Statement of Work

On-Site Shredding Services and Removal of Boxes Updated March 16, 2018

1.0 BACKGROUND

Area Port of Chicago IL requires on site document destruction (shredding) services for two locations, as outlined herein. This service is for the destruction of paper and records in a manner that will render the documents unreadable and non-reconstructable. Data can also include computer disks, tapes, and other storage media. The intent is to fully protect confidential information. Data can be non-sensitive, sensitive and/or confidential.

2.0 SCOPE OF WORK

Contractor will provide complete onsite shredding services up to 3,200 total standard sized file boxes (12 x 14 ¾ x 9 ½) filled with paper correspondence split between two locations quarterly (four times a year). Contractor does all of the manual labor of moving boxes. Contractor will remove each of the cardboard boxes upon completion and provide shredding certificates immediately to the CBP Officer standing by to validate the shredding service.

The contractor shall be capable of providing shredding services as follows:

2.1 Certification: At a minimum, certification by National Association for Information Destruction, Inc. (NAID) is required. A lapse of NAID certification during the term of the contract shall be cause for vendor removal from contract.

2.2 Document Destruction: Paper and records will be shredded in a manner that will render the documents unreadable and non-reconstructable. The shredding equipment must have the capacity to handle the volume of material generated or produced through the course of government business and must safely destroy all materials whether stapled, clipped, bounded, etc. Destroyed documents shall enter the recycling stream. A certificate of destruction must be provided for all jobs.

2.3 Confidentiality: Confidentiality of all information is required. The contractor shall not inspect, view, peruse, copy, or examine any materials or document. In addition, the contractor shall not otherwise disclose, release or communicate any information to any third person, individual, organization or entity. Should any violation or breach of this provision occur, such shall constitute cause for immediate termination of this contact. Furthermore, the Government, at its option, may verbally direct the immediate removal of employees from the contract, upon the occurrence of the above referenced violation or breach. Any resumption of the contractor’s duties will be effected by written communication. The contractor shall maintain confidentiality in accordance with best industry practices in compliance with federal and state laws, and in compliance with the NAID. The Contractor’s personnel designated to work with confidential records may be required to sign a statement of confidentiality guaranteeing non-disclosure of information prior to performing any work described in this contract. The confidentiality statement shall be designed, implemented and maintained by the Contractor. If necessary, the statement of confidentiality shall be made available for viewing by the Contracting Officer.

2.4 Call-In Service Consoles/bins: Request for shredding services shall be arranged between each individual COTR and the Contractor. Scheduling more than one location at a time is permissible. The contact point for each location is outlined herein in order to facilitate scheduling the shredding services. Shredding services shall be performed within seven calendar days after call in. All materials shall be classified as mixed paper without the need to remove staples and paper clips.

Call in services may be cancelled by the Government with 48 hours notice. The contractor may reschedule shredding services with 48 hours notice.

2.5 Mobile On-Site Shredding Equipment. For on-site mobile shredding services, the vehicle(s) shall have lockable cabs and have fully enclosed and lockable refuse boxes. The vehicles shall remain locked when unattended. The vehicle shall be clearly marked with the corporate logo and/or name. .

2.6 Contractor Personnel 2.6.1 Employee Criminal History Background Check: The contractor shall, at this/her expense request a criminal background check for each employee assigned to perform services under this contractor. This shall also include any subcontractor employees. The written results of this check must be maintained on file by the Contractor. If necessary, the background check shall be made available for viewing by the Government. Any prospective employee convicted of a felony or any type misdemeanor involving money, fraud, deceit or untruthfulness within then (10) years prior to their proposed start date will not be allowed to perform services under this contract. All employees under this contract must complete a successful background investigation prior to performing services.

2.6.2 Uniform. All employees of the contractor shall wear on their person a photographic ID badge, to be supplied and furnished by the contractor. The employee ID badge shall contain the company name, address, telephone, the employee’s full name, printed on the badge and employee’s signature. A recent photograph of the employee, not more than one year old.

3.0

CONTRACT TYPE / MINIMUM & MAXIMUM

This is a firm and fixed price contract.

4.0

PERIOD OF PERFORMANCE

The term for this Contract shall be for one year with option to renew for four additional one year periods.

5.0

PLACEMENT OF ORDERS

Each location shall arrange for shredding of documents quarterly. Orders will include the following documentation:

Date of Order

Purchase Order Number

Location where shredding is to be performed

Quantity of containers to be shredded

Quantity of additional boxes/bags to be shredded / w/estimated lbs.

6.0

LOCATION FOR DELIVERIES – QUANTITY OF CONTAINERS

Hours of services for each location is Monday through Friday; 0800 – 1600.

DESCRIPTION OF DELIVERABLES

Each location shall contact the contractor to arrange for shredding services. The contractor shall have seven calendar days to shred documents.

8.0

GENERAL INFORMATION

8.1 Disclosure of Information

8.1.1.

Any information made available to the contractor by the government or its customers shall be used only for the purpose of carrying out the provisions of this contract. This information shall not be divulged or made known in any manner to any persons except as may be necessary in the performance of the contract.

8.1.2 In the performance of this contract, the contractor assumes responsibility for the protection of the confidentiality of government records.

8.1.3 The contractor will adhere to the requirements found in Part 24 of the Federal Acquisition Regulation, Protection of Privacy and Freedom of Information.

8.2 Technical Contact 8.2.1 The Customs and Border Protection technical contact person or Contracting Officer’s Technical Representative (COTR) will be Tamira George. The COTR may be contacted at 847-928-5131 or Tamira.George@dhs.gov.

8.4 Procedures for Payment

8.4.1 Billing and Payment shall be accomplished by Government Purchase Order.

8,4.2 IPP – Invoices must be processed through www.IPP.gov. Registration in IPP must be made prior to award.
