Basis for Award: The Government intends to award firm fixed price contract resulting from the Request for Quote to the total lowest priced quote. Price shall be fair market value for materials, labor and incidentals represented in the specifications necessary to complete the contract for the base year as well as all option years.

STATEMENT OF WORK

Forest-wide Pest Control Service Contract

CARSON NATIONAL FOREST

1.0 Scope of Work:

The Carson National Forest is seeking a licensed Pest Control Service Company to control Pests (primarily mice and spiders) at the Carson National Forest. The Contract shall be comprised of a Base Bid with four

(4) option years.

The Contractor shall schedule to perform the service one (1) time per month at each district and administration site listed in the Items below. Rodent and Insect removal may be needed during the month for unforeseen instances when Rodents or Insects enter the building and need to be removed. The Contracting Officer Representative (COR) shall notify the Contractor if services are required between the Monthly Services. Any services required between the scheduled Monthly Service shall be included in the Monthly Invoice in IPP.

The Contractor shall seal all entry points that are two (2) inches nominal size in diameter and less with Copper Wool, Foam Spray, Caulking, or other Sealant. The Contractor shall notify the Contracting Officer Representative (COR) of any entry points greater than two (2) inches nominal size in diameter.

The Contractor shall provide Unit Pricing listed in the Schedule of Items.

The Contractors bids shall be all inclusive of the Base Year and all four (4) Option Years. The bids will be evaluated and selected based on the Lowest Bid of the Base Year and each of the four (4) Option Years.

The Contractor shall provide Pest Control Service at the following locations:

2.0 Specifications:

1. The Contractor shall comply with all federal and state OSHA and Environmental Regulations including the 40 CFR Chapter 1, Subchapter E and the State of New Mexico Administration Code (NMAC) Title 21, Chapter 17, Part 50.

2. The Contractor shall submit a copy of their Pesticide Applicators License for approval to the Contracting Officer Representative (COR) prior to any application of any Pesticide.

3. The Contractor shall assess the interior and exterior of the buildings to determine the best methods and locations to apply pesticides, insecticides, and place traps.

4. The Contractor shall submit Safety Data Sheets (SDS) for Nisus DSV or equal Disinfectant, Sanitizer, and Virucide for approval to the Contracting Officer Representative (COR) prior to any disinfecting and/or sanitizing of any buildings.

5. The Contractor shall submit SDS for EPA and State of New Mexico approved Pesticide(s) for approval prior to applying any treatment around and inside any forest service buildings.

6. The Contactor shall submit SDS for EPA and State of New Mexico approved Insecticides for approval prior to applying any treatment around and inside any forest service buildings.

7. The Contractor shall dispose of Pests per federal and state regulations.

8. The Contractor shall provide a Monthly Service Report outlining the Service’s provided at each

Task Item above.

3.0 Period of Performance:

1. The Contractor shall begin the Pest Control Service upon receipt of a Notice-to-Proceed (NTP) from the Contracting Officer (CO). The CO is the only federal government employee authorized to issue the NTP, Award, and to make any modifications to the Contract. The Contractor shall coordinate with the Contracting Officer Representative (COR) to schedule the service.

2. The Contractor shall coordinate with the Contracting Officer Representative (COR) to schedule the first initial walk-thru upon receipt of award and the Notice-to-Proceed (NTP).

3. The monthly service shall be scheduled the first week of the month. The Contractor shall notify the Contracting Officer Representative (COR) if the Contractor cannot perform the service the first week of the month to schedule another time.

4. All Districts are accessible via Paved Roads and the Canon Administration Site is plowed by the Forest Service Facility Crew after Winter Snow Storms. The Forest Service follows the local District Public School Severe Weather Closure Schedules during Winter Snow Storms. The Contractor shall coordinate with the Contracting Officer Representative (COR) to reschedule the Monthly Service if the Contractor cannot perform the Monthly Service on the scheduled date(s) because of Winter Conditions.

5. The Contracting Officer Representative (COR) shall coordinate with the Contractor for additional Pest Control Service if needed during the length of the contract. The additional Pest Control Service shall be based on the Unit Pricing provided in the Schedule of Items in the Base Year or Option Year. The COR shall coordinate with the CO to modify the contract for the additional service. The Contractor shall invoice the additional service per the Unit Pricing during the Base Year and for each respective Option Year.

Question No. 1: Who does the Contractor coordinate with for the initial walk-thru of the Districts after the Contract has been awarded?

Answer No. 1: The Contractor shall coordinate with the Contracting Officer Representative (COR) to schedule the first initial walk-thru upon receipt of award and the Notice-to-Proceed (NTP).

Question No. 2: How is weather going to affect accessing the districts during the scheduled Monthly Service during the Winter because of bad Road Conditions?

Answer No. 2: All Districts are accessible via Paved Roads and the Canon Administration Site is plowed by the Forest Service Facility Crew after Winter Snow Storms. Answer No. 3: The Contracting Officer Representative (COR) shall notify the Contractor if services are required between the Monthly Services. The services required between the scheduled Monthly Service shall be included in in the Monthly Invoice in IPP.

Question No. 4: Is the Bid inclusive of the Base Year and all four (4) Option Years?

Answer No. 4: The bids will be evaluated and selected based on the Lowest Bid of the Base Year and each of the four (4) Option Years.

Question No. 5: Is the Contractor expected to “Seal” Pest entry locations?

Answer No. 5: The Contractor shall “Seal” openings that are two (2) inches and smaller with Copper Steel Wool. The Contractor shall include in a Monthly Service Report any Entry Locations with openings larger than two (2) inches.

Question No. 6: Is the Contractor to submit Safety Data Sheets (SDS) for all chemical used?

Answer No. 6: Yes, the Contractor shall submit Safety Data Sheets (SDS) for all chemicals used to the Contracting Officer Representative (COR) upon receipt of award.

Question No. 7: Is the Jicarilla District included in this Pest Control Service Contract because of its remote location in Bloomfield New Mexico?

Answer No. 7: The Jicarilla District “IS NOT” included in the Pest Control Service Contract. The Contracting Officer Representative (COR) to coordinate with the Contracting Officer (CO) to remove the Jicarilla District from the Scope of Work.
