STATEMENT OF WORK

BIE/NMS Jemez Day School and T’siya Day School

Refuse Disposal Services

1. SCOPE OF WORK:

A. Refuse Service Contract:

• The Contractor shall furnish two (2) “Twenty (20) Cubic Yard Dumpsters”, which are to be place side by side at Jemez Day School site.

• The Contractor shall furnish one (1) “Twenty (20) Cubic Yard Dumpster”, which are to be placed at T’siya Day School site.

• Dumping of Dumpsters (trash pickup) is to take place Once a Week from the two school locations. Services are to be performed as follows:

2. SPECIFIC REQUIREMENTS:

A. Pick Up

• Supply two (2) – 20 Cubic Yard Dumpsters.

• Supply one (1) - 20 Cubic Yard Dumpster.

• Solid Waste, to be picked up once (1) a week. Thereof, if pickup date scheduled falls on a day access to premises is denied. Contractor is to contact the Point of Contact (POC) with the Bureau of Indian Education (BIE), Facility Management (FM) to make alternate arrangements for Solid Waste pickup on a different date.

• Driver to notify the BIE FM POC, when notice of large excessive Solid Waste and/or construction debris surrounds dumpster. Facility Management POC to determine if a second pickup needs to be scheduled within the same week. Furthermore, the driver is to notify the POC if he/she should come across unsafe environmental waste that might have been thrown into or against the dumpster. Facility Management POC will make necessary arrangements to have the unsafe material properly disposed of, or to have the overflow of Solid Waste placed back into dumpster.

• Contractor shall take all necessary safety precaution when entering and existing government property.

B. Site Locations:

1. Jemez Day School, 243 Day School Road, Jemez Pueblo, New Mexico 87024, on the Northwest entrance of building #800.

2. T’siya Day School, 1000 Borrego Canyon Road, Zia Pueblo, New Mexico 87053, in the open area between the school Cafeteria and Bus Barn.

3. REQUIRED SUBMITTALS: NONE

4. GENERAL REQUIREMENTS:

a. QUOTE/PROPOSAL: Quotes/Proposals shall be broken down to Lease of equipment, Delivery or Equipment, Collection Service, Fuel Surcharge, Administration Fee, Labor Tax, City Tax, County Tax, and State Tax. The quote/proposal must include an annual renewal cost for the period of one contract year and additional four (4) option years.

b. SITE REGULATIONS: The contractor shall conform to all BIE/BIA Regulations and directive that pertain to security, safety, traffic, fire, and personnel clearance, insofar as they apply to the contractor’s activities.

c. WORKING Hours: Work on this project shall be performed Monday through Friday during normal duty hours of 7:00 am to 3:30 pm, unless other arrangements are made with the BIE Office of Acquisition, and BIE Facility Management POC to deviate from contract specifications.

d. FEDERAL HOLIDAYS: If trash pick up falls on a Federal Holiday, POCs will be contacted so they are aware and arrangements made for trash pick-up day for that particular week.

e. SUBMITTAL REQUIREMENTS: NONE

f. SAFETY: (Refer to regulations specification)

g. ACCESS: Contractor shall always provide access to BIA/BIE Federal Indian Properties (Non- Blockage of roadway access). Contractor shall minimize disruption of services and normal activities expected in such areas, and coordinate street crossings, for minimal traffic disruption.

h. DAMAGES: Where existing surfaces are damaged by performance of work, they shall be repaired and finished to match existing construction. The Contractor shall be responsible for replacement of any items (interior or exterior) damaged during the performance of work to completion of job.

i. CLEANING: The Contractor shall always keep the work area reasonably clean. The Contractor shall properly dispose of all debris at an approved dumping site off BIA/BIE Federal Indian Property, Jemez Day School, 243 Day School Road, Jemez Pueblo, New Mexico 87024, and T’siya Day School, 1000 Borrego Canyon Road, Zia Pueblo, New Mexico 87053.

i. WARRANTY: NONE

j. PERFORMANCE: NONE

k. CHANGES: Any changes to this Statement of Work must be requested in writing with details of what part of the Contract requires Changes, and requests made through the BIE Office of Acquisition and the BIE NMS Facility Management POC identified below. Upon agreement of changes both parties will be notified in writing through Contractor.

l. SALVAGE: All salvageable materials and equipment shall be the property of the contractor unless otherwise noted in the Statement of Work. The contractor is responsible for the removal of all salvageable materials and equipment.

m. OPTION ITEMS: If any option items are listed above, these items must be itemized within the quote. Failure to state in the proposal each of the option item(s) indicated in the SOW may disqualify the quote.

5. PRODUCTS: NONE

6. POINT OF CONTACT:

The BIA/BIE points of contact will be add upon award.
