The CDC Arthritis Program (www.cdc.gov/arthritis) is located in the State Support, Arthritis, Epilepsy, & Quality Of Life Branch (SSAEQLB), Division of Adult and Community Health, National Center for Chronic Disease Prevention and Health Promotion, CDC. Arthritis is the most common cause of disability in the United States and the CDC Arthritis Program was created in 1999 with the mission of decreasing the burden of arthritis among Americans. The 1999 National Arthritis Action Plan: A Public Health Strategy (NAAP) (http://www.arthritis.org/naap.php), created by the Arthritis Foundation, the Association of State and Territorial Health Officials, and CDC, lays out a broad strategic vision for addressing arthritis as a public health problem. The NAAP helped to attract first-ever funding for arthritis at CDC, allowing activities to expand from a part-time epidemiologist in 1998 to a staff of 15, and programs in 12 state health departments. In 2010, CDC and the Arthritis Foundation released the National Public Health Agenda for Osteoarthritis http://www.arthritis.org/naap.php which provides updated recommendations on how to prevent and decrease the impact of osteoarthritis arthritis among Americans. In 2012, the Institute of Medicine released “Living Well with Chronic Illness” which provided recommendations on public health strategies to increase the quality of life of people with multiple chronic conditions including arthritis. The CDC Arthritis Program conducts and supports numerous activities to improve the public health science base to improve the quality of life among people affected by arthritis. These activities encompass surveillance and extramural research efforts and are consistent with those recommended in the 2012 Institute of Medicine Report and the Health and Human Services Initiative on Multiple Chronic Conditions.

To maximize the impact of CDCs intramural and extramural research activities, CDC has identified Winnable Battles which are specific target areas for public health systems (http://www.cdc.gov/WinnableBattles/). The CDC Arthritis Program

PROJECT OBJECTIVE

1. BASE YEAR

a. Estimate national and state specific medical expenditures and earnings losses attributable to anxiety and depression among people with arthritis

A 2012 report by the CDC Arthritis Program demonstrated that anxiety and depression are very common among people with arthritis. Recognizing that mental health conditions are among the leading causes of disability among US adults, preventable medical costs and earnings losses among people with arthritis attributable to these conditions are likely substantial. Results from this analysis will indicate the preventable costs among people with arthritis with the treatment of these conditions. This is highly relevant to the programmatic activities of the CDC Arthritis program because two of the self-management strategies, physical activity and self-management education, are evidence-based strategies for treating anxiety and depression.

1. Conduct analyses of Medical Expenditure Panel Survey and Behavioral Risk Factor Surveillance Survey to estimate the national and state-specific medical costs and earnings losses attributable to each of anxiety and depression among people with arthritis.

2. Prepare manuscript summarizing study findings. Distribute manuscript to coauthors for feedback and revisions.

3. Submit manuscript to a peer reviewed journal for publication and prepare manuscript responses and revisions. Review manuscript proofs and conduct administrative activities necessary for publication of manuscript

4. Prepare a written summary of project results for dissemination which will include CDC Arthritis Program website

b. Stigma and mental health among people with arthritis

A 2012 report by the CDC Arthritis Program demonstrated that only half of US adults with anxiety and depression seek medical treatment. This result is consistent with evidence from other studies that highly treatable mental health conditions such as depression are under recognized by health care providers. The 2012 Behavioral Risk Factor Surveillance Survey included a series of questions on stigma related to mental health and this analysis will examine the potential role of stigma as a barrier to seeking mental health treatment among people with arthritis. Results from this analysis could be used to illustrate the importance of early detection of mental health issues among people with arthritis.

1. Conduct analyses of Behavioral and Risk Factor Surveillance to characterize prevalence of mental health stigma and characteristics associated with mental health stigma among people with arthritis.

2. Submit manuscript to a peer reviewed journal for publication and prepare manuscript responses and revisions. Review manuscript proofs and conduct administrative activities necessary for publication of manuscript

3. Prepare a written summary of project results for dissemination which will include CDC Arthritis Program website

c. Estimating barriers to mental health care treatment among people with arthritis

As indicated for deliverable #4a.1-4, a 2012 report by the CDC Arthritis Program demonstrated that only half of US adults with anxiety and depression seek medical treatment. The 2011 National Health Interview Survey included an extensive series of questions on barriers to mental health treatment. This activity will examine barriers to mental health care treatment; because the content of the National Health Interview Survey questions differs from that in the 2012 Behavioral Risk Factor Surveillance Survey, activities #4c.1-3 will provide a comprehensive perspective on potential barriers to mental health treatment.

1. Conduct analyses of National Health Interview Survey to determine the prevalence of mental health care treatment and identify characteristic s associated with not seeking care

2. Prepare and submit manuscript to a peer reviewed journal for publication and prepare manuscript responses and revisions. Review manuscript proofs and conduct administrative activities necessary for publication of manuscript

3. Prepare a written summary of project results for dissemination which will include CDC Arthritis Program website

d. Conduct weekly reviews by telephone with CDC and CDC partners to review results from deliverables and distribute summaries of call.

e. The contractor must provide ongoing SAS programming technical support to arthritis program personnel.

f. The contractor will make three trips to Atlanta, Georgia, during the Base Period to meet with CDC investigators and conduct presentations on results from deliverables. For purposes of the contract, the trips are currently planned for Fall and Winter 2013 and Spring 2014.

Item #1. Purchase of Statistical Programming and Scientific Writing Services.
