PAST PERFORMANCE QUESTIONNAIRE
NOTICE TO OFFERORS
Contained herein is the draft letter and questionnaire that the Contracting Officer will use to solicit past performance information from customers that the Offeror has identified in response to the solicitation. This questionnaire may also be submitted for references of key personnel.
As stated in the solicitation, the Contracting Officer, in the interest of expediency, will start collecting past performance information before proposal receipt. Offerors are requested to submit their past performance information in advance of the proposal due date. The past performance questionnaire below shall be used by offerors to send to their previous customers.
The questionnaire includes multiple questions regarding past performance and instructs individuals to return the completed questionnaire to the Government. Failure by offerors to submit past performance information early or by customers to submit completed questionnaires shall not be a cause for rejection of the offeror’s proposal, nor shall it be reflected in the Government's evaluation of the offeror's past performance.
Offerors are encouraged to submit the attached cover letter and questionnaire to their references and request the return of the questionnaire to the Contracting Officer as soon as possible, prior to the established closing date of the solicitation. Offerors shall complete Blocks 1 and 2 of the questionnaire.
