STATEMENT OF WORK

Epoxy Flooring Removal & Replacement

June 10, 2021

DESCRIPTION OF SERVICES

1. SCOPE OF WORK Bldg. 898, 224 Galaxy Rd, San Antonio, TX, 78236 is in need of the removal of the existing epoxy flooring and the application of new epoxy flooring in the Structural and Metals Tech shop floors.

SPECIFIC TASKS The contractor shall furnish all labor, supervision, and material to apply/install epoxy flooring for the aforementioned shops. The Contractor shall visit the job site to thoroughly familiarize himself with all the details of the work and working conditions. The contractor is expected to replace and renew approximately 6200 square feet of flooring. However, it is the Contractors responsibility to measure and verify all dimensions. The contractor shall advise SMSgt Wayne White of the 433 MXS/AW Fabrication Flight Office of any discrepancies before performing any work.

The Contractor shall be specifically responsible for the coordination and proper relation of this work to the building, structure, phasing and ensure the safety of employees and workmen. Final top coat color is to be selected by SMSgt Wayne White. Additionally, any damages to the walls during the project shall be repaired at no cost to the government.

2. SPECIFIC TASK DETAILS

2.1 The project is expected to be completed in three distinct phases:

2.1.1 Phase I Removal off nonstationary equipment will be the government’s responsibility prior to the start of

Phase I. Phase I will begin with contractor taping off all remaining stationary equipment. This is to ensure stationary items are protected from the floor covering removal and re-application. Once completed, the contractor will begin by degreasing and scrubbing the floors with an approved cleaner/degreaser.

2.1.2 Phase II Once floor has been sufficiently cleaned, the epoxy coating removal process will begin. The contractor will do so with a combination industrial floor grinder and HEPA vacuum system (to minimize dust). Items, materials and tools will not be provided by the government. The stationary items will continue to be covered and taped by the contractor as necessary to prevent residual dust and debris from contaminating equipment.

2.1.3 Phase III After the floor has been cleared of dust, no less than two coats of a durable, water-based epoxy floor coating shall be applied. The epoxy coating must be of a sufficient stature for either industrial or commercial environments. The final flooring shall be chemical and solvent resistant.

a. New resin flooring shall be applied to provide sufficient coating and thickness and shall be coped and or angled to allow to be applied at the base of the walls and to extend approximately 4-6 inches to the current height of the old epoxy floor covering.

b. The contractor shall apply a heavier non-slip epoxy resin at all door entrances that shall be able to be mopped and cleaned by government and military personnel.

2.2 Site Visit A site visit will be held for contractors to physically evaluate the current condition of the floor and to take appropriate measurements. The site visit will take place after solicitation and before award. A site visit is recommended for all bidders submitting a proposal. Failure to visit the work site shall in no way relieve the contractor from successfully performing the required work at the award contract price. Additionally, attendees are not guaranteed a contract award for attendance.

2.3 Warranties The life expectancy of the final installed product should be expected to last greater than 10 years but not less than 5 years. The warranty shall being immediately upon the date of government acceptance of the invoice. As a deliverable on the contract, the contractor shall provide a readily available contact (phone, email, website etc.) the government may use to contact the contractor in the event of a warranty call within the five years window. The contractor shall be required to respond to the warranty call within a 24 hour timeframe to schedule warranty work. Correction of all defects under this warranty shall be at the expense of the contractor.

3. SUPERVISION AND COMMUNICATION: At all times during the performance, the Contractor's

Superintendent/Foreman/Program Manager (as applicable) shall be available by cellular phone. Prior to beginning any work, the Contractor shall supply SMSgt Wayne White of the 433 MXS/AW with the name and telephone number of the designated person. The designated individual must display competence by meeting all applicable OSHA training guidelines as well as display the ability of making decisions and acting on behalf of the awarded contractor. The designated individual shall be physically located on site at all times during performance of the contract. If the designated individual is absent from the job site for an extended period of time, SMSgt Wayne White of the 433 MXS/AW remains the right to dismiss Contractor/sub-contractor personnel off the job until the designated individual returns.

3.1 Upon award, the contractor shall identify all employees that will be performing services under this contract. This includes sub-contractors, subcontractor employees, suppliers and delivery personnel entering the JBSA Lackland area.

3.2 Contractor's employees should be knowledgeable of standards associated with this type work.

4. QUALITY ASSURANCE: The government will evaluate the contractor’s performance during the various phases.

Quality assurance may be performed randomly by 502d Civil Engineering Squadron, 502d Wing Safety, and/or the contracting officer and representatives. They may observe performance to monitor the quality of work, ensure cleanliness and compliance with safety and security procedures. At the time of award, the contractor shall designate a program manager who will be available throughout the project to discuss with the government inspectors any quality issues that may arise.

GENERAL INFORMATION

5. UTILITIES: The government will provide all utilities (water and electricity).

6. SERVICES Emergency, fire and security police services are available as needed:

a. JBSA Lackland Fire Department shall provide initial response to include EMS and fire protection services to the contractor in the event of an emergency. 911 or 925-8101

b. JBSA Lackland Police/Security 911 or 671-2018

7. TRAFFIC REGULATIONS: Contractor employees shall comply with all base traffic regulations, and are subject to random vehicle speed control checks. Contractor’s personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones, except hands-free phones, is prohibited while driving on military installations in San Antonio, TX.

8. HOURS OF WORK: Normal duty hours are from 6:00 AM to 11:00 PM, Monday thru Friday, except on government holidays. . Work shall be performed during normal work hours between 6am and 6pm Monday thru Friday. Contractor shall coordinate work with 433AW/MXS: Wayne White 210-925-7381 or 210-336-7740.

9. PERIOD OF PERFORMANCE: The contractor will complete work within 30 calendar days during the specified working hours until full description of work is performed and completed.

10. CONTRACTOR FURNISHED MATERIALS: All labor, materials, equipment to perform the job. When "furnish", "provide", "install", or similar term is used it shall mean a complete installation, ready for use.

11. SECURITY AND CLEAN-UP: All areas shall be cleaned of debris at the end of each working day and prior to final acceptance of the work. The Contractor shall be responsible for the removal of all residual equipment, components, materials, and/or debris generated. This is to include, but not limited to any and all hazardous material.

12. SMOKING POLICY: The Contractor shall not allow smoking or vaping in any building. The Contractor shall ensure employees smoke only in designated outdoor areas, 50 feet away from buildings.

13. PARKING AND TRAFFIC REGULATIONS: The contractors employees shall comply with all base traffic regulations and are subject to random vehicle speed control checks. Contractors personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from base or other administrative actions. The use of cellular devices (except for hand free) is prohibited while driving on the military installation. Drivers should be particularly concerned with pedestrian traffic. Yield to pedestrians in crosswalks. Posted speed limits and all other traffic controls are to be observed by operators at all times. Seat belt use is mandatory on the JBSA Lackland grounds.

14. CODES AND REGULATIONS: All work shall be accomplished in accordance with the Air Force Base Regulation

AFI 91-203, and OSHA Requirements 29 CFR 1910, 29 CFR 1919, and 29 CFR 1926. These references are attached separately.
