Janitorial Service for Office Space NRCS Plant Materials Center, Hoolehua, Hawaii

Statement of Work

APRIL 2019

The USDA-Natural Resources Conservation Service (NRCS) is an Agency within the Farm Production and Conservation (FPAC) Mission Area of the United States Department of Agriculture (USDA). The Hoolehua Plant Materials Center (PMC) located in Hoolehua, HI has a requirement for janitorial services for two [2] office spaces: 1.) Hoolehua Field Office (~1200ft2) and 2.) Hoolehua Plant Materials Center (~1600ft2).

The base period of performance for this project will be from June 1, 2019 to May 31, 2020 with four [4] 12-month option periods.

I. SCOPE

A. OFFICE INTERIOR CLEANING

1. Office interior cleaning duties must be performed on a weekly basis

2. All office interior cleaning duties must be completed in a single business day

3. The Contractor and the Technical-Point-of-Contact shall agree upon a scheduled day and time to perform the office interior cleaning duties.

4. Office Interior Cleaning Duties Include:

a. Sweep, mop, and/or vacuum all exposed office floor-space

b. Empty and line all waste baskets

c. Clean all restroom toilets, sinks, mirrors, and shower stalls

d. Clean breakroom/kitchen surfaces including, but not limited to:

i. sinks

ii. countertops

iii. tables

iv. stove-top

v. microwaves

vi. refrigerators

e. Dust/Clean all surfaces including but not limited to:

i. doors

ii. walls

iii. window sills/recesses/blinds

iv. shelving

v. partitions

vi. book shelves

vii. filing cabinets

f. Remove insect and other pest residues wherever they may persist.

B. OFFICE WINDOW CLEANING

1. Required only once during the contract period and be completed within the first month of the contract.

2. Must be scheduled ahead of time and be completed within a 1-week period.

3. Window cleaning includes:

a. All windows of the Hoolehua PMC office build and the Hoolehua Field Office Building.

b. Clean the interior and exterior surfaces of all office windows.

c. Clean window screens of all office windows.

d. Remove insect and other pest residues wherever they may persist.

II. Minimum Accepted Standards A. All surfaces stated above shall be clean and free of debris and residue.

B. Counter tops, tables, sinks, toilets and shower stalls must be cleaned with a disinfectant cleaner.

C. Floors, base moldings and grout shall be clean and free of debris, including but not limited to, (dirt, water streaks, mop marks, string, gum, tar and other foreign matter).

D. Build-up, spills, spots, smears or crusted material on carpet must be removed if possible.

E. All Doors, walls, window recesses, and furniture must be free of scuff marks and dirt markings.

F. All trash containers must be lined and kept clean and free of odor, debris and residue.

G. All window glass shall be clean and free of dirt, dust, streaks, smudges, watermarks, spots and grime, and shall not be cloudy. There shall be no water spots on the glass.

III. Equipment and Supplies A. A dumpster is provided on-site for trash disposal B. Bio-based materials are to be used for this work.

C. The contractor shall provide all supplies, labor, tools, equipment, transportation, and cleaning materials necessary to perform the work. Supplies include, but not limited to:

1. Disinfectants

2. Floor Cleaners

3. All-Purpose Cleaners

4. Trash Bags (13 gal)

IV. Access to Site A. Contractor will have site access only during normal business hours of Monday through

Friday, 7:30am – 3:30pm B. USDA-NRCS Offices are closed on weekends and Federal holidays.

C. If the Contractor is unable to perform the activities on the agreed upon scheduled date(s), the

Contractor must contact the Technical-Point-of-Contact to re-schedule an appropriate time to assure access to the site.

V. Security and Safety FAR Subpart 4.13, Personal Identification Verification of Contractor Personnel, establishes the policy and use requirements for FAR Clause 52.204.9, Personal Identification Verification of Contractor Personnel, which is hereby incorporated into this order by reference. It requires Contractor compliance with Homeland Security Presidential Directive- 12 (HSPD-12), Office of Management and Budget (OMB) guidance M-05-24, and Federal Information Processing Standards Publication (FIPS PUB) Number 201.

USDA policy implementing referred to in paragraph (a) of the clause is at USDA Department Manual 4620-002, USDA Departmental Regulation 4620-002. This policy applies to Contractor employees who require routine access to federally controlled facilities and information systems. All such Contractor employees must have been successfully identity proofed (FBI fingerprint check) and have a successfully adjudicated National Agency Check with (Written) Inquiries (NACI) or Office of Personnel Management (OPM)/National Security (NS) community background investigation to serve on the contract. USDA agencies may grant access to USDA information systems after successful adjudication of the FBI fingerprint check. Completion and successful adjudication of the final NACI results or OPM/NS BI are still required to continue work under this contract. If the fingerprint or background investigation result is unsatisfactory the Contractor employee shall be removed from the contract immediately and replaced immediately. Unless a Contractor employee already has successfully completed these requirements and the investigation is current, the employee will be required to complete an online version of the SF-85, Questionnaire for Non-sensitive Position, two FD-258, Fingerprint Charts (can be provided by the NRCS if the law enforcement agency does not have them), and other required forms. Contractor employees’ fingerprints shall be taken to a Federal, State, municipal, or local law enforcement agency.

The Contractor is responsible for any cost of obtaining the fingerprints. The Government is responsible for the cost of the FBI finger print check and background investigation. The NRCS does not have any control over the timeliness of the background check or that it will be completed within the performance period of this contract, but it is estimated to be completed approximately 30 calendar days once the Contractor provides all of the required documentation and the NRCS submits it to the Office of Personnel Management (OPM).

Fingerprint check results are estimated to be received within 10 calendar days once the Contractor provides all the required documentation and the NRCS submits it to the OPM.

Upon award of the contract, and once the NRCS has provided the Contractor with the required documentation and instructions, the Contractor must complete and submit the required documentation promptly.

VI. Vendor Requirements A. Vendor must be a licensed and insured contractor in the State of Hawaii and follow all regulations and guidelines when fulfilling a U.S. Government contract.

B. Vendor must be currently registered with SAM, the System for Award Management
