are acceptable via peter.bedard@va.gov.
RESULTS INFORMATION: Respondents will not be notified of the results and it is at the discretion of the Government to utilize results. Firms responding to this Sources Sought announcement, who fail to provide ALL of the required information requested, will not be used to help the Government make the acquisition decision, which is the intent of this sources sought announcement.

Performance Work Statement (PWS) Design and Content
1.0 Introduction
The VA Maine HCS requires three Humphrey Field Analyzer 3 from ZEISS foe the Eye clinic.
2.0 Background
In fulfilling the core values of the VA, our clinic strives to maintain excellence by providing the highest quality care while looking for continuous improvement. This technology will help us provide the highest quality care and allow for earlier diagnosis and better management of various ocular diseases. The strategic plan of VA calls for modernizing systems for better efficiency. This visual field technology allows for more efficient testing and better integrates with our new FORUM platform keeping VA competitive with the private market.
3.0 Scope (Provide an overview of the PWS emphasizing the most important aspects of your requirement and defining the boundaries of the contractor’s performance responsibilities).
This project will provide three State of the Art Humphrey Field Analyzer 3 from ZEISS Systems for the Eye Clinic department. Visual fields are a required part of glaucoma management. This new visual field machine allows for faster testing (SITA-Faster) increasing the comfort of patients taking the test. The new 24-2C test allows for more reliable results and earlier glaucoma diagnosis. Supporting the studies showing early glaucomatous changes pericentrally that may be missed on the traditional 24-2 visual field. This visual field is highly compatible with FORUM software and Zeiss OCTs already used in all three eye clinics. The integration of these technologies with the new visual field machine allows for the use of disease progression software to aid in management of these patients. This new visual field has a liquid lens eliminates the need for changing trial lenses between tests, leads to faster and more efficient testing. This reduces the chance of incorrect lenses being used that may give inaccurate test results. Having a single lens to clean rather than multiple trial lenses aids in infections control as well. For some veterans with back/neck issues it can be uncomfortable to keep the posture needed for a visual field test, faster testing methods allow for a shorter time needed in this position. The combination of liquid lens, faster testing methods means our technicians will be able to provide visual field testing at a faster rate, allowing them to aid other veterans’ needs. Increased reliability and accuracy of visual field test may lead to less repeat follow up visits, freeing clinic spots for other veterans. This is the standard of care outside the VA and with essential features as;
· Expand testing options- Optimize your patient management with new SITA™ Faster 24-2 and 24-2C tests.
· Identify progression- Guided Progression Analysis™ (GPA™) helps determine if visual field loss is progressing (where and how fast) to help augment treatment.
· Streamline workflow- Reduce set-up time and provide best test results with the Liquid Trial Lens™ and automated eye alignment.
· Interact with results- Access HFA3 results and the entire patient test history as well as change baselines on the fly.
· Synchronize data for complete patient history- Test patients at any HFA3 or HFAII-i, and generate reports with complete test history.
· See the whole picture. HFA is the cornerstone of the Integrated Diagnostic Imaging platform for glaucoma that provides a new level of information for optimal patient management based on visual field function and corresponding OCT structure data.
The Art Humphrey Field Analyzer package shall adhere to all VA Privacy & Security, the Privacy Act and HIPAA requirements when obtaining and reviewing diagnostic information. This includes any or all log files, error traps, screen capture images and database query results used to aid Technical and Installation in their troubleshooting activities.
Lastly this project will include all labor and outside resources to ensure full operability.
4.0 Applicable Directives (List all documents referenced in the next section 5.0, Performance Requirements, with which the contractor will have to comply in performance of the contract effort. If only portions of a given document apply, specifically identify the applicable portions).
The system must meet the manufacturer’s specifications and be designed and implemented within the vendors 510K system design as an FDA regulated medical device/system.
5.0 Performance Requirements (Identify what the contractor is required to do (major tasks and subtasks).
The vendor will as part of their project plan outline a timeline for implementing the project for the Eye Clinic in the VA Maine Healthcare System.
6.0 Deliverables (List all data required to manage or monitor contractor performance).
The vendor as part of the quote will provide the following to ensure the configuration of the systems and will ensure full functionality.
· One-year manufacturer warranty
· Delivery
· Instillation of stand-alone instrument only
· Training
· Two operation manuals
· Two service manuals
This project will include all and any peripheral items to ensure a fully functional system. (misc. hardware or req. hardware upgrades, network switches, video cards, KVM switches, cabling, mouse, keyboards etc.) to ensure the functionality of the final product.
