INDIAN HEALTH SERVICE OKLAHOMA AREA

(1) Statement of Need.

The purpose of this contract is to provide sophisticated and multi-disciplinary evaluations and treatment planning for adult substance abusers seen in the Lawton Indian Hospital and Tribal health delivery system with complex conditions which require more extensive evaluation and treatment planning than is routinely provided. These would normally be clients who are not easily diagnosed and for whom treatment planning may be especially difficult. Appropriate evaluations of these individuals will require the services of a highly specialized interdisciplinary team skilled in entirely assessing the client for social and psychological problems, which may co-exist with his/her substance abuse problem. Core disciplines, which will be involved in this assessment and treatment planning process, will include psychiatry, psychology, nursing, social work, drug and alcohol counseling and nutrition.

b. Background Statement.

The IHS has a need for inpatient evaluation/treatment services to be provided to adult substance abusers (18 years and over) within the Lawton Indian Hospital.

The contracting officer will be Ronda Longbrake, Chief Contracting Officer, Oklahoma City Area Indian Health Service, 405-591-3999 and the Project Officer will be CDR Karen Hearod, Director, Behavioral Health, Oklahoma City Area Indian Health Service, 405-951-6016.

c. Statement of Work

The contractor shall furnish the facilities, personnel and other requirements to provide multi-disciplinary evaluations, team staffing and treatment planning for the patients referred by the Lawton Indian Hospital.

Upon consultation with the admitting Contractor’s representative the authorized approving Indian Health Service official will formally approve up to twenty-eight (28) days duration of inpatient services. The maximum overall stay will not exceed twenty-eight (28) days. The Government shall assume no responsibility for additional days of stay unless specifically pre-authorized. The Contractor shall be responsible to assure compliance with this provision.

At no time may the contractor directly bill or attempt to bill an Indian Health Service patient who has been referred by Indian Health Service.

The clinics are controlled facilities in which we comply with Homeland Security Presidential Directive (HSPD)-12.

Section 508 (of the Rehabilitation Act of 1973 [29 United States Code (U.S.C) 294(d)], as amended by the Workforce Investment Act of 1998) does not apply.

Upon discharge of the patient, either completion of treatment or if the patient leaves AMA, the program will fax a “discharge statement” to the referring program which will include a diagnosis, medication list and any follow up plans/recommendations.

A full discharge summary by the doctor will follow the “discharge statement” not more than thirty (30) days after discharge.

All information generated by the performance of this contract will remain the sole property of the Indian Health Service, Lawton Indian Hospital, Behavioral Health Department.

The Contractor shall be required to provide an original and two (2) copies of invoices to Oklahoma City Area Indian Health Services, Office of Finance and Accounting, 701 Market Drive, Oklahoma City, Oklahoma 73114.

Each invoice shall include the following information:

(1) Contractor’s name, tax identification number and Invoice Date;

(2) Contract Number or other Authorization for delivery of services;

(3) Description, price and quantity of services actually rendered;

(4) Payment terms;

(5) Other substantiating documentation or information as required by the contract; and

(6) Name, title, phone number, and complete mailing address of official to whom payment is to be sent.

Invoices can be submitted monthly.
