1205K319R0001 Lower Falls Foundation Prep and Backfilling

a. Physical Conditions. The indications of physical conditions on the drawings and in the specifications are the result of site investigations and surveys performed by the Government.

The contractor is also solely responsible for locating all work items in field, and provide appropriate distance measuring instruments to find all work items.

b. Weather. Low temperatures at the project site range from temperatures of less than 20º F in December and January to lows of 50º F in July and August. High temperatures range from 35º F in December and January to over 80º F in July and August. The annual precipitation at the project site is over 50 inches per year, including snow fall. Winter snow storms and freezing temperatures will occur during the winter months that result in a winter project shutdown. Icy conditions on local roads and highways can occur during the fall, winter and spring. Highway curves in shaded areas will remain covered with ice or frost for weeks. Fog may form anywhere within the project area, and the site experiences frequent overcast conditions.

c. Work Hours. No Holidays, unless a written request is submitted to the Contracting Officer for approval. No weekends, unless a written request is submitted to the Contracting Officer for approval.

d. Safety, Emergency Communications and Evacuation. Contractor is responsible for the safety of the Contractor’s employees including Subcontractors employees, Government employees associated with the project, and any visitors to the site both Government and private. The Contractor shall submit a Project Safety Plan to the Contracting Officer for review prior to commencement of Work. The project safety plan shall comply with the appropriate California OSHA and Federal OSHA rules and regulations and must include a medical evacuation and emergency communications plan.

e. Highway Permits. Contractor is responsible for securing any overload or over width limit permits required for the transport of materials or equipment to the project site. The Contractor is responsible for conducting a thorough reconnaissance of all routes leading to the site.

Additional permits will be necessary by the contractor for the (1) bridge and (2) over-sized loads and/or over-weight loads. These applications have also been included in this contract.

These permits will be accomplished once the contractors knows their load weights, load configurations, and travel routes. These permits are separate from a Road Use Permit for this contract, and are accomplished by the Forest Bridge Engineer. f. Site Access and Restrictions. The site can be accessed off Forest Road 90; the area is signed as the Lower Falls Campground. The contractor shall utilize this existing paved roads to transport equipment and materials to the site. The Contractor shall restore the any areas disturbed by construction activities prior to full acceptance of this project. In addition, the Contractor may remove any signs, kiosks and other site improvements, where shown on the drawings, that will impede their ability to mobilize equipment and material, and perform construction activities. However, these features will also have to be re-installed to their pre-contract condition after all construction work is completed. During the entire duration of the project, the campground and day use area shall be kept CLOSED to the public. The contractor will NOT be responsible for providing any public parking, as the site will be CLOSED to the public. The contractor shall propose an area designated for staging and transloading, while ensuring that public transit along the USFS 90 RD is not impeded, for approval by the Contracting Officer.

g. Existing Utilities. Contractor shall be responsible for locating and marking all underground utilities prior to the start of work. The Contractor shall be required to (a) commence work under this contract within _10__ calendar days after the date the Contractor receives the notice to proceed, (b) prosecute the work diligently, and (c) complete the entire work ready for use not later than _May 8, 2020___ . The time stated for completion shall include final cleanup of the premises, final completion of punch list items, and de-mobilization.

Note: Field work shall start September 25, 2019, no exceptions.

F-3 Maximum Workweek – Construction Schedule (AGAR 452.236-75) (NOV 1996)

Within 0 working days after receipt of the notice to proceed, the Contractor must submit the following information in writing for approval:

(a) A schedule as required by FAR clause 52.236-15, Schedules for Construction Contracts, and

(b) The hours (including the daily starting and stopping times) and days of the week the Contractor proposed to carry out the work.

The maximum workweek that will be approved is Monday through Friday, 6:00 a.m. to 7:00 p.m.

Submit the Project Work Schedule on or before the date of the post-award conference.

SECTION G--CONTRACT ADMINISTRATION DATA

G-1 AGAR 452.215-73 Post Award Conference (NOV 1996)

A post award conference with the successful offeror is required. It will be scheduled and held within _5__ days after the date of contract award. The conference will be held at Mt. Adams Ranger Station_ or a mutually agreed upon location.

G-2 Contract Release

G-3 Government-Furnished Property

The Government will provide the following item(s) of Government property to the Contractor for use in the performance of this contract. This property shall be used and maintained by the Contractor in accordance with the provisions of the "Government Property" FAR clause contained elsewhere in the contract.

A.

1. Fabrication of Four (4) Double-Vault Cascadian with Porch vault toilet

2. Fabrication of Two (2) Single-Vault Cascadian vault toilet

3. Fabrication of All precast concrete vaults for each toilet

4. Placement of all vaults and comfort stations only.

B. The Forest Service has a separate contract with CXT, to provide the toilet structures. The toilet structures will be manufactured, delivered, and placed in the designated locations be CXT. CXT will begin delivery and installation of the toilet structures on October 15, 2019. The construction contractor for this contract shall coordinate with the Forest Service and have all demolition and site preparation (including foundations) completed prior to the arrival of the vault toilets. The delivery date and installation dates for the CXT structures will be verified and coordinated with this contracts awarded contractor prior to work on the ground starting.

C. The Forest Service will provide all necessary floor plans and specification, from CXT, to this contract’s awarded contractor prior to work on the ground starting. The specific specifications for installation of CXT products has been incorporated in this contract’s specifications

G-4 Payment – Invoice Processing Platform

Partial and final payments will be made upon receipt and approval of a suitable invoice in the Invoice Processing Platform (IPP) system.

1205K319R0001 Amendment 01- Questions and Answers

Q1: It seems the design mix for the asphalt on this project does not exist a nearby batch plants, the pavers we contacted said that 1/2 inch minus or 5/8 inch minus is available in wsdot design. Please advise.

A1: It is approved to use a WSDOT 5/8” minus mix. .

Q2: Could I get the name of the Manufacturer of the Vault Toilets RFQ 1205K319R0001 for recommended method of eliminating remainder of sewage. Can I request access into the vault area to see the vault chamber? Do you have information regarding the manufacture of the current toilet system (in place)?

A2: The manufacturer has supplied all the needed information in the provided plans. In addition, the current systems are pumped through a year septic contract that the FS has in place and that particular contractor pumps water into the vault to be able to liquefy the sewage material and then pumps all of that material out; this is due to the current system have a drain line that removes waste water from the chamber. Typically, if a contractor was to reach out to a septic pumping company, they would have informed them that this is how these systems are pumped.

Q3: Will we be required to have a fire water wagon outfitted with the necessary tooling on site during construction? Also should the project be delayed due to the IFPL, will the contract time be extended?

A3: The fire wagon would be dependent on the fire plan requirements. The Forest Service will comply with the Industrial Fire Precautions Schedule and what it requires for its varying shutdown levels. .

Q4: Can we use the Lewis River Horse Camp overflow lot for staging/transload area?

A4: The contractor can camp at the Lewis River Horse Camp overflow site. Staging and transload areas can currently be designated by the contractor, the contractor just can’t affect traffic flow along routes.

Q5: Has there been an asbestos report done on the bathrooms? In order to dispose of the building debris legally we must have a report that says the building was free of asbestos or it has been abated.

A5: The site has no report of asbestos, as the site has not been directly inspected for asbestos.

Q6: Do you happen to know the capacity of the vault/tank in each restroom is (for pumping purposes) ?

A6: The plans that have been provided in the plan set will allow you to determine the size of the vaults.

Q7: What material the tank is constructed of?

A7: Refer to the plans that have been provided for the requested information.

Q8: If the campground and day use areas are to remain open during construction, how many sites can we have down/work on at any given time? Can asphalt paving be done in one site visit or do we need to plan on individual site visit.

A8: The site will be closed during construction.

Q9: Will the site remain open to the public?

A9: With this amendment, the solicitation is updated to inform all interested Contractors that the site will be closed to the public. It is the Forest Service’s expectation that the site will be closed.

Q10: Where is the designated waste site?

A10: The contract discusses “waste material” and or “unsuitable material” in numerous locations, and it also discusses different types of “waste material” and “unsuitable material.” If there is any “waste material” or “unsuitable material” that cannot be incorporated in the fill slope or embankments, and consists of standard soil and rock material, the Forest Service will have a designated waste site. The designated waste site is located on the 9055 RD. The directions to this site are as follows:

• Travel NE on the 90 RD for approximately 1.1 miles, turn left onto the 9055 RD and travel for approximately 0.7 miles (there is a “Y” on the 9055, continue right). The Forest Service COR will have a designated site in the rock pit area.

Q11: When will the trees be removed?

A11: The Forest Service is responsible for removal of the trees, as discussed in the contract.

The Forest Service will cut the trees down, the day that the site is closed to the public, and the Forest Service will be responsible for removing the fallen tree debris from the work areas as well.

Q12: What is the elevation of the site?

A12: The site elevation at the 90 road entrance is roughly 1650 ft. The actual toilets sites are at a lower elevation and can be in the range of 1560 ft. This can be confirmed on programs and apps, such as Google Earth.

Q13: Will the contractor be responsible for providing toilets on site?

A13: As stated in the solicitation, the contractor is responsible for providing toilets (porta-potties) for their workers.
