C.1 STATEMENT OF WORK
The Contractor shall furnish all of the necessary personnel, materials, services, facilities, (except as otherwise specified herein), and otherwise do all the things necessary for or incident to the performance of the work as set forth below:
Background The 1994 Amendments to the Social Security Act (SSA) authorizes the U.S. Department of Health and Human Services (DHHS) to review State child and family service programs to ensure conformance with the requirements in titles IV-B and IV-E of the SSA. On January 25, 2000, the DHHS published a final rule in the Federal Register to establish a new approach to reviewing and monitoring State child welfare programs and services. Under the rule, which became effective March 27, 2000, States are reviewed for substantial conformity with certain Federal requirements for child protection, foster care, adoption, family preservation/family support, and independent living services. The Children’s Bureau, part of the Administration for Children and Families (ACF) within DHHS, administers the Child and Family Services Reviews.
The overall goal of the CFSRs is to assist States in improving child welfare services and the outcomes for families and children who receive the services by identifying the strengths and needs within State programs and those areas where technical assistance could lead to program improvements. Other purposes of the review strategy include the following:
• Linking the CFSRs to existing Children’s Bureau Regional Office and States joint planning, technical assistance and program improvement processes;
• Assisting States in becoming more self-evaluating over time;
• Ensuring that Federal funds are spent in accordance with Federal statute, regulation and policy;
• Collecting data that will inform national policy;
• Identifying best practices that can be emulated across the country;
• Identifying State training and technical assistance needs and providing access to assistance needed to implement a program improvement plan (PIP); and
• Providing timely and specific feedback to States that is directly related to program performance and outcomes.
Outcomes are assessed for each State through a Statewide Assessment and an onsite review process. The outcomes and goals are as follows:
• Safety Outcome 1: Children are, first and foremost, protected from abuse and neglect.
• Safety Outcome 2: Children are safely maintained in their homes whenever possible and appropriate.
• Permanency Outcome 1: Children have permanency and stability in their living situations.
• Permanency Outcome 2: The continuity of family relationships and connection is preserved for children.

• Well-Being Outcome 1: Families have enhanced capacity to provide for their children’s needs.
• Well-Being Outcome 2: Children receive appropriate services to meet their educational needs.
• Well-Being Outcome 3: Children receive adequate services to meet their physical and mental/behavioral health needs.
A CFSR team is assigned to each State and consists of individuals representing the Children’s Bureau Central and Regional Offices, which may include non-Federal consultant reviewers and contractors to the CB who are child welfare professionals and subject matter experts. The state child welfare agency staff and its chosen partners are also part of the team.
At the time of this contract award, the Children’s Bureau will be in the third round of CFSRs which started in FY 2015. Prior cycles of reviews were completed in 2001-2004 and 2007-2010.
A contractor is expected to accept transfer of and maintain all work products, information and services provided by the existing contractor and otherwise assure orderly transition and continuity for the CWRP in supporting the CFSRs. A contractor shall need to work in tandem with the incumbent to ensure a successful transition. This includes ensuring that there is a minimal period of recruiting and onboarding of key personnel, including the child welfare contracted specialists, to ensure continued support for the CB’ work with states .The contractor is also expected to be familiar with and support the alignment and coordination of CFSR work with related CB’s initiatives, requirements and monitoring processes, including current and future reviews.
These steps are to be accomplished through the collaboration of the Children’s Bureau Central and Regional Office Staff, and Contractor.
Tasks
Task 1: Orientation Meetings and Progress Conference Calls with the COR and Key Children’s Bureau staff
The Contractor’s Project Director, along with other persons identified by the Contractor as being necessary for the effective operation of this contract, shall meet with the COR and other key Children’s Bureau (CB) staff within two weeks of contract award. The purpose of this meeting shall be to review the project tasks, project timelines, and expected outcomes. Part of the meeting’s agenda shall focus on the importance of the review process as an integral component of the Children’s Bureau’s overall support system. The importance of the Children’s Bureau Regional Office staff and their leadership role throughout the review process shall also be discussed.
Thereafter, The Contractor shall participate in weekly conference calls with the COR and key Children’s Bureau staff to discuss relevant project issues and progress to-date Task 2: Agile Staff to Support the Onsite Portion of the CFSRs

Solicitation # 15-233-SOL-00237 Page 7 of 84 and State Team Training With input from the CB Central Office staff, the Contractor shall maintain, or if needed, develop and implement a plan for conducting targeted outreach to recruit and employ staff who have the ability to support the onsite portion of the reviews in states as a primary matter and, if deemed appropriate by the contractor, can also support the overall goals of the project. The staff must represent a broad and diverse spectrum of knowledge and expertise in public child welfare services and programs to assist State agencies in carrying out the review process and/or Contractor in supporting CB on the review process tasks in this contract.
2.2 Staff for Onsite Portion of Reviews
The contractor shall develop or produce a pool of staff that can be available on an as needed basis to support individual State reviews. The Contractor shall maintain electronic information on the persons who will serve on these reviews, which at a minimum includes: child welfare experience and skills, training provided by the contractor, participation in contract activities and tasks, performance, demographic, availability and contact information.
The Contractor shall develop a system of routine communication with the staff who serves on reviews in consultation with CB, which may include production and distribution of a periodic newsletter about project activities.
The contractor shall develop criteria in consultation with CB for evaluating the performance of staff that are supporting onsite reviews.
The Contractor shall, revise, modify and/or expand the curriculum for training these staff on the CFSRs and their roles in supporting CB and representing the contractor in conducting CFSRs.
The Contractor shall work with the CB to obtain final approval of the training curriculum before delivery.
The Contractor shall provide initial training for all new staff who will serve as review team members on the CFSRs. The contractor may also provide refresher, specialized or ongoing training, as needed and as agreed upon with CB. The training may be in person or use technology as appropriate for the training to be efficient and effective.
2.3 Logistics
The Contractor shall be responsible for any and all logistical arrangements necessary and providing any applicable reimbursement to staff for training, preparations for reviews, and participation in reviews and post-review activities.
Task 3: Provide Child Welfare Specialists to Support Children’s Bureau
3.1 Monitoring Strategies
The Contractor shall provide experienced child welfare specialist staff to support the Children’s Bureau in implementing all aspects of the Children’s Bureau CFSR monitoring strategies including a specific focus on Continuous Quality Improvement progress, Improvement Plans, and other related tasks related to the reviews as assigned.
Solicitation # 15-233-SOL-00237 Page 8 of 84 and State Team Training At least 16 Child welfare specialists shall be assigned to the CB central and regional offices. The specialists shall have expertise in child welfare programs and shall have work experience in Federal, State, or local child welfare programs. Supervision will be shared by a Deputy Project Director, and the Child Welfare Specialist Team Leader.
The Deputy Project Director will manage the administrative tasks related to the Child Welfare Specialists such as hiring, training, subcontractor invoices, and monthly reports. A general work plan for each specialist will be developed by the Deputy Project Director in conjunction with the CFSR Team Lead or Regional Program Manager, as appropriate. Additional mentoring and content-related task management will be provided by the Child Welfare Specialist team leader who will also continue his/her work with States.
The work experience and educational background required for the child welfare specialists are as follows:
Project Manager: Minimum of 10 years of program management experience is preferred, and 12 years of relevant experience in child welfare or related field is required. Candidate must demonstrate an ability to provide guidance, direction and supervision in all required areas. Minimum education of a Bachelor’s Degree (Master’s Degree is preferred) in a related discipline.
Technical Research Specialist I: Minimum of 8-12 years of relevant experience in child welfare or related field. Must have expertise in specialized area and demonstrated record of accomplishments in specialized field. Bachelor’s degree in related discipline but a Master’s Degree is preferred with 10 years of general experience, of which at least 8 years must be specialized.
The Contractor shall provide logistical offsite support, training, editorial services or other expertise available per task 2 to ensure that child welfare specialist staff can complete the following tasks:
3.2 Collaborating
The child welfare specialists shall work with the CB central and regional office staff in CFSR and related CB strategies and efforts. This collaboration may include coordinating and participating with the CB, State child welfare agency staff, CB’s training and technical assistance providers, grantees and others as designated by the CB in CFSR related work. The child welfare specialists shall facilitate and support collaborative meetings and initiatives between key stakeholders, State child welfare systems, and State court systems.
3.3 Training
The child welfare specialists shall participate in CB CFSR and CFSR-related trainings and task groups as directed by the CB. The specialists will assist the CB in developing or revising content for CFSR protocols and trainings and supporting CB implementation of those protocols and trainings.

Solicitation # 15-233-SOL-00237 Page 9 of 84 and State Team Training The child welfare specialists shall participate in program development to improve, revise and re-design of CFSR monitoring strategies. This may include developing new work products such as tools for the regional offices to track monitoring and implementation of CFSR mechanisms. It also may include updating and revising existing work products.
3.5 Technical Assistance
The child welfare specialists shall be involved in analyzing State child welfare programs for cross-cutting themes and technical assistance needs. The child welfare specialists shall also conduct research on promising practices in child welfare and prepare reports, briefings and presentations, as required. Information gathered as a part of State monitoring mechanism and other relevant sources of information shall provide the basis for the analyses and research referenced above.
3.6 Monitoring Oversight
The child welfare specialists shall assist the CB in providing monitoring oversight, development and implementation. The child welfare specialists shall assist States in identifying areas needing improvement, making States aware of technical assistance opportunities, and identifying possible strategies for improvement.
The child welfare specialists shall monitor State progress including those contained in PIP progress reports by identifying strengths and promising practices, analyzing areas needing improvement, providing feedback to the State on whether it is showing progress towards improvement, and achieving PIP goals.
The child welfare specialists shall assist States and the CB in incorporating relevant CFSR information into the State’s Child and Family Service Plan (CFSP) and the Annual Progress and Services Report (APSR) and vice versa
3.7 Recruitment process
When vacancies occur, the Contractor shall recruit, hire and train the child welfare specialists.
The child welfare specialists are considered Key Personnel and are subject to the requirements of
HHSAR 352.270-5 KEY PERSONNEL (JAN 2006) found in Section G.10 below.
3.8 In accordance with current Federal Travel Regulations:
The Contractor shall be responsible for reimbursing the Child Welfare Specialists for all allowable out of pocket expenses for monitoring related travel.
Task 4: Maintain and Update Internal and External Automated Databases and Websites Related to CFSR
4.1. Provide databases. The contractor must maintain, develop or update a database(s) that at a minimum is capable of containing in electronic form the official reports, documents and instruments, submitted by States and to CB and from CB to States related to:
- The review overall, including state schedules and key dates during the review process

- Review participants
- Systemic factors
- Outcomes and national standard performance
- Measurement plans, including relevant information on sample
- Program improvement plans
- Other contextual State information pertaining to the reviews and/or state’s ongoing CQI system and performance, as requested by CB
The databases will contain, at minimum, similar information already collected from prior rounds of review and transferred by prior contractors.
To the extent possible, the contractor shall devise methods to have the above record version information and be searchable, extensible and cross-interfacing to facilitate the exchange of related information to meet CB’s needs for user training, review planning, data analysis, tracking, reporting, and archiving.
The contractor shall create methods to have differing security and/or permission levels to allow varying levels of access to information by the public, CB staff and users consistent with CB needs and government standards of security and confidentiality.
4.2 Provide CFSR Portal.
The contractor must maintain a single secure point of entry portal (website) with variable access by CB and defined users to manage activities related to CFSR data collection, reporting, assessment and analysis. The portal (website) will serve as the gateway for automated CFSR instruments and documents. The portal will be built or transitioned and then maintained on a .gov website consistent with government rules for the agency’s public-facing presence.
The portal (website) must have user-specific access and have functionality that includes: a messaging system, access to reference guidance and announcements, access to State data profile information, capacity to download tools and templates as well as to upload data and reports, links to external sources of information that are relevant for CFSR and ability to perform other CFSR tasks and activities as needed.
In consultation with CB, the contractor shall create and maintain the ability for defined users to input, upload case-level review data and export case-level review data. The portal (website) will be utilized to manage case level data submissions, monitor data quality, track reporting history and provide a database for analysis and reporting of case review results. This data must be assessable to both CB and other defined users.
The contractor shall identify and inform the training and technical assistance needed by and offered to defined users and CB as it relates to the instrument or document. The contractor shall work with CB to determine which parts of the training information developed under the contract will be part of the electronic-training platform.
The portal (website) must provide defined users with reports and view-only access to information about the status of their instruments or other documents.

4.3 Maintain Data Management System
The Contractor shall continue to maintain the CFSR Data Management System (DMS) and any new monitoring databases as needed which allows easy and ongoing access for CB Central and Regional Office staff, state CFSR team members and enables them to track review related activities within and across regions. The DMS includes the CWRP Document Management System, the Dynamic Data Repository, PIPStATS, Court Databases, the E-Training Platform and the Participant database.
4.4 Data Repository
The data repository shall be maintained by the Contractor and include all completed Onsite Review Instruments (OSRI) and Stakeholder Interview Guides (SIG) and successor documents, electronic data or files of those types. It is another component of the data management system and should have the capacity to provide a number of reports to be utilized in writing a specific State’s Final Report and provide aggregated information across all reviews.
Archive all CFSR related electronic and hard copy documents for easy access and/or distribution to CB Central and Regional Office staff.
The Contractor shall continue to ensure that the Children’s Bureau has a comprehensive archive of CFSR related materials, including but not limited to, Statewide Assessments, Data Profiles, Preliminary Assessments, Court Summaries, Policy Submission documents, Final Reports, Program Improvements Plans, PIP Approval Letters, PIP Quarterly Reports, PIP Release Letters, Close-out and Final Determination Letters, Letters after PIP Close –out, as well as, materials about the reviews produced by the Children’s Bureau-funded technical assistance providers on Child Welfare and papers, reports, articles etc. produced by subcontractors.
The Contractor shall forward all relevant materials for posting on the Children’s Bureau website by another contractor.
The Contractor shall continue to produce 508 compliant documents whether from States, such as PIP’s or from the Children’s Bureau, such as revised instruments/procedures manuals for the remaining option years.
4.5 OMS Help Desk
The contractor shall develop or maintain a technical assistance plan to meet the needs of users utilizing the OMS system. This plan will include timely telephone and electronic support options. The help desk will be maintained Monday through Friday.
4.6 Maintain the CFSR Data Warehouse and Performance Dashboard (DWPD) (This task will take place starting during option period 1 and go through the end of the contract) The Contractor shall assume full responsibility for managing and maintaining the DWPD web-based repository. The contractor shall collaborate with federal staff and the developer of the DWPD in the transfer of the entire database (including data profiles and other relevant data), software, computer code, documentation and related materials for all system applications
Solicitation # 15-233-SOL-00237 Page 12 of 84 and State Team Training associated with the data warehouse and performance dashboard, and possibly an optional public dashboard.
The contractor shall participate in training and testing of the dashboards, tools, strategies, schedules and requirements for maintaining the DWPD, and facilitate access for system users to analyze and visualize the data. The contractor shall also be responsible for the implementation of system enhancements and providing technical support to system users.
The contractor shall budget for costs related to maintaining and updating all required licenses.
Task 5: Logistical Support for Monitoring
The Contractor shall arrange program and logistical support for CFSRs and CFSR related work including, but not limited to the following:
Assist the Children’s Bureau Regional and Central Office staff in developing a consistent process for scheduling and facilitating conference calls with States, other CB staff and/or T/TA providers throughout the CFSR conference calls, which will be held with the State. The number of such calls may vary depending on State and CB needs.
Conduct an ongoing analysis of the project support to the reviews, and continue to develop and implement a plan for streamlining and enhancing review procedures and systems during the review process. This will include updating any Guidelines, forms and other materials for use in monitoring strategies.
Develop and update pertinent information for the CFSRs and distribute this information to review team members and other entities upon CB request.
The contractor shall develop ways to enhance the current system for routinely communicating information with staff about review related procedures and findings.
Maintain a Review Planning Package for distribution to States and review team members.
Content of the package will be determined in consultation with CB staff.
Develop electronic updates related to CFSRs for electronic distribution and posting on websites as directed by CB.
Task 6 State Team Training
6.1 Provide Training to States scheduled for review
The Contractor shall provide training to states in preparation for their reviews. The scope of the training may vary as requested by CB and the state and be dependent on the type of review that will be conducted in the state.
Solicitation # 15-233-SOL-00237 Page 13 of 84 and State Team Training The contractor shall contact each State Child Welfare agency on the review schedule to discuss the training purpose, dates, and other logistics necessary to provide training to states scheduled for review in person or via technology.
The following steps will be completed to conduct the training:
• In consultation with the CB the Contractor shall obtain a current list of States scheduled for a review
• The trainings will be concluded at least two weeks prior to each State’s review.
• Determine which, if any, CB Central Office and Regional Office staff will plan to be present to observe or participate in the trainings
• Prepare and submit to the CB standard correspondence to be used in planning the trainings, including but not limited to:
(1) initial letter to the States introducing the training;
(2) follow-up letter which will provide States will more detail about the trainings;
(3) follow-up calls with the State to confirm all training arrangements one week prior to the scheduled training; and
(4) letter to the CB Central and Regional Office staff providing information on an upcoming training in which they are scheduled to participate as attendees to receive and discuss updated information.
Finalize arrangements, send e-mail confirmations of the trainings to all participants, provide training materials and if necessary, ship any supplies to the training site.
Each training session will cover no more than 2 days. The CB will determine with the Contractor how many State staff will be trained. This will average approximately 50 State staff per training.
In addition, each training location will accommodate CB staff to allow them to observe the training in person or remotely.
No travel or per diem will be paid under this contract for training participants.
In coordination with the State, the Contractor shall ascertain if information technology will be necessary to provide training to outlying areas and, if so, will arrange for such (video, Webinar, teleconferencing, etc).
The Contractor shall finalize the training agenda and ensure that it and any other necessary meeting materials are available to each location for the training session.
The Contractor shall analyze training event and participant evaluations and prepare and submit to the CB a report that includes recommendations for changes to the training.
6.2 Training Curriculum
The Contractor shall maintain the training curriculum for State review team members for CB during the reviews, and continually update and refine both the training materials and the interactive computer-based training through the following steps:

• Discuss the training revision process with the CB.
• Work closely with CB staff to continually identify issues or changes that warrant updating the training material, and automated instruments and electronic training.
• Meet with CB staff to discuss the trainings developed for the State training and for the consultant training.
• Develop in coordination with the CB a schedule for making revisions and enhancements to the training.
Based on feedback from the CB, the Contractor shall refine and modify the training curriculum as needed.
6.3 Electronic Training Curriculum
The contractor shall maintain, revise and/or expand the training curriculum. The Contractor shall enhance the curriculum, as needed to train the States on the CFSR and related CFSR processes (related to task 3.3). Ongoing web-based refresher training will be provided for State reviewers and Consultants, and may be made available to others per CB direction.
The Contractor shall maintain the electronic training and develop new training by adding or enhancing introductory materials on the use of the platform to supplement training, history and context of the reviews, preparing for reviews, as well as other key content areas determined by the CB.
The COR and key CB staff will coordinate with the Contractor to determine appropriate refinements to the training curriculum, including the electronic platform.
Task 7: Ongoing Communication and Collaboration
The Contractor shall maintain regular contact and communication with other CB’s contractors and/or partners to exchange information, foster partnership, coordinate, and improve services.
The Contractor shall efficiently engage these partners and create new partnerships as necessary to achieve the goals of this project.
Task 8: Literature Reviews, State Final Reports and other reports and studies related to
CFSRs
The Contractor shall prepare State final reports for the CB that document the state’s strengths, areas needing improvement, determination of substantial conformity or nonconformity in each area reviewed and supported through information obtained from the monitoring strategies, including such information from the state’s final report in prior rounds of review.
The Contractor shall further assist the CB, as required, with editing State final reports and preparing analyses of identified issues and trends resulting from the monitoring strategies.
The Contractor shall provide a cumulative annual analysis of the aggregate findings of child welfare monitoring activities, and provide the Children’s Bureau with an annual report that describes the findings, identifies trends and issues, makes statistical associations and correlations
Solicitation # 15-233-SOL-00237 Page 15 of 84 and State Team Training among and between items/outcomes/systemic factors included in the review, and includes other analyses as needed.
In addition the Contractor shall use rigorous research skills and conduct ongoing analysis of both the quantitative and qualitative data gathered throughout the review process, and/or other relevant data sources as requested by the Children’s Bureau to advance the CB’s understanding of what is needed to improve child welfare outcomes at the national level.
Task 9: Develop, update, edit, format review instruments and materials
In consultation with the COR and other CB staff the Contractor shall assist in developing, revising, formatting and packaging of camera-ready copies of review instruments, forms, procedures manual, etc., as needed. A system of tracking the rationale for revisions shall also be developed and maintained, including providing note takers to capture salient points during relevant meetings. The Contractor shall arrange for selected materials to be posted on the Children’s Bureau website and Information Portal.
Task 10: Generate monthly, annual and periodic reports regarding the CFSRs and related activities.
The Contractor shall develop a periodic reports component and work with the Children’s Bureau to determine the need for collecting additional data or providing additional automated reports
The Contractor shall develop and submit the following types of reports to the CB Central Office and Regional Office Team Leaders, as requested by CB:
10.1 Monthly National Status Reports include:
• Updates on progress of all project related tasks and subtasks;
• Data base development tasks;
• Discussion of any problems encountered or remaining from the prior month, expected approach to resolving problems from the prior month, tasks for the current month, and any budget implications or significant concerns to be addressed by the COR;
• The Contractor shall develop and submit the Monthly National Status Report on the project’s progress to the COR and the Contracting Officer, within 15 days after the end of each month.
• Issues related to the logistical aspects of the review processes;
• Feedback from CB Central and Regional Office staff regarding the quality and effectiveness of review staff
10. 2 Annual Reports:
On an annual basis, the Contractor shall submit a report to the CB and Regional Office staff that summarizes and highlights project activities. Electronic copies of the annual report shall be submitted to the COR and contracting officer within 30 days after the end of each year of the contract and summarize the work that occurs up to 1 month prior to the final year of the contract’s operation.

Task 11Contract Information Transfer The Contractor throughout the year will prepare and be ready to transition and transfer to a subsequent contractor all products and information developed or maintained under this contract to ensure that there is complete continuity of services to the Children’s Bureau and states as of the end of the contract.
C.2 HHS SECTION 508 ACCESSIBILITY STANDARDS NOTICE
(SEPTEMBER 2009)
This contract is subject to Section 508 of the Rehabilitation Act (the Act) of 1973 (29 U.S.C.
794d), as amended by the Workforce Investment Act of 1998, and the Architectural and Transportation Barriers Compliance Board (Access Board) Electronic and Information Accessibility Provisions (36 CFR Part 1194). Section 508 of the Act requires that, unless an exception applies, all communications products and services that require a contractor or consultant to produce content in any format that is specifically intended for publication on, or delivery via, a federally owned or federally funded website permit the following:
(1) Federal employees with disabilities to have access to and use information and data that is comparable to the access and use of information and data by federal employees who are not individuals with disabilities.

C.3 HHS-CONTROLLED FACILITIES AND INFORMATION SYSTEMS
SECURITY
(a) To perform the work specified herein, Contractor personnel are expected to have routine (1) physical access to an HHS-controlled facility; (2) logical access to an HHS-controlled information system; (3) access to sensitive HHS data or information, Solicitation # 15-233-SOL-00237 Page 17 of 84 and State Team Training whether in an HHS-controlled information system or in hard copy; or (4) any combination of circumstances (1) through (3).
(b) To gain routine physical access to an HHS facility, logical access to an HHS-controlled information system, and/or access to sensitive data or information, the Contractor and its employees shall comply with Homeland Security Presidential Directive (HSPD)-12, Policy for a Common Identification Standard for Federal Employees and Contractors; Office of Management and Budget memorandum (M-05-24); and Federal Information Processing Standards Publication (FIPS PUB) Number 201; and with the personal identity verification and investigation procedures contained in the following documents:
(1) HHS Information Security Program Policy.
(2) HHS Office of Security and Drug Testing, Personnel Security/ Suitability Handbook, dated February 1, 2005.
(3) HHS HSPD-12 Policy Document, v. 2.0.

Note: Based upon information provided by the Project Officer, the Contracting Officer shall insert references to OPDIV and/or local procedural guideline(s), if any; indicate if they are readily accessible to the public; and, if so, specify where they may be found. (c) This contract/order will entail the following position sensitivity level(s): --------------------.
Note: At the time of solicitation, based upon information provided by the Project Officer, the Contracting Officer shall specify all known levels. If the position sensitivity levels are not known at that time, the Contracting Officer shall insert the words ``To Be Determined at the Time of Award.'' However, the Contracting Officer must include the definitive position sensitivity levels in the awarded contract/order.
(d) The personnel investigation procedures for Contractor personnel require that the Contractor prepare and submit background check/investigation forms based on the type of investigation required. The minimum Government investigation for a non-sensitive position is a National Agency Check and Inquiries (NACI) with fingerprinting. More restricted positions--i.e., those above non-sensitive, require more extensive documentation and investigation. As part of its proposal, and if the anticipated position sensitivity levels are specified in paragraph (c) above, the Offeror shall notify the Contracting Officer of (1) its proposed personnel who will be subject to a background check/investigation and (2) whether any of its proposed personnel who will work under the contract have previously been the subject of national agency checks or background investigations.
(e) Investigations are expensive and may delay performance, regardless of the outcome of the investigation. Delays associated with rejections and consequent re-investigations may not be

Accordingly, if position sensitivity levels are specified in paragraph (c), the Offeror shall ensure that the employees it proposes for work under this contract have a reasonable chance for approval.
(f) Typically, the Government investigates personnel at no cost to the Contractor. However, multiple investigations for the same position may, at the Contracting Officer's discretion, justify reduction(s) in the contract price of no more than the cost of the additional investigation(s).
(g) The Contractor shall include language similar to this ``HHS-Controlled Facilities and Information Systems Security'' language in all subcontracts that require subcontractor personnel to have the same frequency and duration of (1) physical access to an HHS-controlled facility; (2) logical access to an HHS-controlled information system; (3) access to sensitive HHS data/information, whether in an HHS-controlled information system or in hard copy; or (4) any combination of circumstances (1) through (3).
(h) The Contractor shall direct inquiries, including requests for forms and assistance, to the Contracting Officer or designee.
(i) Within 7 calendar days after the Government's final acceptance of the work under this contract, or upon termination of the contract, the Contractor shall return all identification badges to the Contracting Officer or designee.''

Pursuant to the appropriate inspection clause as provided below, all work described in the statement of work to be delivered under this contract is subject to final inspection and acceptance by an authorized representative of the Government. The authorized representative of the Government is the Government's COR, who is responsible for inspection and acceptance of all services, materials, or supplies to be provided by the Contractor.

E.3 QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)
The Government intends to utilize a Quality Assurance Surveillance Plan (QASP) to monitor the quality of the Contractor’s performance. The oversight provided for in the QASP will help to ensure that service levels reach and maintain the required levels throughout the contract term.
Further, the QASP provides the COR with a proactive way to avoid unacceptable or deficient performance, and surveillance results will be taken into consideration when leaving past performance information for work under this contract in the Contractor Performance Assessment Reporting System (CPARS).
