B-1 PURPOSE:
This is a Firm-Fixed Price type contract for Digital Radiographic Imagining Systems and Technical Support Services for the Gallup Service Unit, Gallup, New Mexico.
The project includes removal, disposal of the old system and installation of the new Digital Radiographic Imaging Systems, on-site training (initial and ongoing), technical support, warranties, software upgrades, and service agreements. Contractor shall provide Digital Radiographic Imaging Systems that are turnkey operational, whereby the Contractor is expected to supply the equipment and construction. The construction work shall be considered 100% complete based on the final product performance.
B-2 FEE SCHEDULE:
The total cost shall be inclusive to include all personnel, supplies, equipment, labor, supervision and all other costs associated with the removal, disposal of the existing radiology equipment and installation of the new radiology equipment in the required areas of Gallup Indian Medical Center (GIMC) and Tohatchi Health Center (THC). The Contractor shall provide an itemized listing of equipment, accessories/supplies, installation, training, preventative maintenance, and applicable taxes for each contract line item (CLIN) below, in accordance with the specifications and requirements identified in Section B-7. CLIN is listed in the level of priority for delivery and installation. Davis-Bacon prevailing wage requirements apply to the turnkey construction.

4.1 Acceptance: Constitutes acknowledgement that the supplies or services conform to the applicable contract quality and quantity requirements, except as provided in FAR subpart 46.5 and subject to other terms and conditions of the contract.
4.2 Approval: Acknowledgment by the designated Government official that submittals, deliverables, or administrative documents (e.g., insurance certificates, installation schedules, planned utility interruptions, etc.)
conform to the contractual requirements. Government approval does not relieve the Contractor from responsibility for compliance with contract requirements.
4.3 Contracting Officer (CO): A Government employee with the authority to enter into, administer, and/or terminate a contract and make related determinations and findings.
4.4 Contracting Officer’s Representative (COR): A Government employee responsible for administration of this contract. The COR closely monitors the contract for Contractor compliance and provides feedback to the Contracting Officer.
4.5 Contractor: The organization awarded a legal binding contract to provide supplies and services.
4.6 Contractor Performance Assessment Report (CPAR): A report that assesses a contractor’s performance and provides a record, both positive and negative, on a given contract for specific period of time.
4.7 Federal Acquisition Regulation (FAR): The FAR is the primary regulation for use by all Federal Executive agencies in their acquisition of supplies and services with appropriated funds.
4.8 Gantt Chart – Illustrates the start and finish dates of the terminal elements and summary elements of a project.
4.9 Health Center: A facility physically separated from a hospital, with a full range of ambulatory services including at least primary care providers, nursing, laboratory, and x-ray which are available at least 40 hours a week for outpatient care.
4.10 Original Equipment Manufacturer: OEM
4.11 Ordering Activity: An authorized user of IHS that may issue a task order to obtain required services under this contract.
4.12 Ordering Activity Contracting Officer: A Government employee of IHS authorized and warranted to issue task orders and to make subsequent task order modification(s) under this contract. The Ordering Activity CO has the authority to make initial determinations on all matters of dispute regarding task orders.
4.13 Picture Archiving and Communication System (PACS): is a medical imaging technology which provides economical storage and convenient access to images from multiple modalities (source machine type).
4.14 Personal Protective Equipment (PPE): The equipment used to protect medical personnel from exposure to biological, chemical, and radioactive hazards.

4.17 Source to Image Distance (SID) - is the distance from the x-ray tube to the image receptor.
4.18 Turnkey: when a vendor is responsible for providing both the equipment and construction services required to make imaging equipment and space fully functional. This incorporates renovations to the rooms such as:
construction of walls; electrical power runs; additional cooling; lighting; flooring; wall finishes; ceiling tiles; and other work needed so the equipment and space is ready to be used by the Government.
B-5 GOVERNMENT FURNISHED INFORMATION AND SERVICES: Government unique information related to this requirement, which is necessary for Contractor performance, will be made available to the Contractor. The Contracting Officer or designee will be the point of contact for identification of any required information to be supplied by the Government.
GIMC and THC will provide access to the service area as well as sufficient power required for a complete and useable project.
B-6 CONTRACTOR FURNISHED PROPERTY: The Contractor shall furnish its own equipment for the performance of the job. For security reasons, all Contractor equipment shall be identified with a company label or logo to show proof of ownership. It is standard practice for security personnel to challenge visitors to show proof of ownership of their equipment. The Contractor shall ensure security policies are followed and met.
B-7 STATEMENT OF WORK:
7.0 GENERAL: The scope for the Digital Radiographic Imaging Systems includes all personnel, supplies, equipment, labor, supervision and all other costs associated for the successful removal and disposal of the old radiographic systems and the installation and performance of the new digital radiographic systems in the required area of the Radiology Department at Gallup Indian Medical Center and Tohatchi Health Center.
Background: IHS is an agency within the U.S. Department of Health and Human Services and is responsible for providing federal health services to American Indians and Alaska Natives. The provision of health services to members of federally recognized tribes grew out of the special government to government relationship between the federal government and Indian tribes. The IHS is the principal federal health care provider and health advocate for the Indian people. The goal is to assure that comprehensive, culturally acceptable personal and public health services are available and accessible to American Indians and Alaska Natives. The IHS currently provides health services to approximately 1.5 million American Indians and Alaska Natives who belong to more than 557 federally recognized tribes in 34 states.
Navajo Area Indian Health Service (NAIHS) administers health centers and hospitals providing health care to members of the Navajo Nation. The Navajo Nation is the largest Indian tribe in the United States and has the largest reservation, which encompasses more than 25,516 square miles in northern Arizona, western New Mexico, and southern Utah, with three satellite communities in central New Mexico. The NAIHS is the primary provider of inpatient, ambulatory care, preventive and community health, and environmental health services for members of the Navajo Nation and the San Juan Southern Paiute Tribe.
Gallup Service Unit (GSU) consists of the Gallup Indian Medical Center and Tohatchi Health Center.
Gallup Indian Medical Center (GIMC) is a 74-bed hospital in Gallup, New Mexico, on the border of the Navajo Reservation. Clinical specialties at GIMC include Internal Medicine, Cardiology, Family Medicine, 8 | P a g e
Dental, Anesthesia, OB/GYN, General Surgery, Orthopedics, ENT, Optometry, Radiology, Pathology, Pediatrics, Physical Therapy, Psychiatry, Emergency Medicine.. The workload at Gallup is one of the largest in the Indian Health Service with 350,000 outpatient encounters and 3,300 inpatient admissions annually. GIMC has the largest staff of all Navajo Area IHS facilities.
Tohatchi Health Center (THC) is located in the Southeastern portion of the Navajo Nation approximately 40 miles North of Gallup, New Mexico. Clinical specialties at THC include Family Medicine, Eye Clinic, Dental, Physical Therapy and Pharmacy. The workload at Tohatchi consists of 20,000 outpatient encounters annually. Inpatient needs are met by referral to the Gallup Indian Medical Center.
7.1 PERFORMANCE/SYSTEM REQUIREMENTS: Contractor shall meet the minimum qualifications and/or requirements set forth within the Request for Proposals and shall be capable of providing all core services set forth in the specifications/system requirements.
GALLUP INDIAN MEDICAL CENTER:
PHASE 1: ROOM 1:
Requirements for Room 1 includes a new system for a general Digital Radiographic and Fluoroscopic imaging procedures with high frequency x-ray generator, ceiling or floor mounted X-Ray tube, and two flat panel detectors for the floor mounted horizontal table which can tilt to vertical position to enable chest imaging. A Universal Uni- Strut Support System installation shall be included in the procurement. The system specifications are:
80kW X-Ray Generator (minimum) Voltage stabilizer or UPS for generator, and UPS for the computer system X-Ray tube and tube assembly Digital Fluoroscopy System (2 removal Flat Panels) with minimal Active Area of 17 inch x 17 inch Remote controlled Fluoroscopy System and Table Main Console Work Station In-Room Work Station.
Floor mounted Horizontal Table Detector Unit and Image Acquisition (Radiography, Fluoroscopy, storage of dynamic series of fluoro images and capability to play back those images as video) Operator’s Console and image Processing System Computer Connectivity with IHS (MOSAIQ), PACS/RPMS Integrated Dose Area Product (DAP) Meter Air Kerma Dose Monitoring Rate Meter to be included in the system.
Two (2) Monitors for the system or one (1) split monitors for capture and live.
Stitching Package shall be included in the system
Infrastructural specifications:
Installation of a ceiling Uni-Strut X-Ray Support System, a Universal Grid or equivalent support to hold overhead X-Ray equipment.
The height of existing shield varies; however, shielding will be in accordance of existing facility to meet the National Council on Radiation Protection and Measurements (NCRP).
X-RAY Generator specifications:
The X-Ray generator to be microprocessor controlled high frequency, with a single generator power of 80kW minimum at 100kV, and 150kVp maximum.

Fluoroscopic Voltage range in increments of 40kV to 125kV
X-RAY Tube and Tube Assembly specifications:
X-Ray tube of rotating anode compatible to the quoted generator, suitable for flat panel detectors producing digital images. Automatic collimators with filtering system to be incorporated.
X-Ray Tube shall meet 43/100kW, Focal Spots of 0.6/1.2HS Anode heat storage capacity of at least 600kHU minimum is preferred.
Collimator shall include Field Coverage – 17 inches X 17 inches at 40 inches SID The system shall have an integrated Dose Area Product (DAP) meter with a minimum of three ION
Chambers. Describe the DAP meter used and its position in the beam path. Describe the method for deriving the patient dose or Effective dose from the DAP meter.
Horizontal Table specifications:
A horizontal table (absorption factor of at least 1.5 mm Aluminum equivalent at 100kVp) with floating tabletop, motorized height and angle adjustment and electromagnetic brakes.
System shall provide a 90/30 Tilting R/F Table, with a Flat Table Top.
Shall have a minimum capacity of 550 pounds (lbs.) or more of patient weight, with full dynamic range.
Footboard shall have a minimum of 440 pounds (lbs.) of patient weight
Table-side Controls for Imaging:
State if table-side has controls to enable the following:
1. Indicate if paging up / down in the examination is possible.
2. Ability to change from single image to slit monitor display is possible.
3. State if marketing images for documentation is available.
4. System shall be able to store and display dynamic fluoroscopy series of images. State if this is possible with a table-side control.
5. Annotation of patient orientation left / right is required. State if this capability is available through a table-side control.
6. State if reduction of frame rate during the series is possible with a table-side control.
7. State if switching to other fluoro modes during the dynamic fluoro series is possible with a table-side control.
8. State any other Table-side controls and indicate their function, and indicate what table accessories comes with the Table such as handgrips, individual shoulder support, headend grip protection, compression band, detachable-adjustable Footboard, etc.
Digital Image Receptor(s) specifications:
The receptor(s) dimension shall accommodate varying patient sizes and shall be a floor-mounted counter balanced or ceiling suspended with all Field of View selection.
State if the unit has separate image receptors that can be removable or interchangeable. A minimum of two (2) receptors are required (wall and table).
State if image receptors can move horizontally.
Detector Unit and Image Acquisition (Radiography and Fluoroscopy) specifications:
A detector of appropriate size is needed, in order to cover the full chest or abdomen anatomy of all patients in a single view.
State the type and number of Flat Panel Detector(s) (i.e. Direct technology, Amorphous Silicon or
Selenium are not acceptable). A minimum dimension of 17 inches X 17 inches is required. State the type of material and Scintillator.

Digital Fluoroscopy:
1. State the mode of digital high-definition fluoroscopy (progressive, interlaced etc.). Provide
Frames per second, Matrix size (pixels), Gray scale resolution (bits/pixel), and what Noise reduction techniques applied. Indicate if digital real-time filtration for image improvement is available (programmable),
2. State if Motion detector is programmable. State if the unit offers storage of the last fluoroscopic image LIH,
3. State if storage of dynamic fluoroscopic image series is possible.
4. State if the duration of storage sequence is variable.
5. State if dose reduction is possible (i.e. with 0.2 mm Cu pre-filtration) during pulsed fluoroscopy.
Describe how many pulse frequencies are available and what are their valves (pulses / sec).
6. State if the system can do on-line subtraction during fluoroscopy for road mapping.
Chest Stand Specifications:
Adjustable and angulates for chest, across gurney and ankles. State the minimum floor to detector distance.
Source to image shall have a distance up to a minimum of 48 inches to a maximum of 72 inches for chest exams.
Scoliosis and long leg examinations, source to image distance shall be appropriate for accurate measurement (i.e. may be 80 inches).
Digital Subtraction Angiography or equivalent:
State if the system can perform on-line subtraction in angiographic examinations.
State if the system provides characteristic logarithmic curve for optimum DSA image quality.
System all display of the maximum / minimum filled image (peak opacification) for iodine or CO2 contrast medium.
State automatic setting for mask image Shifting of the mask image within the scene Shifting of the filled image within the scene State the Pixel shift in x / y direction State the maximum number of pixels offered State the pixel pitch range Anatomic background (Landmark) Entrance Air KERMAS at receptor surface according to RQA5 shall be stated
Operator’s Console, Image Display and Image Processing Unit specifications:
Operator Control Console for image acquisition, display, processing and communication, comprising of monitor, computer, keyboard and all accessories.
The main operator console shall have all system controls available, including generator settings and imaging processing.
The main operator console shall include the following:
1. Flicker-free display for radiography and for Fluoroscopy
2. Progressive scanning
3. Image refresh rate (60 Hz or higher preferred)
4. Paging up/down
5. Dynamic image display
6. Simultaneous display of several images for overview (Indicate max. number of images)
7. Automatic and manual electronic collimations

8. Image zoom, window leveling, image invert and image rotate
9. Exposure Dose Area Product (mGycm2) shall be displayed on digital image
Indicate other possibilities for image display available in the package offered.
The main operator console shall have the following capabilities for Image Processing:
1. Distance measurements, annotations, anatomical view, and patient data entry
2. Manual setting of contrast and brightness can be done on the console. Indicate number of gray scales available for windowing.
3. Edge enhancement shall be available on the console and do dynamic display of the series.
4. Black/white inversion can be done on the console. State if on-line image filtration can be done on the console.
5. Image enlargement (zoom), roaming (Pan), and pixel shift.
The main operator Console shall have the following:
1. Post processing algorithms used to eliminate Artifacts. State if available on the console
Connectivity / Upgradeability specifications:
The system shall be DICOM3 Standard compliant. The DICOM3 conformance statement shall be submitted with the offer.
The system shall provide image Communication through Ethernet Interface in DICOM3 format required to support:
1. Send
2. Print
3. Communicate with the PACS/RPMS
4. Archive
5. Store (Data storage is Government Furnished Responsibility)
6. Query / Retrieve
7. Modality Work List
8. Other DICOM 3 conformance statements
The system offered shall have DICOM modality performed procedure step (MPPS) for sending a message back to IHS / RIS (exam status, patient information).
Image Quality specifications:
The Image Quality of the system should be as high as possible with the lowest possible dose.
The maximum resolution achieved on the monitor, to be stated, in line pairs per millimeter, together with the conditions used (kV, mAs, or exposure (mR) to the skin).
PHASE 2: ROOM 3:
Requirement for Room 3 includes a System for general Digital Radiographic procedures with a high frequency X-Ray generator and overhead tube crane combined with elevating table. The system specifications are:
X-Ray Generator Voltage stabilizer or UPS for generator, and UPS for the computer system X-Ray Tube and tube assembly Floor mounted Horizontal Table Elevating table with 550 pound (lb) patient load capacity, fully dynamic.
Detector Unit and Image Acquisition for Radiography images Operator’s Console and Image Processing System Computer Connectivity with IHS (MOSAIQ), PACS/RPMS Dose Area Product (DAP) Meter

7.2 SYSTEM OPERATIONS, RELIABLITY AND TRAINING OF RADIOGRAPHER, RADIOLOGISTS AND
OEM TRAINING.
• Reliability - The system shall be protected against the loss of acquired images and data. For this reason, the Contractor shall supply an Uninterruptible Power Supply (UPS) with Surge Protector and sufficient capacity to support the computer system/image storage in case of power failure, and guarantee against loss of information. Provide details of the UPS system offered.
Describe the type of training offered for the Radiologist and Technologists. Include the number of days needed for the training. The Government will not accept in-kind contributions.
The Contractor shall provide application specialist for adequate period of time for the training of all Radiology staff and Radiologists (up to two weeks or more, if needed).
Training for Service Engineers: shall include two (2) Service Engineer (Bio-Med) OEM Training to be included in the purchase, with no exception.
• All manuals shall be updated appropriately to reflect each new software update release and implementation phase.
7.3 WARRANTY, MAINTENANCE CONTRACT, MANUFACTURER SUPPORT AND
SOFTWARE/HARDWARE UPDATES AND UPGRADES FOR THE DIGITAL RADIOGRAPHIC IMAGING
SYSTEMS:
No Third Party Vendors to repair and/or service equipment during one (1) year warranty period. Only OEM staff.
Warranty and Service Contract Periods – Verify that the digital radiographic systems offered shall have a minimum of one (1) year full warranty, including service and all spare parts, starting on the date of successful completion of the acceptance testing of the unit by the Medical Physicists for the Navajo Area, in collaboration with the installer.
All updates related to the safety and reliability of the system shall be made available, at no-cost during both the warranty period and during the service contract.
During the warranty, the Contractor shall cover parts (including any vacuum tubes, X-Ray tubes and flat panel detectors), labor, and all associated costs, including damages caused by power failure.
Corrective Maintenance work services during the warranty and maintenance contract duration under this contract shall be available twenty-four (24) hours, seven (7) days a week (24/7).
The X-Ray System shall include remote diagnostic for troubleshooting should the system experience malfunctions.
Manufacturer support, software and hardware updates and upgrades: Operating System shall be supported by the OEM for minimum of seven (7) years and shall include all updates and/or upgrades of software.
State who will be responsible for software up-grade/updates during warranty & when warranty expires, will it be the responsibility of Gallup Service Unit or will the offeror cover the cost.
Offeror shall provide full support guarantee for the useful life of the x-ray unit in writing (10 years).
Offer shall ensure operating system is supported and updated for entire life of x-ray unit.

7.4 CONSTRUCTION PERFORMANCE REQUIREMENTS: The construction contractor shall provide all necessary personnel, supplies, equipment, labor, supervision and all other costs associated for a successful demolition and renovation of three Radiology Rooms located at Gallup Indian Medical Center and Tohatchi Health Center at an estimated time of three weeks per room. Scope of work description listed below.
Demolition and renovation of the radiology room is required before the installation, state the process and the cost. Room 1: Demolish east wall, create a new door way, and close off existing doorway.
General conditions – Contractor shall have a Project Supervisor on-site at all times and clean up after the day is completed.
Contractor shall include an Architectural and Engineering Fees, including an Engineering Stamp.
Demolish existing drop ceiling.
Install a new partition wall with a window opening for the work station which includes a lead lined window system.
Patch & Repair Drywall/Blocking and Backing.
Contractor shall install an overhead grid layout utilizing the Unistrut System that can support a Medical
Support System such as an X-Ray System with a weight capacity of 900 lbs. All rail support and bracing MUST be engineered/designed by a REGISTERED STRUCTUAL ENGINEER. This is required for all 3 rooms.
Install a new countertop for the work station.
Install a new ceiling Grid and Tile.
Shall paint all walls, door, trimming, and ceiling if applicable.
Remove VCT tile and install seamless vinyl flooring.
Sprinkler Head modification is required after a new grid is installed.
HVAC modification is required for the project.
Lighting fixtures, air diffusers, sprinklers and other devices must be recessed for clearance.
Contractor shall include a standard fluorescent dimmer lighting system for the room.
Contractor shall provide 80kW of Electrical Power to supply to system that must include conduits, copper wiring and lighting fixtures to meet/conform to N.E.C. and Local Codes. A detailed drawing is required detailing the electrical work to complete the project.
Contractor shall comply with all applicable OSHA regulations and other regulatory requirements in removal of Lead Paint and Asbestos.
Disposal of all debris as a result of this project is the responsibility of the contractor.
Contractor shall continue to keep surrounding buildings operational at all times during this repair and/or construction period (Work before 8AM and after 5PM is permitted to minimize disturbance to Patients Services).
The Contractor is responsible for conducting a Pre-Construction Meeting before starting with any work including mobilization and provide specific details regarding the date, time, and location of the meeting, any need for attendance by subcontractors, and information regarding the agenda items to be discussed.
Survey: Before any work is started, the Contractor shall make a thorough survey with the Facilities Manager of areas in which alterations occur and areas which are anticipated routes of access and furnish a clear construction schedule with specific start and finish dates for each task.
120 hours (5 Days) prior notice shall be given to Facilities Manager or designee before conducting any work (to coordinate for security badges, parking, and staging areas for equipment).
The contractor shall inform Gallup Maintenance personnel daily of the exact locations of work being performed.
A safety plan is to be presented by the Contractor before the start of the project.

Patients, staff rooms and office windows are to be covered for privacy if necessary. Contractor shall provide temporary safety barriers to protect patient care areas from dust and debris during the installation phase
Contractor shall develop and maintain a comprehensive safety program. All contractors’ personnel onsite must use Personal Protective Equipment (PPE) that meets OSHA requirements.
Contractor shall comply with Hospital’s Interim Life Safety Measure (ILSM) and Infection Control Risk Assessments (ICRA) and while doing business on IHS property. All work areas shall be maintained in a clean and orderly condition with NO piling of demolished materials.

Any scratches or other damages caused during construction of this project must be fixed immediately.
Any major repairs or any shutdowns are to be approved by Facilities Manager.
Train and provide recommendations to Gallup Indian Medical Center and Tohatchi Health Center Staff, on the best solution to continuously maintain buildings in excellent conditions.

B-9 PERIOD OF PERFORMANCE:
Contractor shall deliver and install equipment within 90 calendar days from receipt of contract award.
Prior to installation, the contractor shall assess the conditions of the area with regards to the available power and any other concerns that need to be addressed prior to installation of the new digital radiographic systems
B-10 SCHEDULE: Contractor shall submit a proposed schedule of all work necessary for each phase of 1)
Construction of removal of old equipment, demolition and renovation of room. 2) Installation of new radiographic imaging system for each room. This schedule shall reflect a “rolling” transition so as to ensure maximum monitor coverage at all times. The schedule shall be coordinated with the Government COR or designee.
B-11 HOURS OF OPERATION: Normal hours of operation: 8:00am – 5:00 pm, Monday through Friday, excluding federal holidays. Overtime and holiday pay are not authorized. However, some services may be required to be performed after normal hours of operation. In this case, the contractor shall coordinate times and access to the building with the Contracting Officer Representative or designee.
ALL Contractor personnel shall check-in and check-out with any of the Points of Contact listed in B-12 before and after conducting any work on hospital premises.
