About this file

This document is a Deviations Form, which requires vendors to disclose any intended deviations from the specified project requirements. The form mandates that if a vendor plans to use different materials, items, treatments, finishes, tailoring details, or construction methods that differ from the standards listed in the specifications, they must explicitly list those deviations. Failure to list such deviations in detail will result in disqualification of the proposal or bid.

The form provides spaces for the vendor to print their name, provide an authorized signature, date, contact information (address, city/state, zip code, phone, and fax), but does not contain any specific project details or deviations. The key requirement is complete transparency about any proposed variations from the original specifications, with a clear warning that omitting such information will lead to automatic disqualification of the bid.
