Description of Requirement:
Statement of Work
Provide annual inspection, testing, maintenance, and certification of Tongass National Forest sprinkler systems and backflow preventers in accordance with NFPA 13 and NFPA 25, to provide at least the same level of performance and protection as designed.
Provide corrective action plans and inspection reports.
Provide all management, tools, supplies, equipment, and labor.
Maintain the sprinkler systems and backflow preventers in a manner that will ensure continuous and safe operation.
Definitions
• Certification – Written documentation of a site visit during which inspection, testing, and maintenance were performed.
• Critical Deficiency – A deficiency that, if not corrected, can have a material effect on the ability of the fire protection system or unit to function as intended in a fire event.
• Deficiency – A condition that will or has the potential to adversely impact the performance of a system or portion thereof but does not rise to the level of an impairment.
• Impairment – A condition where a fire protection system or unit or portion thereof is out of order, and the condition can result in the fire protection system or unit not functioning in a fire event.
• Inspection – A site visit by a technician performed in compliance with NFPA 13 and
NFPA 25
• Maintenance – Work performed to keep the equipment operable.
• Noncritical Deficiency – A deficiency that does not have a material effect on the ability of the fire protection system or unit to function in a fire event, but correction is needed to meet the requirements of this standard or for the proper inspection, testing, and maintenance of the system or unit.
• Repair – Restore to sound working condition or to fix damage.
• Site – An individual building being serviced by this contract.
Award Sites and Number of Awards
All sites are:
• In Forest Service owned buildings
• Located in various Southeast Alaskan cities
• Accessible by regularly scheduled passenger airline, barge, and ferry
The offeror must provide a bid for sprinkler system certification and backflow preventer certification on all sites.
The government contemplates awarding one contract for the certification of sprinkler systems and backflow preventers.
Scheduling
Site visits will be performed during normal duty hours (Monday through Friday, 8:00 am to 4:30 pm).
The contractor shall develop a schedule and submit it to the CO prior to 1 May of each contracting year. 14 days in advance of the planned inspection dates, the contractor must notify the Administrative Team and the City’s Forest Service Contacts, or they shall be denied access.
For all facilities located in Juneau: Perform site visits prior to October 1 of the contracting year.

Annual Inspection, Testing, and Maintenance Requirements
• The contractor shall perform inspection, testing, and maintenance to certify each fire sprinkler system and backflow preventer in accordance with the most recent editions of NFPA 13 and NFPA 25.
• Tag the unit with date of inspection, and with the technician’s name.

Before leaving a building, provide a written corrective action plan to the Forest Service. Include the following in the plan:
• Descriptions of all impairments and deficiencies. Distinguish between critical and noncritical deficiencies.
• Recommended corrective actions to resolve all impairments and deficiencies.
• Separate price proposals to implement all corrective actions. Proposals to be valid for 24 hours.
Submit the report to at least one of the on-site Forest Service representatives listed in Section C.04, and to the Contracting Officer, immediately after completing the inspection.
Inspection Reports
Submit a PDF copy of certification to the Contracting Officer via email within 2 weeks of the site visit. The entire report shall be in one single pdf document. Send the original certification to the on-site Forest Service Contact listed in Section C.04.
For all sites located in Juneau, submit a copy of all Backflow Preventer certifications to the City and Borough of Juneau, Community Development Department, Permit Center.
Annual Inspection reports shall contain:
• Statement of work accomplished during the visit
• Results of the inspections and tests
• Description of any maintenance and repairs performed
• Any part replaced or adjusted
• All findings that meet the definition of Impairment or Deficiency.
• Corrective action plan (CAP) and price proposals to implement all items on the CAP.
Requirements for the CAP and price proposals are the same as found in Section C.09, except prices to be valid for 30 days.
Invoicing
Submit invoice following performance period for certifications. Certifications must be received prior to the invoice being paid.
Safety
The contractor is solely responsible for complying with all applicable Occupational Safety and Health Act regulations.
Quality Assurance
The government will periodically evaluate the contractor’s performance in accordance with the Quality Assurance Surveillance Plan.

Storage of Materials
The contractor shall not store material and equipment in the work area without approval and coordination with the Government. Materials and equipment approved for storage shall be placed in safe and secure location and shall not hinder service to the equipment. No flammable liquids shall be stored near the equipment. Flammable liquids must be placed in an approved flammable locker.
Disposition of Materials
All debris and spent material shall be disposed of at an approved off-site location by the Contractor. The contractor shall dispose of all material in an environmentally sound manner at no additional cost to the government.

(d) Comptroller General Examination of Record. Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate—
(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;
(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;
(3) Consist of providing goods or services to marginalized populations of Sudan;
(4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization;
(5) Consist of providing goods or services that are used only to promote health or education;
or
(6) Have been voluntarily suspended.
“Sensitive technology”—
(1) Means hardware, software, telecommunications equipment, or any other technology that is to be used specifically—
(i) To restrict the free flow of unbiased information in Iran; or
(ii) To disrupt, monitor, or otherwise restrict speech of the people of Iran; and
(2) Does not include information or informational materials the export of which the President does not have the authority to regulate or prohibit pursuant to section 203(b)(3) of the International Emergency Economic Powers Act (50 U.S.C. 1702(b)(3)).
“Service-disabled veteran-owned small business concern”—
(1) Means a small business concern—
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and
(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.
(2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).
“Small business concern” means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR Part 121 and size standards in this solicitation.
“Small disadvantaged business concern”, consistent with 13 CFR 124.1002, means a small business concern under the size standard applicable to the acquisition, that—
(1) Is at least 51 percent unconditionally and directly owned (as defined at 13 CFR 124.105) by—
(i) One or more socially disadvantaged (as defined at 13 CFR 124.103) and economically disadvantaged (as defined at 13 CFR 124.104) individuals who are citizens of the United States;
and
(ii) Each individual claiming economic disadvantage has a net worth not exceeding
$750,000 after taking into account the applicable exclusions set forth at 13 CFR 124.104(c)(2);
and
(2) The management and daily business operations of which are controlled (as defined at 13.CFR 124.106) by individuals, who meet the criteria in paragraphs (1)(i) and (ii) of this definition.
“Subsidiary” means an entity in which more than 50 percent of the entity is owned—
(1) Directly by a parent corporation; or
(2) Through another subsidiary of a parent corporation.

(1) Not less than 51 percent of which is owned by one or more veterans (as defined at 38 U.S.C. 101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and
(2) The management and daily business operations of which are controlled by one or more veterans.
“Successor” means an entity that has replaced a predecessor by acquiring the assets and carrying out the affairs of the predecessor under a new name (often through acquisition or merger). The term “successor” does not include new offices/divisions of the same company or a company that only changes its name. The extent of the responsibility of the successor for the liabilities of the predecessor may vary, depending on State law and specific circumstances.
“Women-owned business concern” means a concern which is at least 51 percent owned by one or more women; or in the case of any publicly owned business, at least 51 percent of its stock is owned by one or more women; and whose management and daily business operations are controlled by one or more women.
“Women-owned small business concern” means a small business concern—
(1) That is at least 51 percent owned by one or more women; or, in the case of any publicly owned business, at least 51 percent of the stock of which is owned by one or more women; and
(2) Whose management and daily business operations are controlled by one or more women.
“Women-owned small business (WOSB) concern eligible under the WOSB Program” (in accordance with 13 CFR part 127), means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States.
(b)(1) Annual Representations and Certifications. Any changes provided by the offeror in paragraph (b)(2) of this provision do not automatically change the representations and certifications posted on the SAM website.
(2) The offeror has completed the annual representations and certifications electronically via the SAM website accessed through http://www.acquisition.gov. After reviewing the SAM database information, the offeror verifies by submission of this offer that the representations and certifications currently posted electronically at FAR 52.212-3, Offeror Representations and Certifications—Commercial Items, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation (including the business size standard applicable to the NAICS code referenced for this solicitation), as of the date of this offer and are incorporated in this offer by reference (see FAR 4.1201), except for paragraphs
