Questions and responses for Request For Quote Number 16-250-SOL-00176; New Coding Schemes for Adverse Action and Malpractice Payment Reports –
1) Statement of Work: Schedule of Deliverables On the Schedule of Deliverables (pages 10-12 of the solicitation document) the percentages listed in the “Payment Schedule by percentage of total amount- determined at award” column add up to 105%. Can you please clarify which items are to be included in the payment schedule and their relative percentages? Should the items on our payment schedules be rounded to the nearest dollar or nearest cent?
Response(R): The payment for the Task 9.1 Revise and Submit Draft Final Report, Data Dictionary and Recommendations, has been reduced from 10% to 5%, so the total will add to 100%. All the other payments remain unchanged. Please round the items on our payment schedule to the nearest dollar.
2) List of Attachments On the list of attachments (page 2 of the solicitation document) it lists the Standard Form 1449 as Attachment F; however, the last page of the solicitation which is titled “Attachment F- Standard Form 1449” is blank. Will we be provided with this form for completion of Price Quote item 1(a) (pages 16-17 of the solicitation document). Or are we supposed to fill in the blocks requested on a blank SF-1449 form?
R: A revised Standard Form 1449 has been uploaded. Offerors are required to complete the blocks per the solicitation instructions under the pricing factor.
3) . . .a PAT is required for this solicitation and it is not clear what the Product Name should be for the PAT. We are not quite sure what exact name you’re looking for, please provide any available feedback.
R: Please refer to the instructions in the HHSAR 352.239-74 Electronic and Information Technology Accessibility. The purpose of the clause is to ensure the required deliverables meet the Section 508 requirements. Please make the product name consistent with the title on the technical proposal that is submitted.
4) Our company is considering submitting a response to the above opportunity. One of our criteria for submitting a response is to know if there is an incumbent and if so, who the incumbent may be. Would you be able to answer these questions for us ?
R: There is no incumbent as this is a new requirement. However, similar work in the past was performed by a company named, MSM Data and Analytics, Mesa, Arizona under contract number HHSH250201400012C.
5) Has a contractor performed similar work to this for the government in the past?
If available, please provide the incumbent contract number. If you are unable to provide a contract number, is it safe to assume this is a new requirement for the government?
R: See the response to number four above.
