The contractor will be responsible for completion of seven monitor wells installed and completed following requirements outlined herein. Contractor should provide fixed prices for drilling (7) (total depths could range anywhere from a minimum of 75 ft to a maximum of 400 ft.) Pricing should be established at a fixed priced for minimum of 75 ft well plus adjustment pricing PLUS fixed priced per foot for overage depths greater than 75’ up to 400’. Final payment will be based on wells drilled and completed.

SECTION C. DESCRIPTION/SPECIFICATIONS/PERFORMANCE WORK STATEMENT

C.1. Performance Work Statement

General information: This performance-based statement of work (PB-SOW) describes site conditions, anticipated subsurface conditions, drilling requirements, monitoring-well specifications, and other information and requirements for seven monitoring wells to be installed east of the City of Hot Springs.

The precise drilling locations have not been determined. However, the wells will fall within the proposed bypass area shown in Exhibit A, fig. 1 -12b., a corridor of approximately 5 miles in length, and meet all Arkansas Department of Transportation (ArDOT) Permit Requirements, including final ArDOT approval.

Introduction: The ArDOT plans to construct a two-lane divided highway approximately five miles in length, east of the City of Hot Springs, that will traverse the eastern extent of the recharge area for the Hot Springs National Park 52(HOSP) geothermal flow system. In order to provide information regarding potential impacts to shallow wells and the hot springs of HOSP associated with the highway construction, the Arkansas Office of the Lower Mississippi- Gulf Water Science Center will install monitor wells to support research and assessment of potential effects on shallow groundwater from construction activities associated with the proposed bypass, east of Hot Springs, Arkansas.

Background: The monitor wells to be installed will be used to (1) collect hydrogeologic information to characterize and increase the understanding of the shallow groundwater system along the proposed bypass route prior to, during, and following construction activities for a proposed highway near Hot Springs, Arkansas, and (2) identify potential effects of construction on the regional, shallow aquifer system that serves as part of the HOSP thermal springs recharge area. Geophysical logging of wells will be conducted during the drilling and completion process. Travel distances will not be large as well will be drilled within an approximate 5-mile by 1-mile corridor. As highway construction has not been initiated as of yet, travel routes between sites will be indirect, using existing county roads, lease roads, and logging roads. The terrain in the area is steep and rugged; however, drilling locations will be selected near to passable roads.

Scope: This statement of work covers the installation of seven (7) monitoring wells completed in hard-rock sequences, at an average depth of approximately 150 feet for all wells (this will vary from well to well), along the proposed highway corridor (fig 1).

Work Requirements: A USGS geologist will ensure that contractors follow this PB-SOW and meet all requirements. Geophysical logging will be conducted after drilling, but prior to casing installation and completion of the monitoring well. Geophysical logging will take approximately three to six hours, depending on whether the well is cased prior to logging. Fewer logs will be run in cased wells, taking less time. USGS personnel will be on location during drilling, logging, and borehole-completion operations. Drill rig relocation will not be required for geophysical borehole logging.

The contractor is responsible for providing all equipment, materials, and labor required to successfully complete monitoring well installation as described in this PB-SOW. The contractor will collect cuttings samples every five feet of drilling, at minimum. The driller will be responsible for any and all permits required by the State of Arkansas. The driller will conduct all field activities in a safe manner and remove all construction materials after well completion, in accordance with relevant OSHA, Federal, State, and local laws and regulations regarding environmental compliance, safe working conditions, etc. The contractor will document completion, including photos (before and after) with minimum of three different views with sufficient detail to see the entire work location; record and document any waste materials removed and delivered to approved disposal sites.

Technical Requirements (Exhibit B fig. 1): Travel time between all sites should be less than one hour, as all sites will be between Hwy 70 and Hwy 7/5 (approximately a 5-mile distance). Well drilling and completion techniques should be adopted and applied to allow annular space adequate for proper sealing of all casing and emplacement of screen (where necessary),sealing material will be emplaced to provide good hydraulic connectivity with the aquifer and vertical isolation from the surface, and from underlying and overlying formations. The contractor is responsible for providing all monitoring-well materials, including casing, annular seal, grout, cement, protective cover, etc. Materials will be as follows:

a. Monitoring Well Casing and Screen: Initial well-completion design calls for open-hole completion, wherein surface casing will be set from surface to a few feet into bedrock, and the remainder of the bore hole from bottom of surface casing to total depth will be open to the rock formation. Where bedrock is unstable and caving and closure of the borehole is a possibility, such as is common in highly fractured cherts and sandstones, the well will be cased and screened with PVC casing and screen to maintain the bore hole and allow hydraulic communication with target aquifers. Therefore, the driller should have casing and screen available (20-foot screened interval) to complete each boring. Final well casing will be a minimum 4-inch diameter (at least Schedule 40 thickness) PVC.

b. Annular Seal: The annulus (the space between the casing and borehole wall) above the screen and sand interval well in wells where casing is used will be back filled with a suitable approved material such as cuttings or by using a 5% bentonite-cement slurry emplaced using pressure grouting (i.e. through a tremie pipe) to ensure a competent and continuous fill between the casing and borehole wall.

c. Well Packing: Where open-hole completion is not feasible and the well is cased and screened, a coarse, clean sand approved as packing material for wells, should be emplaced across the screened zone. The sand will extend a minimum of 2 feet vertically above and below the screened interval to ensure that bentonite does not contact screen. A minimum of one-foot of bentonite will be emplaced above the sand pack and the annulus fill.

d. Well Completion: A steel protective casing equipped with a lockable cover shall be placed over the well casing.

The top of the casing should be slightly (1 – 3 inches) beneath the top of the protective casing (in the open position) such that a protective cap can be placed over the well casing which does not interfere with the opening or closing of the protective casing.

e. Concrete shall be used to fill the annulus between the inner casing and the protective casing from the top of the backfill up to the land surface, and as part of the same pour, a concrete pad will be poured around the protective casing to secure it. The concrete pad should be a minimum 3 feet square and 4 inches thick, and should slope away from the well protective casing to provide for drainage. A drainage hole of approximately ½-inch diameter shall be drilled into the side of the protective casing within one inch of the top of the concrete pad to drain any water that enters the annular space between the well casing and the protective casing. The annular space between the well casing and the protective casing above the level of the concrete pad shall be filled with clean sand or gravel to prevent insect entry through the weep hole and to prevent loss of items down the annular space. The USGS will provide a lock for the protective casing.

f. Well Development: Within two weeks after well construction is complete, but no sooner than 48 hours after installation of the annular seal is completed, the monitoring well should be developed. Well development shall not include the use of dispersing agents, acids, or explosives. The objectives of well development are:

1. Assure the groundwater enters the well freely, yielding accurate water-level measurement.

2. Remove very fine-grained sediment in the borehole and nearby surrounding formation, so that groundwater is not highly turbid and silting of the well does not occur. Break down any wall cake generated during drilling, remove blockage, and restore good hydraulic connectivity between the well and surrounding formation.

The well will be developed by some combination of pumping at a high rate, bailing, and surging. Development shall continue until water representative of the screened formation is discharged–as indicated by stabilized specific conductance measurements and decreased turbidity. Monitoring of specific conductance will be done by on-site USGS geologist. At a minimum, the volume of water removed should exceed the volume of water lost during drilling and equal to at least 10 times the volume of water in the well bore. Once development is complete, any sediment that entered the well during the development process will be removed from the sediment trap.

Because subsurface conditions in the area will vary, the actual drilling plan for individual wells may be different than what is listed below. However, to maintain uniformity for all potential vendors, the following general, descriptive scenario will be used for proposal purposes.

a) A minimum 6-inch hole will be drilled from land surface to total depth. Total depths could range anywhere from a minimum of 75 ft to a maximum of 400 ft. Total drilling depth will be determined by the on-site USGS geologist and based on drilling to a depth 40 to 60 feet below the depth at which water is intercepted .

b) 4-inch diameter minimum schedule 40 PVC casing will be installed.

c) 4-inch diameter minimum schedule 40 PVC screen, slot size 0.010-inch slot size, will be installed

d) The costs for other items previously mentioned such as installation of protective cover, well development, etc. need to be included in the fixed price.

C.2. Performance Measures and Deliverables:

Work will be considered successful when all materials and equipment have been removed from the drill sites and all debris removed with document completion including site photos(before and after) with minimum of three different views with sufficient detail to see the entire work location; document any waste materials removed and delivery to the approved disposal site.

C.4. GS0342 Hazardous Material Identification March 2008

(a) In accordance with clause FAR 52.223-3, Hazardous Material Identification and Material Safety Data, Alternate I, the contractor shall provide Materials Safety Data Sheets (MSDS) for the following products prior to their use

140G118Q0003 Page 11 of 34 under this contract: adhesives, caulking, sealants, insulating materials, fireproofing or firestopping materials, paints, carpets, floor and wall patching or leveling materials, lubricants, clear finish for wood surfaces, janitorial cleaning products, pesticides, rodenticides, and herbicides.

(b) The Government reserves the right to review such products at any time. The Contracting Officer may eliminate from permitted use under this contract products with significant quantities of toxic, flammable, corrosive or Carcinogenic material and products with potential for harmful chemical emissions.

C5. GS0343 Energy Efficient Products March 2008

(a) The clause FAR 52.223-15, Energy Efficiency in Energy-Consuming Products, applies to any power-consuming equipment to be installed or used in USGS facilities, whether title to the equipment will pass to the Government or BBBbe retained by the Contractor.

(b) For products that consume power in a standby mode (as listed on FEMP’s Standby Power Devices product listing), the contractor shall –

(1) Purchase items which meet FEMP’s standby power wattage recommendation or document the reason for not purchasing such items; or

(2) If FEMP has listed a product without a corresponding wattage recommendation, purchase items which use no more than one watt in their standby power consuming mode. When it is impracticable to meet the one watt requirement, purchase items with the lowest standby wattage practicable.

(c) The requirements of this clause do not apply if (1) no compliant product’s utility and performance can meet the contractor’s need or contract specifications, (2) compliance would not be life-cycle cost effective or (3) compliant solutions are otherwise not available.

C.6. GS0346 Construction Waste Management March 2008

(a) Items and material existing in the space or to be removed from the space during the demolition phase are eligible for reuse in the construction phase of the project. The reuse of items and materials is preferable to recycling them;

however, items considered for reuse must be in refurbishable condition, and must meet the quality standards set forth by the Government in this solicitation/contract. In the absence of definitive quality standards, the contractor is to ensure that the quality of the item(s) in question will meet or exceed accepted industry or trade standards for first quality commercial grade applications. Before use, the contractor shall submit a reuse plan to the Contracting Officer. The Government will not pay for existing fixtures and other existing improvements accepted in place.

(b) Recycling construction waste means providing all services necessary to furnish construction materials or wastes to organizations which will employ these materials or wastes in the production of new materials. Recycling includes required labor and equipment necessary to separate individual materials from the assemblies of which they form a part.

(c) The Offeror shall maintain a recycling program to dispose of or recycle construction waste in compliance with this clause. The program must cover the following requirements.

(1) The contractor will provide continuous facilities for the recycling of incidental construction waste during initial construction.

(2) Products may be excluded from the recycling program when the small quantity of material involved, the extraordinarily complex nature of the waste disposal method, or prohibitive expense for recycling would represent a genuine hardship.

(3) Records of construction materials recycled shall be maintained and accessible to the Contracting Officer.

Records shall include materials recycled or landfilled, quantity, date, and identification of hazardous wastes.

(d) As a minimum, the contractor will recycle lighting and associated support system, carpet and carpet pad, hardware, drywall, and electrical components during both the demolition and construction phases of the project, subject to economic evaluation and feasibility.

(e) If any waste materials encountered during the demolition or construction phase are found to contain lead, asbestos, PCBs (such as fluorescent lamp ballasts), or other harmful substances, they are to be handled and removed in accordance with State and Federal laws and requirements concerning hazardous waste.

(f) The requirements of paragraphs (b) through (d) of this clause do not apply when no construction recycling and salvage operation is reasonably available in the locality where work is to be performed.

E.1. 52.246-12 Inspection - Dismantling, Demolition, or Removal of Improvements (AUG 1996)

(a) Unless otherwise designated by the specifications, all workmanship performed under the contract is subject to Government inspection at all times and places where dismantling or demolition work is being performed. The Contractor shall furnish promptly, and at no increase in contract price, all reasonable facilities, labor, and materials necessary for safe and convenient inspection by the Government. The Government shall perform inspections in a manner that will not unduly delay the work.

(b) The Contractor is responsible for damage to property caused by defective workmanship. The Contractor shall promptly segregate and remove from the premises any unsatisfactory facilities, materials, and equipment used in contract performance, and promptly replace them with satisfactory items. If the Contractor fails to proceed at once in a workmanlike manner with performance of the work or with the correction of defective workmanship, the Government may -

(1) By contract or otherwise, replace the facilities, materials, and equipment or correct the workmanship and charge the cost to the Contractor; and

(2) Terminate for default the Contractor's right to proceed. The Contractor and any surety shall be liable, to the extent specified in the contract for any damage or cost of repair or replacement. (End of clause)

In accordance with the clause FAR 52.232-05, Payments under Fixed-Price Construction Contracts, the contractor shall, within 10 days after receipt of notice of award, submit to the Contracting Officer for approval, a Schedule of Prices. This schedule shall consist of a detailed breakdown of the contract price, segregated under each of the significant construction categories. It shall state the quantities for each of the various types of work, the unit prices, and extended totals. Subcontractors who may be involved in more than one category of work must provide the same breakout data for each category. The submission of the required data shall not otherwise affect the contract terms.

F.8. GS1354 List of Subcontractors (Construction) (July 2001)

For purposes of monitoring compliance with the insurance and payroll reporting requirements of this contract, the contractor shall furnish a listing of all subcontractors to the Contracting Officer within 10 calendars days after contract award.

SECTION G. CONTRACT ADMINISTRATION DATA

G.1. Contract Administration Requirements

G.1.1. The following items must be submitted to the Contracting Officer (mail items to the address provided in your award notice) within 10 days after contract award:

G.1.1.1. Payment protection - Standard Form 25a payment bond preferred (see clause 52.228-13, payment bond only

G.1.2. In accordance with clause 52.211-10, the contractor must commence work under this contract within 10 calendar days after Notice to Proceed, (b) prosecute the work diligently, and (c) complete the entire work ready for use not later than 60 days after Notice to Proceed.

G.1.3. Coordinate the scheduling of all work to be performed and technical questions regarding the project with Technical Liaison. The Technical Liaison will be provided at the time of award.

G.1.4. The contractor is required to submit copies of all payrolls to the Contracting Officer at carrieres@usgs.gov. G.2. DOI ELECTRONIC INVOICING Electronic Invoicing and Payment Requirements - Internet Payment Platform (IPP) (September 2011)

Payment requests must be submitted electronically through the U. S. Department of the Treasury's Internet Payment Platform System (IPP).

"Payment request" means any request for contract financing payment or invoice payment by the Contractor. G.3. Unilateral Deobligation of Unexpended Funds (May 2013)

The contractor shall submit all invoices under the award no later than 90 calendar days after the period of performance has expired, unless a request for extension has been submitted to the Contracting Officer. After 120 days has passed since the expiration of the performance period, the government reserves the right to issue a unilateral modification deobligating any unexpended funds, and to initiate closeout procedures.

G.4. 52.000S-5079 Technical Liaison - Technical Direction October 1997

(a) The performance required herein shall be subject to the technical direction of the Technical Liaison (TL) as identified below. As used herein, "technical direction" is defined as direction to the contractor that fills in details, suggests possible lines of approach, or otherwise supplements the scope of the work set forth herein and shall not constitute a new assignment, and does not supersede or modify any article or clause of this contract.

(b) The Technical Liaison is not authorized to perform, formally or informally, any of the following actions:

(1) Promise, award, agree to award, or execute any contract, contract modification, or notice of intent that changes or may change this contract;

(2) Waive or agree to modification of the delivery schedule;

(3) Make any final decision on any contract matter subject to the Disputes Clause;

(4) Terminate, for any reason, the contractor's right to proceed;

(5) Obligate in any way, the payment of money by the Government. Only a warranted Contracting Officer is authorized to obligate funds on this or any other contract action.

(c) The contractor shall immediately notify the Contracting Officer in writing if the Technical Liaison has taken an action (or fails to take action) or issues direction (written or oral) that the contractor considers to exceed the above limitations.

SECTION H. SPECIAL CONTRACT REQUIREMENTS

H.1. 52.900S-1152 Drilling-Government Representative October 1997

The performance required herein shall be subject to the technical direction of a government representative. As used herein, "technical direction" is defined as direction to the contractor that fills in details, suggests possible lines of approach, or otherwise supplement the scope of the work set forth herein, will not constitute a new assignment, and does not supersede or modify an article or clause of this contract. The government representative will be named at time of award.

H.2. 52.900S-1153 Drilling-Site Prep Restoration October 1997

After completion of all other work described herein and before final acceptance by the government representative, the site shall be restored (to the appropriate state restoration requirement applicable) to its original condition. Such restoration may include, but is not restricted to the following: 1. All equipment belonging to the contractor shall be

140G118Q0003 Page 16 of 34 removed from the site; 2. Holes shall be filled in and depressions and tire ruts smoothed out; 3. Any debris associated with the drilling shall be cleaned up and removed. The work specified in this contract shall not be considered complete, nor shall the final invoice for the work be approved, until site restoration is completed to the satisfaction of the representative.

H.3. 52.900S-1154 Drilling-Permits October 1997

The contractor shall be responsible for obtaining the necessary permits and for the disposal of all water pumped under this contract. The bidder shall be held as having examined the conditions in the field and shall have the responsibility of determining the system(s) to use as well as obtaining the necessary permits for using them.

Potential hazards during period of low temperatures can result from clogging of ditches by ice causing additional water to spread from the drainage ways onto the roadways and then freeze. The contractor shall provide all necessary means to prevent erosion or sedimentation along the ditches and shall maintain the culverts and ditches during the period of discharge to them.

H.4. 52.900S-1155 Drilling-Toilet Facilities October 1997

The contractor shall provide and maintain sanitary toilet facilities for his personnel, which shall be removed upon completion of this contract, leaving the premises clean.

H.5. 52.900S-1156 Drilling-Abandoned Hole October 1997

A satisfactory well that is acceptable under these specifications shall be completed to the depth designated by the government representative. Any hole that does not satisfy the requirements described herein and which cannot be made acceptable shall be declared an abandoned hole. Any hole in which the contractor voluntarily stops work and/or fails to complete in a satisfactory manner shall be considered as abandoned by him. An abandoned well is of no value to the government; therefore, no payment will be made for the drilling, casing, development or supplies of the abandoned well. An abandoned well shall be filled with puddled clay, cement, and/or fine drilling cuttings from the bottom of the hole to the land surface by contractor at his own expense. The contractor shall then move to another government designated location and drill a second hole in accordance with these specifications.

H.6. 52.900S-1157 Drilling-Contractor Representative May 2005

The contractor shall have an experienced foreman or driller, with full authority to act on behalf of the contractor in directing all field operations, constantly in charge of the work at the drill site when work is underway. This representative shall be experienced in cable installation of casing and well development as described herein. This representative must be acceptable to the government representative in the event that he becomes unacceptable to the representative, the contractor shall receive written notice from the government and shall replace him with an acceptable representative within five (5) working days of receipt of the notice.

H.7. 52.900S-1160 Drilling-Drilling Equipment October 1997

The contractor shall maintain all equipment in satisfactory operating condition for the duration of the contract. If, based on the work completed to date the equipment is considered unsatisfactory by the government representative, thus endangering performance under this contract, the representative shall notify the contractor of such in writing, and a copy of such notification shall be sent to the contracting officer for corrective action. The contractor shall then cease work immediately and within ten (10) days of receipt of notice replace the unsatisfactory equipment with equipment approved by the government representative. Acceptance of replacement equipment shall be made only by the government.

H.8. 52.900S-1161 Drilling-Government Liability for Damage October 1997

The government will not be liable for any injury to contractor liability personnel or damage to contractor injury or property unless such injury or damage is due to negligence on the part of the government.

H.9. 52.900S-1162 Drilling-Responsibility for Damage to Contractors Equip. October 1997

The contractor shall assume all responsibility for damage to his equipment whether from elements, vandalism, operation under the contract, or moving to, from or between drilling locations.

H.10. 52.900S-1163 Drilling-Safety and Health October 1997

The contractor shall take all reasonable precautions in the performance of the work under this contract to protect the safety and health of contractor employees, government employees, and the public shall comply with all applicable
