Questions and Answers

RFQ 246-20-Q-0027

1. The RFQ SOW identifies Hologic Selenia 9000 2D/3D or equal. The Hologic 9000 is no longer available and has been replaced by one of two systems. Hologic 3Dimensions. What model would the alternate be?

-To our knowledge the Hologic Selenia 9000 is available. An alternate would have equal specs to the Hologic Selenia 9000

2. Would it be possible to clarify any requirements that may be necessary other than electrical and/or modifications needed to be completed in the existing mammography room prior to install? Will cosmetic repairs from the removal of the trade-in system be completed by IHS Facilities Maintenance personnel? Example: Repairs such as floor repair or paint touch up where existing mammography system/panels may be removed and/or relocated?

-Pre-Award requirement clarification shall not be accepted after the due date for questions. A site visit was scheduled and a due date for questions was stated in solicitation notice. Post- Award: A request for clarification of contract documents will be allowed after award prior to install.

-Cosmetic damage caused by the Contractor shall be the responsibility of the Contractor to repair. Cosmetic repairs such as the example in question will be the responsibility of Wewoka staff.

3. Do you require the Contractor to use a specific Physicist? Or, are we allowed to utilize one of our commonly utilized Certified Physicists? If required to utilize a specific Physicist, it will be necessary for that Physicist contact information to be provided so that we may request his/her Acceptance Survey fee.

-No, you can use your normal person if you choose to as long as they are certified.

4. In one RFQ announcement paragraph “small business” is specified, and in a second location “SDVOSB” is specified. I am assuming consideration for Buy Indian Set-Aside is not being considered?

-This solicitation is set-aside 100% for a SDVOSB.

5. We are respectfully requesting an opportunity to schedule a time to conduct a site visit in order to evaluate the existing room to determine what if any renovations may be required.

-Due to patient and other project schedules we are unable to schedule a second site visit. We will upload a drawing and/or photos of the room

6. Are you okay with a refurbished unit with a 2018 detector?

-Unit shall be brand new. No refurbished items

7. Please explain DRX Accident Protection plan for one year or Equal -This is a typo disregard. We will amend the Statement of Work to reflect the deletion.

8. All hardware associated with this equipment shall be compatible with the current Carestream Version 12 Picture Archiving Communication System (PACS) that is currently being used. We need to speak to your PACS Admin to see if your existing PACS can support "BTO images" If it does not, no matter with 3D unit they get it won't be compatible.

-Our Current PACS system can support “BTO Images”.
