I. SAM (System for Award Management) Entity

II. Response to Evaluation Factors (Technical and Price Criteria)
All quoters must respond to all Evaluation Factors as described in the Evaluation Factors section and include all specifically requested information. Incomplete responses may not be considered.
III. Site Visit (Pre-Quote)
No official site visit is scheduled, but interested parties are urged and expected, prior to submitting a quote, to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. FAR 52.204-9 PERSONAL IDENTITY VERIFICATION OF CONTRACTOR PERSONNEL (JAN 2011)
(a) The Contractor shall comply with agency personal identity verification procedures identified in the contract that implement Homeland Security Presidential Directive-12 (HSPD-12), Office of Management and Budget (OMB) guidance
M-05-24 and Federal Information Processing Standards Publication (FIPS PUB) Number 201.
(b) The Contractor shall account for all forms of Government-provided identification issued to the Contractor employees in connection with performance under this contract. The Contractor shall return such identification to the issuing agency at the earliest of any of the following, unless otherwise determined by the Government:
(1) When no longer needed for contract performance.
(2) Upon completion of the Contractor employee’s employment.
(3) Upon contract completion or termination.
(c) The Contracting Officer may delay final payment under a contract if the Contractor fails to comply with these requirements.
(d) The Contractor shall insert the substance of this clause, including this paragraph (d), in all subcontracts when the subcontractor’s employees are required to have routine physical access to a Federally-controlled facility and/or routine access to a Federally-controlled information system. It shall be the responsibility of the prime Contractor to return such identification to the issuing agency in accordance with the terms set forth in paragraph (b) of this section, unless otherwise approved in writing by the Contracting Officer.

(d) Comptroller General Examination of Record. FAR 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of
Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days of the present contract end date.
FAR 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 7 days provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.
Note for FAR 52.217-8 and FAR 52.217-9: Within the given deadline, the Government may unilaterally exercise an option. After the given deadline, the Contractor and Government may mutually agree to exercise an option.
FAR 52.232-19 AVAILABILITY OF FUNDS FOR THE NEXT FISCAL YEAR (APR 1984)
Funds are not presently available for performance under this contract beyond the base period or presently exercised option period. The Government’s obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the
Government for any payment may arise for performance under this contract beyond that which is currently funded, until funds are made available to the Contracting Officer for performance and until the Contractor receives notice of availability, to be confirmed in writing by the Contracting Officer.
FAR 52.236-7 PERMITS AND RESPONSIBILITIES (NOV 1991)
The Contractor shall, without additional expense to the Government, be responsible for obtaining any necessary licenses and permits, and for complying with any Federal, State, and municipal laws, codes, and regulations applicable to the performance of the work. The Contractor shall also be responsible for all damages to persons or property that occur as a result of the Contractor’s fault or negligence. The Contractor shall also be responsible for all materials delivered and work performed until completion and acceptance of the entire work, except for any completed unit of work which may have been accepted under the contract.

(b) The Contractor shall provide and maintain an inspection system acceptable to the Government covering the services under this contract. Complete records of all inspection work performed by the Contractor shall be maintained and made available to the Government during contract performance and for as long afterwards as the contract requires.
(c) The Government has the right to inspect and test all services called for by the contract, to the extent practicable at all times and places during the term of the contract. The Government shall perform inspections and tests in a manner that will not unduly delay the work.
(d) If the Government performs inspections or tests on the premises of the Contractor or a subcontractor, the Contractor shall furnish, and shall require subcontractors to furnish, at no increase in contract price, all reasonable facilities and assistance for the safe and convenient performance of these duties.
(e) If any of the services do not conform with contract requirements, the Government may require the Contractor to perform the services again in conformity with contract requirements, at no increase in contract amount. When the defects in services cannot be corrected by reperformance, the Government may—
(1) Require the Contractor to take necessary action to ensure that future performance conforms to contract requirements; and
(2) Reduce the contract price to reflect the reduced value of the services performed.
(f) If the Contractor fails to promptly perform the services again or to take the necessary action to ensure future performance in conformity with contract requirements, the Government may—
(1) By contract or otherwise, perform the services and charge to the Contractor any cost incurred by the Government that is directly related to the performance of such service; or
(2) Terminate the contract for default.

CONTRACT DOCUMENTS, EXHIBITS AND ATTACHMENTS
SCOPE OF THE CONTRACT
The Contractor is responsible to maintain the building interior(s), and indicated exterior portions, at a level of cleanliness standard for commercial administrative buildings. The quantity of office furniture or utilization of floor space is subject to change during the contract period. In cases where utilization of floor space is significantly altered, adjustment of contract price may occur through mutual agreement. Performance shall be based upon the Government's evaluation of the results of the Contractor's efforts, not upon the performance methodology itself.

Option to Extend Services
By accepting award of this contract, the Contractor is agreeing in advance to the exercise of any option periods.
Thus, option periods may be exercised (enacted) by the Government without further Contractor agreement so long as exercise occurs at least one month before the end of the present period of performance. Exercise of option periods after that time may occur through mutual agreement only. The clauses that pertain to option periods are FAR 52.217-8 and FAR 52.217-9.
If the Government elects to extend the period of performance, continuation of services will be contingent on funding for the extended period. The Government may issue a Delay of Work should continued funding be projected to be unavailable.
PERFORMANCE OBJECTIVES
The Contractor shall perform janitorial duties in the Mendocino National Forest Upper Lake Ranger Station offices located at 10025 Elk Mountain Road in Upper Lake, California. The county of performance is Lake. The
Upper Lake compound is approximately 1,400 square feet. This area includes 1 office building, 4 restrooms (2 with showers); 1 conference room (which can separate into 2 rooms); 1 lobby area; 1 food service area;
hallways; and 1 janitor closet.
The Contractor shall provide all transportation, labor, equipment, supervision, materials, and supplies, except those listed as Government Furnished Components, for the successful performance of this contract.
Standard janitorial practices, and proper use of cleaning materials and equipment, are required.

PERFORMANCE SCHEDULE
Interior services shall be provided two times per week (except Federal holidays) between the hours of 6:00PM and 6:00AM weekdays so as not to conflict with Forest Service work, or anytime weekends, unless otherwise agreed. Preferred days of service are Saturday and Tuesday, but other service days may be acceptable as negotiated.
Should a Federal holiday occur on a scheduled cleaning day, the Contractor shall work either the night immediately prior to the holiday or the night immediately following the holiday. It is the Contractor's responsibility to notify the COR of inability to perform duties on any particular day, and notification is required prior to 3:00 p.m. on the day of the service (or on Friday, if the day of service is a weekend).
In the event services are not provided or required because the facility is closed due to inclement weather, unanticipated holidays declared by the President, failure of the Congress to appropriate funds, etc., the CO or
COR shall either:
a. Direct the Contractor to perform the work contractually specified over a number of immediately subsequent day(s) equal to the duration of the shutdown, or
b. Forego the work and reduce payment due to the Contractor for work not performed. The deduction rate in dollars per day shall be pro-rated. Appropriate adjustments will be made by the CO in the event services are provided for portions of days, or
c. Reschedule the work on day(s) satisfactory to both parties. Work shall not be required on the following holidays: New Year’s Day; Martin Luther King, Jr.'s Birthday; Presidents' Day; Memorial Day;
Independence Day; Labor Day; Columbus Day; Veteran's Day; Thanksgiving Day; Christmas Day.
Routine Tasks
Tasks that are expected to be performed at each frequency include, but are not limited to: trash collection;
vacuuming; cleaning & disinfecting bathrooms, drinking fountains, common kitchen area floor and sink; cob web removal from multiple surfaces (e.g. window sills, porches, and other areas prone to cobweb buildup); and wiping down of common use surfaces (e.g. fax machine surface areas, conference room tables, conference room desks, and clean front door glass).
Periodic Tasks
Periodic tasks is defined as janitorial or related services that are performed not on a daily or weekly basis, but at least quarterly. Periodic tasks would include such items as window washing; wall cleaning; window blind cleaning; dusting high surfaces such as lights and ceiling fixtures; and floor waxing.

Items on desks and other working surfaces or storage areas shall not be disturbed.
The Contractor shall not expose electronic equipment, including computer monitors, to liquids of any kind.
Any offices locked or designated off-limits could permanently or periodically be exempt from performance tasks, with the exception of emptying trash containers left outside the door of an excepted area.
EMERGENCY OR SPECIAL EVENT CLEANING
Any situation requiring cleaning services beyond those normally encountered in routine cleaning work will be considered "emergency". Situations requiring emergency cleaning will be referred to the Contracting Officer or
COR prior to commencing work. If the Contracting Officer determines that the situation requires extra effort on the part of the Contractor, equitable reimbursement will be provided to the Contractor for the level of effort over and above that normally required, (e.g. fire or flood damage, construction work, etc.). Residue from relocating furniture or litter left from meetings or routine gatherings are not considered emergency cleaning.

CONTRACTOR PERSONNEL AND REQUIREMENTS
Contractor Personnel
All work performed under this contract shall be in a skillful and professional manner. The Contractor shall employ personnel qualified and capable to perform janitorial work and shall provide a list of Contractor personnel who will be on the premises. Persons under the age of 18 are not permitted on the premises while work is performed. Only the Contractor and Contractor's employees shall be permitted in the building during the working period. The Contractor shall prohibit employees from disturbing papers on desks, opening desk drawers or cabinets, using government telephones or other office equipment, leaving the Government building unlocked or using any Government employee's property. The Contracting Officer may, in writing, require the
Contractor to remove from the work any employee the Contracting Officer deems incompetent, careless, or otherwise objectionable, or for theft, possession and/or removal of materials, supplies, equipment, or any
Government property.
Subcontracting
The Contractor shall alert the Contracting Officer in writing of intent to utilize a subcontractor for work under this contract, prior to entering into any subcontract arrangement. The subcontractor shall have the experience and be equipped for such work. The written notification shall include as a minimum:
a. The name, address and telephone number of the subcontractor.
b. A detailed description of the work to be subcontracted.
c. Documentation of the subcontractor's representative authority.
Subcontracting any portion of the contract shall not relieve the Prime Contractor of any responsibility under this contract. Any subcontract agreement shall contain all terms and conditions of the prime contract.
Conservation of Utilities and Building Security
The Contractor shall be directly responsible for instructing employees in utility conservation practices. Upon completion of the work at each site, the contractor personnel shall ensure that (unless otherwise instructed by the Contracting Officer or COR) all lights are off, all wall windows and entrance doors are locked and the alarm system armed. Mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by Contractor employees. Lights shall be used only in areas at the time when work is being performed.
Water faucets or valves shall be turned off after the required usage is complete. Windows shall be closed and lights and fans turned off when not in use. If the building is occupied by the Government after normal working hours, the Contractor's personnel shall notify the Government personnel of their departure and leave the exterior doors locked. The Contractor shall not disturb papers on desks or bulletin boards, open drawers or cabinets, or use telephones or any office equipment provided for official Government use except for emergency calls to the
Contracting Officer, police or fire department.
Safety and Liability
The Contractor shall be responsible for his or her own safety and the safety of the Contractor's employees or
Subcontractors.
Safety: The Contractor shall be responsible for instructing company employees in appropriate safety measures. The Contractor must comply with the applicable Occupational Safety and Health
Administration (OSHA) Standards specified under 29 CFR part 1910 and other Federal, State, and local regulations for the purpose of providing safety precautions to all of employees subject to exposure as a result of custodial work performed under…
