Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID:_36C249-19-AP-1702

1. Contracting Activity: Department of Veterans Affairs, VISN 9, Chaplain Service, James H. Quillen VA

Medical Center, PO Box 4000; Mountain Home, TN 37684. Transaction number: 621-19-3-092-0026.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: Weekly

Mass/Service of Word to be held every Tuesday (unless cancelled or rescheduled). Provide Rite/Sacraments at bedside on a weekly basis, normally on Tuesdays. Provide patient visitation, general visitation of Catholic patients desiring visits from the Catholic Priest on a weekly basis.

Provide Assessment Consultation for patients and residents who are Catholic and present complex religious and spiritual needs. Provide emergency call backs, which occur outside of normal duty hours when a patient or resident is seriously ill or has died and requires presence of a Catholic Priest. Total cost of base period and each of four option periods is $XX,XXX,XXX.XX.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: As Priests of the Roman Catholic Church of the Diocese of Knoxville, TN, St.

Mary's Catholic Church ministers to and serves all the faithful within the boundaries of their parish.

For St. Mary's Roman Catholic Church, this includes all of Washington County in the State of Tennessee. St. Mary's Roman Catholic Church is also the only Roman Catholic Church parish within Washington and surrounding counties within geographical proximity to the Medical Center to allow for timely (within 30 minutes) response for emergency notification.

4. Description of market research conducted and results or statement why it was not conducted: A notice of intent to sole source to Saint Mary’s Catholic Church was posted on the government point of entry (GPE) on May 3, 2019 requesting capability statements from contractors indicating how they could provide said services. All responses to the notice were due May 18, 2019. However, no responses were received. Further, the Department of Veterans Affairs (VA) Vendor Information Page (VIP) was researched under the North American Industry Classification System (NAICS) code 813110, Religious Organizations. The undersigned called all sixteen organizations that certified under said NAICS in VIP. But the sixteen organizations either couldn’t provide the service or did not call the undersigned back after the undersigned call them.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
