STATEMENT OF WORK
T’siya Day School
HVAC Control Panel Repair
1. SCOPE OF WORK:
The contractor shall repair HVAC Control Panel at T’siya Day School by repair/rebuilding it.
Contractor will provide all parts, materials, equipment, and labor to make repair. Contractor will remove non-working parts, materials, and dispose of them. After rebuild control panel with new parts and materials, contractor will run test to make sure HVAC Control Panel is back in operating condition.
2. PROJECT LOCATION:
T’siya Day School 1000 Borrego Canyon Road Zia Pueblo, New Mexico 87053 Phone: (505) 867-3553
3. PURPOSE:
The purpose of the project is to rebuild the HVAC Control Panel so the students and staff can learn and work in a comfortable environment.
4. SPECIFIC REQUIREMENTS:
A. SCHOOL SITE VISIT:
Not required.
B. SERVICES:
The contractor shall perform the following:
• Remove bac HVAC Control Panel at T’siya Day School.
• Send to manufacturer to remove and dispose of existing non-working parts.
• Install rebuilt HVAC Control Panel.
• Start up and check operation.
C. GENERAL REQUIREMENTS:
a. QUOTE/PROPOSAL:
Quotes/Proposal shall be itemized by material, supplies, equipment, labor costs, and NM State Taxes.
b. WORK HOURS:
Work hours for this project shall be schedule with school maintenance worker. With safety in mind, contractor prefer to spray before or after school with minimal staff in the building.
c. SAFETY:
The contractor shall adhere to all BIA/BIE adopted codes and the contractor shall abide by all OSHA regulations while on school grounds, and shall review all applicable safety regulations before beginning the project. The contractor shall provide signs, barriers, and or caution tape as a safety precaution around the work site. The contractor shall have a fall protection plan if workers are to be performing duties above six (6) feet from the ground or finished floor. The contractor shall bear sole responsibility of taking all appropriate actions necessary to prevent accidents, injuries to individuals and damage to property in and around the work area.
d. BACKGROUND INVESTIGATION:
All rebuilding done off site. For installation, will have escort of maintenance worker or other staff member.
e. ACCESS:
BIE Facility Management personnel shall provide access to T’siya Day School at all times. The contractor shall minimize disruption of classroom instructions and normal school activities expected in such areas.
f. DAMAGES:
Where existing surfaces are damaged the contractor shall repair and finish with matching existing structure. The contractor shall be responsible for replacement of any items (interior or exterior) damaged during the performance of this project.
g. CLEANING:
The Contractor shall keep the work area reasonably clean at all times. Contractor shall accomplish a complete cleanup at the end of the project. The Contractor shall properly dispose of all debris at an approved dumping site.
h. PERFORMANCE:
Performance time for this project is 30 Calendar Days from Notice to Proceed.
(Exception: Work days maybe interrupted with closures of the Pueblo due to Tribal Traditional Activities). All work shall be completed in a professional manner and without delay once the on-site work is started.
i. SALVAGE:
All salvageable materials and equipment shall be the property of the contractor unless otherwise noted in the Statement of Work and/or drawings. The Contractor is responsible for the removal of all salvageable materials and equipment from the installation.
5. PRODUCTS:
A. WARRANTY:
The Contractor shall provide a written warranty on the material and workmanship from date of acceptance by the Government.
B. MATERIALS:
The Contractor shall provide a written statement noting all material being utilized are asbestos free.
