Southwest Indian Polytechnic Institute Statement of Work
IPM Services Date 7/26/18
Rev. 1/23/2019
1. General Requirements
1.1 Introduction
The Southwestern Indian Polytechnic Institute (SIPI) requests Integrated Pest Management (I PM) servi es to campus buildings, designated communal areas, landscaped areas, maintained grass areas adjacent tooror within the cam us loop road. A full building list is attached as APPENDIX A. Contractor shall provide the services in accordance with this st tement of work (SOW).
Any item(s) of work, inspections, parts and service listed in this statement of work (SOW) shall not be c strued as limiting the contractor to those items but rather as minimum requirements.
1.2 Location
SIPI is located at 9169 Coors Blvd. NW, Albuquerque, NM 87120.
1.3 Performance Time
The base year of performance shall begin on the effective date of the notice to proceed through a 12 mo th period and exercise four (4) option years and may be revised as deemed necessary by the Contracting Officer (CO).
Contractor hours of operation at SIPI shall commence no earlier than 8:00 a.m. (MST) and continue no I ter than 5:00 p.m., Monday through Friday or as agreed upon between the contractor and the Contracting Officer Represen ative (COR). All work shall be coordinated through the COR including work that may need to be scheduled after hours and we kends.
Holiday Schedule: If a holiday falls on a scheduled service day, the Contractor will be responsible for res heduling services for the first day post the holiday observance.
Federal Holidays.

Pest Managers shall provide a written plan which will serve as an IPM SERVICE PROGRAM.
Role of IPM Program Manager
Prevent pest problems through no pesticide means as much as possible.
Monitor each pest habitat to determine pest population, size, occurrence, and natural enemy population if present. Identify decisions and practices that could affect pest populations. Keep records of such monitoring.
Set for each pest at each site and identify in an IPM implementation plan, an injury level, based on how much biological, aesthetic or economic damage the site can tolerate.
Consider a range of potential treatments for the pest problem. Employ no pesticide management tactic first. Consider the use of pesticides only as a last resort and select and use pesticides only within an IPM program and in ace rdance with the provisions of this section. No regulated pesticides or pest control devices may be used on campus prop rty unless they are applied by persons certified and licensed in the applicable pesticide applicator category by the New Me ico Department of Agriculture (NMDA).
Monitor treatment to evaluate effectiveness. Keep monitoring records and include them in the IPM impl mentation plan.
Keep current on IPM technologies.
2. Scope of Work
The contractor shall furnish all supervision, labor, materials, tools, supplies, equipment, transportation a d applicable licenses and permits necessary to perform quality Integrated Pest Management (IPM) services at Southwestern ndian Polytechnic Institute. IPM services shall be performed in a manner that will ensure the health and general well-being of the public, students and SIPI personnel.
The primary objective of the IPM program is to achieve long term, environmentally sound pest control th ough the use of a wide variety of technological and management practices. These practices include inspectionlsurveillanc , identification, exclusion, trapping, communication, sanitation, biological and mechanical means of control, and where ppropriate, application of pesticides as well as recommendations for structural and procedural modifications intend to eliminate the food, water and shelter available to pests. Frequency of treatments for all services will be determined IPM Manager.
2.1 Integrated Pest Management
A. The contractor's IPM Program shall address abatement of the following pests:
1. Indoor and outdoor populations of cockroaches, ants, fleas, ticks, mites, spiders, silverfish nd other insects, arachnids, and arthropods.
2. Individuals of all excluded pest populations that are incidental invaders inside the specified buildings, including winged termite swarmer's emerging indoors.
3. Mosquitoes, Bees of varying species, Wasps of varying species and other indoor land outd or populations of winged stinging insects including nests, native to New Mexico and Albuquerque metro are .
4. Nesting Swallows, Nesting Geese, Cranes, Pigeons and other flying aviary not protected b state or federal laws, mechanical means of deterring from nesting, defecating on roof top equipment and roofing embrane material
5. Snakes, all species, poisonous, non-poisonous, native to New Mexico and surrounding are s not protected under state or federal law, abatement by means of deterrent and or trapping, keeping snak s to the outer perimeter of the building campus locations.
6. Prevention, monitoring and treatment for Bedbug services in Bldg. 113 (Four Winds Lodge) and Bldg. 114 (Golden Eagle Lodge), contractor shall provide all-inclusive cost for preventive, monitoring nd treatment of the entire dormitory building and its complete furnishings located in each room at Four Winds L dge and Golden Eagle Lodge respectively.
7. Pockets Gophers, moles, voles and other subterranean pests identified and located on SIPI campus.
8. Indoor and outdoor populations of mice, rats and other disease carrying pests identified an located on SIPI campus.
1. The contractor shall use non-pesticide methods of control wherever possible. Trapping de ices, rather than pesticide sprays, shall the standard method for indoor fly control.
2. As a general rule, the contractor shall apply all insecticides as "crack and crevice" treatme ts only, defined in this contract as treatments in which the formulated insecticide is not visible to a bystander duri g or after the application process.
3. Application of insecticides to exposed surfaces or as space sprays (fogging) shall be restri ted to exceptional circumstances where no alternative measures are practical. No surface application or spa e spray shall be made while visitors, students, and SIPI personnel are present. The contractor shall take all nece sary precautions to ensure visitor, student and SIPI employee safety, and all necessary steps to ensure the c tainment of the pesticide to the site of application.
4. Bait formulations shall be the standard pesticide technology for cockroach and ant control, with alternate formulations restricted to unique situations where baits are not practical.
5. Sticky traps shall be used to guide and evaluate indoor insect control efforts wherever nec ssary.
2.2 Detail of Services
The contractor's IPM Program shall use safe, effective, environmentally sound methods including biolo ical/genetic/cultural control, mechanical/physical control, and where necessary, the judicious use of least hazardous pestici es. All services shall conform to the requirements specified herein.
A. Insect Control
Use of Pesticide for insect control
1. The application of all pesticides shall adhere to all federal, state and local laws and regulat ns, including all applicable Environmental Protection Agency (EPA), Occupational Safety and Health Admi istration (OSHA), state/local regulatory guidance on pesticide storage, disposal and applications.
2. Pesticides shall be applied according to the methods, rates and precautions on the manufa turer's label.
3. Pesticide application shall be according to need and by schedule. As a general rule, applic tion of pesticides in any inside or outside area shall not occur unless visual sighting, inspection or monitoring d vices indicate the presence of pests in that specific area.
4. When pesticide use is necessary, the contractor shall employ the least hazardous material, most precise application technique, and minimum quantity of pesticide necessary to achieve control.
5. Pesticides shall only be applied by personnel with a Commercial Pesticide Applicator Certif ate or License.
6. The contractor shall not store any pesticide product on the job site.
7. The contractor shall obtain the approval of the COR prior to any application of pesticide liqu d, aerosol, or dust to exposed surfaces, or any space spray treatment. The contractor shall take all necessary pr cautions to ensure tenant and employee safety, and all necessary steps to ensure the containment of the pesti ide to the site of application. Other than crack and crevice treatments, no liquid, aerosol, or dust application shall be made while tenant personnel are present.
1. Indoor trapping
B. Rodent Control
As a general rule, rodent control inside buildings shall be accomplished with trapping devi es only. All such devices shall be concealed out of the general view and in protected areas so as not to be affected by routine cleaning and other operations. The contractor shall check all trapping devices for rodents n frequency deemed necessary by the IPM Manager and shall dispose of the rodents in an appropriate manne .
Use of Rodenticides for Rodent control
The contractor shall use rodenticides in circumstances when rodenticides are deemed essential fo adequate rodent control inside buildings. All rodenticides, regardless of packaging, shall be placed either in location not accessible to children, pets, wildlife and domestic animals, and in EPA-approved tamper-resistant bait boxes. As a general rule, rodenticide application outside buildings shall emphasize the direct treatment of rodent burrows wh rever feasible.
Contractor shall be responsible for the dispose of dead rodents and shall do so in an appropriate m nner. On occasion treatment of storm and/or sanitary drains may be required, this will be accomplished using appropri te methods for such treatment.
Use of Bait Boxes for Rodent Control
All bait boxes shall be maintained in accordance with EPA regulations, with an emphasis on the saf ty of non-targeted organisms. The contractor shall adhere to the following:
a. All bait boxes shall be placed out of the general view, in locations where it will not be disturbed by routine operations.
b. The lids of all bait boxes shall be securely locked or fastened shut.
c. All bait boxes shall be securely attached or anchored to floor, ground, wall, or oth r immovable surface, so that the box cannot be easily picked up or moved.
d. Bait shall always be secured in the feeding chamber of the box and never placed n the runway or entryways of the box.
e. All bait boxes shall be labeled on the inside with the contractor's business name, hone number and address, and dated by the contractor's technician at the time of installation and e h servicing.
f. The contractor shall install and maintain bait boxes where ever is deemed necess ry by the IPM manager, inside and/or outside of buildings for rodent control including, but not Ii ited to, the vicinity of dumpsters, roll-off containers and trash compactors around the Bldg 100, Bldg 10 , Bldg 104, Bldg 109, Bldg 113, Bldg 114. There are approximately eight (8) dumpsters, roll ofts and co pactors at SIPI.
g. Contractor will assess current number of bait boxes and replace obsolete, damag d or add additional to meet IPM goals for SIPI campus facility.
h. Contractor will identify all bait box locations per building on floor plans provided by SIPI.
All buildings will be assessed before placement of any commercial repellent, bird spikes, netting or other means of deterrent designated by the IPM manager. Contractor will designate areas of usage and what met od will be used to abate Pest Birds on campus buildings. Contractor will follow and adhere to all federal, state and I cal laws and regulations deemed applicable to the capture, removal, repelling of Pest Birds identified to SIPI ca pus and according the prescribed method therein:
C. Pest Aviary/Bird Control
1. Use of Commercial Bird Repellents for Pest Aviary/Bird control
Bird repellants shall be non-toxic, non-lethal and harmless to metal structures, sealed mason and ornamental metal and shall not be harmful to people, pets and children. SDS will be provided to COR for pproval of usage.
2. Use of Commercial Bird Spikes for Pest Aviary/Bird control
Dense, transparent needle strips to block pest from their landing areas. Contractor shall place bird spikes in accordance to all applicable federal, state and local laws and shall not be harmful to people, p ts and or children.
3. Use of Commercial Netting for Pest Aviary/Bird control
Netting method is to prevent birds from entering covered areas for nesting purposes. Netting ust be manufactured from knotted polyethylene twine, flame resistant and suitable to the environment being placed n. Netting will not interfere with the aesthetics of SIPI campus buildings.
D. Snake/Reptile Control
All buildings within the campus loop road will be assessed for habitation and before any placement of a y commercial repellent, trap or other means of deterrent deSignated by the IPM manager. Contractor will oesiqnate a eas of usage and what method will be used to abate snakes in the area interior to the campus loop road and all deslqnate areas of occupation by students, staff and visitors. Contractor will follow and adhere to all federal, state and local laws and egulations deemed applicable to the capture, removal, repelling of poisonous and non-poisonous snakes identified to SIPI mpus and according the prescribed best practices.
E. Bedbug Prevention, Monitoring, Inspection and Treatment
Building 113 (Four winds Lodge) and Building 114 (Golden Eagle Lodge) will be assessed for habitation nd before any placement of any commercial repellant or abatement pesticide by the IPM manager. Quarterly Bed bug nspections will be done to a random room on each wing, each direction, each floor, each building. A report of findings will e drawn up with identifying information, what building? What wing? What floor? What room? or any pertinent information f r preventative maintenance purposes.
F. Gopher/Mole or Burrowing Pest Monitoring, Trapping & Removal Services
The gopher, Mole or burrowing pest removal process consists of identifying existing tunnels and placing echanical traps well below the ground surface, covering and tagging area and identified on grounds map provided by SIPI of pproximate location of traps. Traps are to be checked often then replaced in location of gopher activity. No pesticides or che icals are to be used and all materials must be environmentally safe. Once an area is deemed clear of any burrowing pests he mounds must be leveled out in that area, to reduce trip and fall hazards.
3. SUBMITIAL REQUIREMENTS:
The Contractor submitting the Proposal shall be able to provide the following:
1. Documentation verifying experience in successful trapping of gophers, moles and other burrow ng pests.
2. Any licenses required by the New Mexico Dept. of Agriculture.
3. Provide copies of all appropriate liability insurance, business license and taxation certificates.
4. Vendor must provide their own mechanical traps; any traps lost or stolen during the trapping ar not the responsibility of the Southwestern Indian Polytechnic Institute.
5. Provide records of the number of gophers trapped from which locations.
6. The invoice must contain cost for trapping the gophers as a per gopher fee.
3.1 Minimum Qualifications
Licensing, Insurance, and Credentials
i. Current Insurance Certificate
ii. Current state licenses of all personnel that provide service to the site.
iii. Any other validations or credentials of site services
5. Work Place Safety
Due to the nature of the educational environment of the facility and daily operations, all work shall be in ccordance with common safe work practice.
The Contractor shall comply with all applicable Federal laws and with such additional measures as the C ntracting Officer may find necessary. The Contractor is required to implement a ZERO INJURY PROGRAM for this project.
6. Deliverables and Reporting Requirements
Approved or agreed upon pesficlde list, labels, and SDS. Labels and SDS of all pesticide materials used or that might be used for covered or ancillary pests are listed on a master list, electronic and binder format.
Service Reports and Treatment Records: All captured data from each service event is placed in a report 0 ensure work has been accomplished.
Changes to pesticides used. In the event that changes have to be made to the chemicals used, contract r shall provide the COR with the Label and SDS.
7. Contracting Office Representative (COR)

8. Progress Payments
Payments shall be made in accordance with the payment provisions contained in the base contract
7.1 Invoice Submission
Payments shall be made via the Governments Invoice Processing Platform (IPP) System. Submit curtesy invoice (via email listed below) to COR for review of accuracy and approval to submit for IPP processing. Invoices sh II be submitted monthly.
Improperly prepared invoices shall be returned to the contractor, resulting in delayed payment.

As a minimum, a properly prepared invoice shall include:
I. Invoice date and invoice number II. Purchase Order Number and line item number III. Contract name, address, and telephone number
8. Clearance Requirements
The Contractor shall obtain personal identification passes for all employees and vehicle passes for II contractors' vehicles requiring entry onto SIPI campus. The Contractor must complete required documents and submit it rough the COR to the Security Office. Vehicle registration, proof of vehicle insurance and a valid driver's license must be p esented for all vehicles to be registered.
8.1 Background Clearance
The contractor's supervisory/lead personnel shall a completed DOl Access/e-Qip request form to th Security Point of Contact after the contract has been approved.
8.2 Safety & Security
8.2 Contractor/Employee Identification, Sign-In and Safety Orientation
A. All employees shall sign-in/out at the Facilities Department prior to beginning servic s and end of services.
Employee(s) will require an escort until background clearance has been approved.
B. All employees shall wear a nametag or other identifying apparel.
C. All employees shall go through SIPI's Safety Orientation the first time they provide ervices on the SIPI campus. This is to be completed by the Security Department.
