4. Describe the supply or service to be acquired and the total estimated dollar value:

Air Ambulance Services to transport an inmate from Marianna Airport Authority and an inmate from Tuscaloosa Regional Airport to Fort Worth Me,acham Airport, Fort Worth, Texasfor continued medical treatment at FMC Carswell in addition to three staff. $40,388.00 Quoted Pricing (includes ground ambulance from Jackson Hospital to Marianna Airport Authority. It is determined to be unusual urgency because a delay would result in serious injury, financial, mother liabilities to the Government. The Health Services mission at FCI Marianna includes the care of the inmates. When urgent medical treatment for an inmate (or inmates) arises, Health Services initiates a TIO transfer Request for transfer to an institution that is better equipped to provide treatment that a Care Level 2 institution as is FCI Marianna. Upon approval, Medical Designations issues a Transportation Authorization which determines the time frame in which to transfer the inmate/inmates.

The contractor has previously pro~ided services and was listed on the BPA for Air Charter/Air

Ambulance Services that ·expired on June 3 0, 2018. Solicitation from a single source was necessary due to the urgent and compelling need to transfer two inmates for continuation of treatment.

6. Describe your market research efforts and efforts to ensure that offers are solicited from as many potential sources as is practicable:

A request for quotes was sent via email to five (5) vendors for air charter services. InFlight Medical Services International provided a quote, no other vendors did responded to our request. Our best effort was made to secure quotes from as many sources as is practical while still meeting the transfer requirement without causing delays. Quotes were requested from the following vendors:

The contractor has previously provided services and was listed on the BPA for Air Charter/Air Ambulance Services that expired on June 30, 2018. Pricing is consistant with BPA pricing. We deem this pricing to be fair and reasonable.
