Puget Sound Naval Shipyard & IMF, Bremerton Site and Naval Base Kitsap Document #: 1200-3240
1. SCOPE
1.1. Scope. This attachment covers the site specific requirements of Naval Base Kitsap (NBK) and Puget Sound Naval Shipyard & Intermediate Maintenance Facility (PSNS & IMF) for contract work.

3. REQUIREMENTS
3.1. Prior to commencing work. The Contractor representative(s) shall meet in conference with the Receiving Activity’s Point of Contact, shop supervision, and other necessary Shipyard personnel to discuss and develop mutual understandings relative to environmental, safety, methods and schedules, security, waste management, and any other subject necessary for a smooth and successful execution of this contract. This meeting shall be held at the Receiving Activity within 30 business days of contract award.
3.2. Methods and schedules. The work shall be executed in a manner and at such times as to cause the least practicable disturbance to the occupants of the buildings and normal activities of the Shipyard.
3.3. Plan of Action and Milestones. The Contractor shall provide a Plan of Action and Milestones (POAM) detailing key events with dates of the following:
a. Design
b. Manufacturing
c. Equipment Delivery
d. Equipment Set-up/Installation
e. Equipment Testing
f. Training
g. Final Completion Date
3.4. Field Supervisor. The Contractor shall provide a full-time (start of installation to final acceptance) Field Supervisor to direct installation and testing of the equipment. The Field Supervisor shall be capable of reading, writing, and conversing fluently in the English language and have full authority to implement field decisions expeditiously. No work shall be accomplished when the Field Supervisor is not in the immediate work area. The Field Supervisor shall be identified to the Receiving Activity’s Point of Contact prior to the start of the installation and be responsible for coordinating security clearances of all Contractor http://www.navfac.navy.mil/navfac_worldwide/specialty_centers/ncc/about_us/resources/p307.html http://www.navfac.navy.mil/navfac_worldwide/specialty_centers/ncc/about_us/resources/p307.html personnel expected to perform on-site work. The Receiving Activity’s Point of Contact shall be notified of any change in personnel performing this duty.
3.5. Calibration. The Contractor shall provide calibration control instructions for each calibrated item.
3.6. Outages and permits. The Government’s process of review/approval for Utility, Area, and Track Outages may take up to 20 business days from the date the Contractor provides acceptable drawings, forms, and any other required information to the Receiving Activity’s Point of Contact.
3.7. Excavation or drilling. An Excavation Permit request form will be provided to the contractor by the Receiving Activity’s Point of Contact. Prior to any excavation or drilling, the contractor shall:
3.7.1. Perform a Locate Service Survey of the installation site to locate and identify any subsurface interference(s). A copy of the Subsurface Interference Survey Report and a completed Excavation Permit form shall be provided, in electronic format, to the Receiving Activity’s Point of Contact for review, and approval by NAVFAC, no later than 20 business days prior to the start of excavation or drilling.
3.7.2. Post the approved Excavation Permit onsite from the beginning of excavation or drilling through completion.
3.7.3. Construct adequate containments around the excavation or drilling site to minimize dust, and to prevent foreign material from becoming airborne, and intrusion on or into existing equipment in the vicinity.
3.7.4. Two Waste Information Sheets (WIS, Section 8.2) shall be submitted for concrete cut water. One for pH less than 11, and one for pH 11 or greater. The Environmental Safety and Health Office (Code 106.33) will classify each type of concrete cut water and supply pH test strips.
3.7.5. Upon completion of excavation or drilling, the Contractor shall provide the information on the Excavation Permit indicating completion and submit the permit to the Receiving Activity’s Point of Contact for closing.
3.8. Building columns. Welding, drilling, cutting, or clamping to original building columns and column bracing is restricted. Prior to performing any of these evolutions, the Contractor shall submit a request to the Receiving Activity’s Point of Contact for approval. The request shall include locations, components, materials, static loads, dynamic loads, moment forces, and structural calculations demonstrating that the load will not adversely affect the structural integrity of the column or its bracing.
3.9. Internal combustion engine operation inside buildings. The Contractor shall utilize the following ventilation controls when operating internal combustion engines inside buildings:
a. Open doors, windows, roof vents, and/or use blowers or other mechanical systems to move fresh air through work areas. In the event internal combustion engines will be operated for more than 30 continuous minutes, the receiving activity’s Environmental, Safety, and Health Department shall be notified in advance to survey the area to ensure there is adequate ventilation.
b. When operating internal combustion engines longer than 30 continuous minutes with less than 3 minutes between operations, attach an approved ventilation exhaust hose and exhaust the fumes to the exterior of the building where they cannot re-enter the workplace. Regularly inspect the hose for defects and damage, and repair or replace as required. The Contractor shall provide an engineering solution for the use of long exhaust hoses to prevent engine back pressures.
c. Ensure internal combustion equipment is properly maintained and routinely inspected to minimize exhaust emissions.
d. Use of internal combustion equipment shall be coordinated with the Building Manager. This pre-planning will help ensure that indoor air quality issues are minimized.
3.10. Stationary Internal Combustion Engines. All stationary internal combustion engines over 50 horsepower shall be identified prior to being brought on to PSNS & IMF or NBK to prevent violation of Title V, Air Operating Permit. C/106.31 will determine the need for additional air pollution controls when using stationary internal combustion engines (i.e. skid or trailer mounted generators and pumps).
3.11. Utilities. Reasonable amounts of shop air (80-90 psi) and electricity (120 Volt) will be made available adjacent to the installation site at no cost to the Contractor. The Contractor shall be responsible for any costs incurred in connecting, converting and transferring the utilities to the work.
3.12. Storage. Lay-down area(s) will be provided within the vicinity of the installation site for storage of Contractor’s materials and tools. The Contractor shall submit an Area Outage Request to the Receiving Activity’s Point of Contact no later than 20 business days prior to the outage date. The Government does not accept responsibility for security of Contractor’s materials or tools. The area must be kept clean and orderly, free of rags, paper, and other debris. Failure to maintain the area in a clean condition may result in the loss of the area. The Contractor shall be responsible to restore the storage area to original condition after use.
3.13. Contractor Training. The following training is available for Contractors whose work requires it. The training classes are held on Naval Base Kitsap (NBK) and are provided at no cost to the Contractors. For access to NBK see section “Access to NBK”.
3.13.1. Contractors that anticipate generating or encountering hazardous waste during the performance of this contract shall attend Site Specific Hazardous Waste Accumulation Area Operator Training for Contractors, Course HW49 prior to generation of waste. This training is typically held on the third Wednesday of each month and is approximately three (3) hours in length. Contractors shall provide a list of names to the Receiving Activity’s Point of Contact of those who will be attending the training. The Receiving Activity’s Point of Contact will coordinate this training with the Shipyard Environmental, Safety, and Health Office (Code 106.33).
3.13.2. Any welding, burning, flame cutting or grinding during the performance of this contract requires a Hot Work Permit and Fire Watch Training. This training is typically held on the second Friday of each month, is approximately one (1) hour in length, and is provided without charge. Contractors shall provide a list of names to the Receiving Activity’s Point of Contact of those who will be attending the training. The Receiving Activity’s Point of Contact will coordinate this training with the Shipyard Fire Inspector.
3.14. Lifting and rigger services. The use of privately owned cranes, multi-purpose machines, material handling equipment, or other construction equipment that may be used in a crane-like application to lift suspended loads on NBK and PSNS & IMF property by Contractor personnel is restricted (see NAVFAC P-307 Management of Weight Handling Equipment for information and forms). Contractors requiring this type of equipment in the performance of this contract shall provide the Receiving Activity’s Point of Contact with sufficient information (crane manufacturer documentation of maximum axle loads in a traveling configuration, crane manufacturer documentation of maximum outrigger loads (loaded and unloaded), compliance certification, crane certification, operator qualifications, crane wire certification, lifting sketches and designs, loads, locations, time, etc.) at least five
(5) business days prior to equipment arrival for arranging Government inspection services and approvals. The Contractor shall furnish labor and provide all material handling and lifting equipment, rigging support services, and other support equipment (such as cranes, forklifts, Bobcats, man-lifts, etc.) necessary for continuous support of the installation and shall provide qualified personnel to operate it.
3.15. Crane clearance envelope. To allow for the safe travel of overhead traveling cranes, jib cranes, and hoists the Contractor shall not modify existing buildings or place equipment or temporary structures in any way, which would affect the working envelope of overhead traveling cranes, jib cranes, and hoists. Minimum clearances for working envelopes are:
3.15.1. Top clearance. A minimum of three (3) inches between the highest point of the crane and the lowest overhead obstruction must be maintained. In areas where truss sag may become a significant factor, the Government will evaluate the need to increase the minimum clearance.
3.15.2. Side clearance. A minimum of two (2) inches between the end of the crane and side obstructions (building columns, knee braces, walls, etc.), assuming the crane is skewed in the worst case situation on the runway rails or when jib cranes are rotated, shall be maintained.
Piping, electrical conduits, etc. must not reduce this clearance.
3.16. Lift points. The equipment shall be readily liftable via lift points meeting the requirements of this section to allow lifting by crane during equipment off-loading, installation, operation, and removal. Lift points shall be located, designed, and marked per the following to ensure the equipment is readily liftable when it arrives.
3.16.1. Integral (i.e., not bolt-on), lifting point items such as lugs, thru-holes, pad-eyes, trunnions, and ISO container corner fittings are suitable for use at PSNS & IMF.
3.16.2. Removable lift points such as swivel hoist rings, eye bolts, etc., are not preferred for use at PSNS & IMF due to the corrosive environment. To be permitted, the contractor must receive approval from the Receiving Activity POC prior to contract award.
3.16.3. Lift points shall accommodate ASME B30.26 common rigging gear (e.g., shackles, wire rope pendants/grommets, etc.).
3.16.4. Lift point allowable strength shall be based on ASME BTH-1 (latest revision), Category B, Service Class 0.
3.16.5. Welding involved shall comply with applicable AWS Standards.
3.16.6. When possible, to maximize stability, locate lift point(s) symmetrically around, and above, the Center of Gravity (CG), on the same horizontal plane. This helps ensure equal length slings converging to a common point will lift the equipment visually level.
3.16.7. Locate the lift points such that the routing of rigging gear is clear of obstructions.
This is to avoid damaging equipment, chaffing, etc.
3.16.8. The minimum lift point Working Load Limit (WLL) shall be based on the entire weight of the equipment plus 10%. Account for additional loads induced by 45° sling angles with respect to horizontal.
3.16.8.1. Lift points for three- or four-point lifts shall be sized such that any two points carry the entire load based on worse case load distribution with respect to center of gravity.
3.16.8.2. For padeyes, align their primary planes toward the CG so that the slings do not pull out of plane by more than 5°. Exceptions may be acceptable if they are designed to account for the resultant out-of-plane loading.
3.16.8.3. The use of spreaders or strongbacks is not preferred. If spreaders or strongbacks are required, a lift sketch detailing the required spread, capacity and orientation of that gear shall be submitted.
3.16.9. Lift point supporting structures shall be designed to sustain the various loads imparted by the lifting arrangement.
3.16.10. Provide all arrangement drawings, lift sketches, calculations required for the design and usage, and manufacture’s documentation of the lift points. All calculations required for the design of the lift points shall be stamped by a Professional Engineer. A minimum of ten business days is required for Government reviews and approvals of design(s). The Government will determine if lift sketch labels are required.
3.16.11. When required, lift sketch labels shall be permanently and conspicuously located on the equipment and must be suitable to withstand outdoor, potentially corrosive, environments. Lift sketch labels shall include, at a minimum, the following:
3.16.11.1. The maximum weight of the equipment expressed in pounds.
3.16.11.2. The CG location(s).
3.16.11.3. The lift point locations and their capacity.
3.16.11.4. Recommended minimum sling lengths and capacities.
3.16.12. Permanently label all lift points, or immediately adjacent to them, with the WLL, and test date.
3.16.13. Load test lifting points to 125% (+5%, -0%) of the WLL and hold for at least two (2) minutes. Acceptance criteria shall be: no bending, cracking, or permanent deformation of the lift points or associated structure. The contractor shall perform load tests plus a pre- and post-load visual inspection (VT) in accordance with applicable standards prior to painting.
Documentation of satisfactory results shall be submitted prior to Government acceptance.
3.16.13.1. Alternative to a load test, a magnetic particle testing (MT) of the lift point welds per ASTM E709 is permitted. The acceptance criteria is Section 6.2 of MIL-STD-2035A, or equivalent standard(s) as approved by the government, provided the testing is performed by a certified third party. Documentation of satisfactory results shall be submitted prior to Government acceptance.
3.17. Arrival of personnel. The Receiving Activity’s Point of Contact shall be notified no less than 48 hours prior to the arrival of Contractor personnel associated with the contract.
3.18. Shipment of materials. Shipment of materials shall be coordinated with site preparation and installation. Material transportation from the manufacturer’s facility to the work site shall be the responsibility of the Contractor. Limited secured storage areas at the receiving activity will not permit the Government to store material for extended periods of time. Early shipment of materials, without the permission of the Receiving Activity’s Point of Contact will be refused.
3.19. Packing material. The use of shredded paper, whether newspaper, office scrap, computer sheets, or wax paper, in packing material for shipment to Navy activities, is prohibited.
3.20. General security requirements. The Contractor shall comply with security regulations imposed by the installation Commander and/or agency occupying the space where the work is to be performed. Additional information may be found in the “Visitor and Contractor Handbook” P5100 (14). Access badges/passes and vehicle permits are obtained at Pass & ID (Bldg. 981) from 7:00 am -3:20 pm, Monday through Friday except federal holidays.
3.21. Access to NBK. A Defense Biometric Identification System (DBIDS) card will be issued at the Pass & ID office once the Department of the Navy Local Population I.D. Card/Base Access Pass Registration form (SECNAV 5512/1) has been processed and a satisfactory background check has been performed. The DBIDS card will allow access to NBK.
3.21.1. A Department of the Navy Local Population I.D. Card/Base Access Pass Registration form (SECNAV 5512/1), provided by the Government, shall be completed by the contractor.
The Contractor shall hand-carry the completed form along with a valid government issued photo identification and proof of United States citizenship to Pass & ID when picking up a badge/pass. Valid proofs of United States citizenship are:
a. Birth Certificate (with a raised state seal)
b. Valid United States Passport
c. Certificate of Citizenship (Form N-560 or Form N-561 issued by the Immigration and Naturalization Service)
d. Certificate of Naturalization (Form N-550 original only)
e. Report of Birth Abroad (Form FS-240)
f. Certificate of Birth (Form FS-545 or Form DS-1350)
g. A document provided that does not contain a photograph, a current state issued photo identification (Driver’s License or Identification Card) shall also be presented
3.21.2. Foreign Nationals shall submit a copy of their valid passport and the Department of the Navy Local Population ID Card/Base Access Pass Registration (SECNAV 5512/1), when submitting the Visitor Badge Request to the Receiving Activity’s Point of Contact. This will allow Immigration and Customs Enforcement (ICE) to complete a background check prior to arrival at NBK.
3.22. Access to the Controlled Industrial Area (CIA). The Contractor shall require access to the Controlled Industrial Area (CIA) of PSNS & IMF and shall be admitted to the work site only after they have been issued a NAVSEA badge from Pass & ID. The NAVSEA badge is separate from the DBIDS card.
3.22.1. Persons who are currently on probation or parole from a felony conviction cannot qualify for a NAVSEA badge and will be denied access to the CIA.
3.22.2. Contractor personnel requiring unescorted entry into the CIA will be required to provide personal background information to the extent necessary to obtain a NAVSEA badge.
3.22.3. A Visitor Badge Request form and Foreign Interest Determination questionnaire shall be completed by the Contractor and submitted to the Receiving Activity’s Point of Contact not less than twenty (20) business days before the badges are needed. Visitor Badge Request forms and Foreign Interest Determination questionnaires submitted less than twenty (20) business days will cause a delay in processing and/or approval.
3.22.4. The Government will issue badges without charge.
3.22.5. Contractors requesting access to the CIA will be required to view an orientation videotape lasting approximately 30 minutes prior to receiving a badge.
3.22.6. Contractor shall allow approximately two (2) hours for each employee to acquire a badge.
3.22.7. Each employee shall visibly display/wear the NAVSEA badge chest high, over the front of their outermost clothing.
3.22.8. It shall be the Contractor’s responsibility to collect and account for all NAVSEA Badges issued to their personnel upon termination of any employee, expiration of the badge, completion of the contract, or when access is no longer required. When badges, passes, and permits are no longer needed they shall be returned to the Pass and I.D. Office immediately.
3.23. Vehicle permits. Contractors may be allowed to bring contract company registered and insured vehicles into the CIA, based upon the nature of their work, as determined by the Commanding Officer in conjunction with the Industrial Security Officer. Rental vehicles may also be allowed. Vehicle rental agreements shall be in the name of the contract company and authorized driver. Personally owned vehicles (POVs) are not allowed.
3.23.1. The Contractor shall provide the following, in electronic format, to the Receiving Activity’s Point of Contact, a minimum of twenty (20) business days business days in advance of when the vehicle pass is needed:
a. Current vehicle registration. The registration shall be in the name of the contract company
b. Current vehicle insurance information. The insurance shall be in the name of the contract company.
3.23.2. Contractors shall clearly display a company sign or logo on each vehicle.
3.23.3. Contractor vehicles are not allowed to enter the CIA with more than three (3) people onboard.
3.23.4. Contractor vehicles shall not travel within the CIA between 4:02 pm and 4:09 pm.
3.23.5. Each Contractor shall provide the state registration, or a photocopy, and proof of insurance documents of each approved vehicle to the Pass and I.D. Office where one of the following Vehicle Permits may be issued:
3.23.5.1. A “lay-down” permit authorizes the vehicle to be brought in to transport tools, parts, or materials to/from the site or function as a work platform. Vehicles with “lay-down” permits are kept at the negotiated job site when not traveling to/from the gate.
3.23.5.2. A “load/unload” permit authorizes the vehicle to be brought in to drop off tools, equipment and machinery (which cannot be hand carried) then is taken out of the CIA.
Vehicles with “load/unload” permits shall not be left unattended at the job site for more than 30 minutes.
3.23.5.3. A “service permit” authorizes the vehicle to be brought in and used as a mobile work platform because it contains tools, parts, materials, supplies and/or fabrication equipment. Vehicles with “service permits” allow the vehicle to be used at job sites throughout the CIA where no negotiated “lay-down” area exists.
4. RESTRICTIONS.
4.1. Regular working hours. All work is to be performed during PSNS & IMF regular work hours from 7:30 a.m. to 4:00 p.m., Monday through Friday, except for Federal Holidays.
Work performed outside of regular working hours requires notification two business days in advance for consideration and approval. Work that disrupts the normal operations of PSNS & IMF will be required to take place outside of regular working hours/days mentioned above.
Disruptive work may include excavation and movement of equipment through buildings.
4.2. Restricted colors. This Shipyard uses the colors magenta, yellow, red, and blue to identify specially controlled materials. The Contractor is specifically prohibited from using magenta, yellow, red, or blue colored plastic wrapping, packaging materials, bags, tape, or other covering materials.
4.3. Radio restrictions. Operation of privately owned citizens band or amateur radio equipment (receive and transmit) within the geographic limits of PSNS & IMF is prohibited.
All radio equipment installed in privately owned motor vehicles must be turned off upon entering NBK or PSNS & IMF.
4.4. Privately owned personal computers. The use of privately owned personal computers by contractor personnel at PSNS & IMF is restricted. Contractors requiring use of privately owned personal computers in the performance of this contract shall complete an AIS Security Survey for Computer Equipment. The completed survey shall be returned to the Receiving Activity’s Point of Contact for processing a minimum of five (5) business days prior to the arrival date. An appointment will be scheduled on the arrival date noted on the survey.
Bluetooth devices, USB thumb drives and double ended USB cables are prohibited.
4.5. Sanitation. PSNS & IMF only allows its employees to consume food in designated eating areas. Hardhats, gloves and any other regulated work clothing shall not be worn, or placed, in designated eating areas. Contract employees shall comply with OSHA 29 CFR 1910 (Sanitation).
4.6. Smoking. Smoking is permitted in a Designated Tobacco Use Area (DTUA) only.
4.7. Sheltering in place. For drills or actual events, Contractors shall shelter in place as required by emergency announcements and personnel. If inside a building, stay inside the building and follow directions from the emergency response team. If outside, enter the nearest building at the designated entrance and remain at the entrance until given direction by the emergency response team. If inside a vehicle, pull off of the road, shut off the engine, exit the vehicle, and enter the nearest building or stay inside the vehicle, secure the heat and air conditioning, and close vents and windows. If shipboard, remain shipboard until notified by emergency personnel.
4.8. Required Speed Limits. The following speed limits shall be adhered to in the CIA:
a. Farragut Ave: 20 MPH
b. All piers, wharves, and drydock routes: 5MPH
c. All other streets: 10 MPH
5. PROHIBITED ITEMS AND ACTIVITIES.
5.1. Prohibited Items. Weapons or other dangerous materials, alcoholic beverages, and any other item, the possession of which is prohibited by Federal or Municipal law, Department of Defense or Department of Navy Instruction, directive and policies, shall not be brought on NBK or PSNS & IMF property.
5.2. Prohibited photography/recording. Contractor personnel are prohibited from having devices with cameras or activated WI-FI/Bluetooth, tape recorders, zip drives, or other recording devices in their possession while inside PSNS & IMF’s Controlled Industrial Area
(CIA).
5.3. Cellular phones with cameras. Cellular phones with cameras are prohibited at PSNS & IMF. The use of cellular phones while driving at PSNS & IMF is prohibited.
6. ENVIRONMENTAL SAFETY AND HEALTH
6.1. Environmental, safety and health requirements for PSNS & IMF and NBK.
Contractors shall comply with the Environmental, Safety, and Health requirements for PSNS & IMF and NBK. The Visitor and Contractor Handbook (P5100 (14)) is intended to provide
Contractor personnel with information regarding environmental, safety, and health issues they may incur during their work at PSNS & IMF and NBK.
6.2. Environmental Compliance. Contractors working at PSNS & IMF are required to perform their work in compliance with all Federal, State, and local regulations pertaining to the environment. Contractors shall ensure all their personnel and their supervisors are familiar with the contents of “Contractor’s Guide to Environmental Compliance,” P5090 (4), and “Contractor’s Guide to Hazardous Waste Compliance” P5090 (5).
6.3. Environmental Protection. The equipment shall be designed and constructed so that during the operation, service, transportation, and storage conditions described herein, including final disposal, the equipment will comply with all applicable Environmental Protection Agency (EPA), Occupational Safety and Health Agency (OSHA) and State of Washington Department Of Ecology (WDOE) restrictions for materials classified as hazardous to the environment in effect on the date of the contract. The equipment described herein shall not contain or emit material hazardous to the ecological system as prescribed by federal, state and local statutes in effect at PSNS & IMF.
6.4. Spill Events. The following information is provided to ensure all contractor personnel performing work at PSNS & IMF are aware of and understand spill prevention, spill events, and the proper response for each type of spill event.
6.4.1. A spill event is any unpermitted or uncontrolled release of oil or a hazardous substance to the water or ground. This includes any spilling, leaking, pumping, emitting, discharging, injecting, escaping, leaching, disposing, or dumping of liquid or solid material not authorized by the Contract.
6.4.2. Contractors shall take all reasonable and necessary precautions to prevent Oil and Hazardous Substances (OHS) from reaching the air, ground, or waterway.
6.4.3. Emergency spill events are defined as follows:
a. An immediate threat to human health or the environment, or
b. A material not known to the person discovering the spill, or
c. Has the immediate potential to enter, or has entered, a drain or waterway, or migrate off Government property, or
d. Requires assistance from the Government for cleanup, or
e. The quantity spilled is greater than ten (10) gallons.
6.4.4. A non-emergency spill event is anything not specified as an emergency spill event.
6.4.5. The following applies to spill events resulting from work being performed or equipment being provided by Government Contractors in the performance of their current contract:
6.4.5.1. In the event of an emergency spill event, the Contractor shall immediately notify the Government Regional Dispatch Center by calling 911 on a Government phone, or (360) 476-3333 on a non-Government or cellular phone.
6.4.5.2. The Contractor shall isolate the spill area and stay upwind until arrival of the Government clean-up crew.
6.4.5.3. If the Contractor knows the properties of the spilled material they shall, providing it can be done without endangering the safety or health of the Contractor or other personnel, try to stop the spill and/or contain the spill to prevent it from going into drains or waterways.
6.4.5.4. The Contractor shall notify the Contracting Officer and/or the Receiving Activity’s Point of Contact as soon as possible.
6.4.5.5. The Government will respond to all emergency spill events.
6.4.6. The Contractor may be requested to assist the Government clean-up crew. If requested, all available technical data (Safety Data Sheets (SDS) and waste profiles) the Contractor possesses, for the material spilled, shall be provided to emergency response personnel.
6.4.7. The Contractor shall assist Government personnel in the preparation of the spill reports as requested.
6.4.8. The Contracting Officer shall be provided with all relevant data necessary to determine financial impact and liability of the spill event, and reimbursement for assistance of spill clean-up and disposal services.
6.4.9. In the event of a non-emergency spill event, the Contractor shall stop the source of the spill, contain the spilled material, (to keep it away from drains or waterways), and block any drains near the spill if there is a chance the spilled material will reach them.
6.4.10. Contractor personnel shall wear the proper personal protective equipment while cleaning up a spill.
6.4.11. Waste debris shall be turned over to the Government Accumulation Area Operator (AAO) as Waste Awaiting Designation (WAD).
6.5. Water Pollution Prevention and Best Management Practices (BMPs). In no event shall waste or any other material be disposed of in the storm sewer system.
6.5.1. This system is normally identified with a metal fish tag stating “DO NOT
DISCHARGE - DRAINS TO BAY”.
6.5.2. Discharging to a sanitary sewer drain (e.g., sinks & toilets) is prohibited unless prior authorization has been obtained from the Receiving Activity’s Environmental, Safety, and Health Department.
6.5.3. Pamphlets entitled Water Pollution Prevention Practices (BMPs) and “Contractor’s Guide to Environmental Compliance”, P5090 (4), are available from the Receiving Activity’s Point of Contact upon request. The pamphlet and guide will help explain what types of practices need to be identified and utilized for Contractor activities.
6.5.4. Pollution prevention practices include, but are not limited to: good housekeeping, proper materials storage and handling, drip pans, control of dust and overspray, over water protection, protection of storm drains, and preventative maintenance.
6.6. Air Pollution Control and Reporting. Containers of paint, epoxy, solvents, or other volatile organic compounds (VOCs) are not to be left open to the atmosphere unless they are being used. All containers are to be secured at the end of each shift. Evaporation of solvents shall not be used as a means of minimizing or disposing of dangerous waste.
6.6.1. Report the usage of Volatile Organic Compounds (VOCs) and Toxic Air Contaminants to the Receiving Activity’s Point of Contact on the Receipt and Monthly Usage Form that will be supplied upon completion and submission of the Contractor Hazardous Material Inventory Form (CHMI), specified in section 7, Hazardous Materials.
7. HAZARDOUS MATERIALS
7.1. Globally Harmonized System (GHS) Safety Data Sheets (SDS). Offerors and Contractors shall submit Globally Harmonized System Safety Data Sheets (SDS), whenever the supplies being acquired or supplies used during performance are identified as hazardous materials. Hazardous Materials include any material which, by virtue of its potentially dangerous nature (e.g., toxic, flammable, corrosive, oxidizing, irritating, sensitizing, reactive), requires controls in its use, packaging, handling, storage, or stowage, to assure adequate safety to life and property. This definition is intended to apply to proprietary industrial, commercial, or locally prepared blends, mixtures, formulations, or compounds of gases, liquids, and solids intended for use at the job site. Any other material designated by a Government technical representative as potentially hazardous and requiring safety controls shall also be supplied with a SDS. SDSs shall be available at work sites where materials are being used.
7.2. Contractor Hazardous Material Inventory Request (CHMI). Hazardous materials to be used in the performance of this contract require advance approval by the Government and monthly reporting by the Contractor. Prior to arrival and use at NBK or PSNS & IMF, the Contractor shall provide an inventory of all hazardous materials anticipated to be used during the performance of the contract. Hazardous materials shall be listed on a Contractor Hazardous Material Inventory (CHMI) form. The CHMI form can be obtained from the Receiving Activity’s Point of Contact.
7.2.1. The Contractor shall provide, in electronic format, the completed CHMI form and the Globally Harmonized System Safety Data Sheet (SDS) for each item listed on the CHMI.
The SDS shall be not older than five (5) years.
7.2.2. CHMI submittal shall be no less than 14 business days prior to hazardous material being brought on PSNS & IMF to allow review and approval by the Environmental, Safety and Health office. A Receipt and Monthly Usage Form will be issued for all approved hazardous materials.
7.2.3. The Receipt and Monthly Usage Form shall be completed and returned to the Receiving Activity’s Point of Contact within five (5) business days of the following month.
If the Receipt and Monthly Usage Form is not the final submission, the following month’s Receipt and Monthly Usage Form will be sent to the Contractor.
7.3. Containers. Containers shall have a labeling system to identify the contents of all containers on site, per 29 CFR 1910.1200, Hazard Communications Standard. This shall include as minimum the following:
a. Chemical Name
b. Manufacturer’s Name and Address
c. Explanation of the Chemical Hazard
7.4. Hazardous material exclusions. Supplies being used in the performance of this contract, or materials being provided as part of the equipment shall be free of known hazardous materials. Hazardous materials shall not be brought on site without prior approval from the Government, Code 106.33. Hazardous materials not needed for this project are prohibited.
Definitions of hazardous materials are specified in the latest version, including revisions adopted during the term of the contract, of Federal Standard No. 313.
7.4.1. Notwithstanding any other hazardous material usage permitted under this contract, radioactive materials or instruments capable of producing ionizing radiation as well as materials which contain asbestos, mercury, methylene chloride, lead, or polychlorinated biphenyls are prohibited. Contractor exceptions to the use of any of the above excluded materials must be submitted for approval, prior to usage, by the Contracting Officer.
7.4.2. Class I Ozone Depleting Substances, as defined in 40 CFR Part 82, shall not be used in the performance of this contract, or be provided as part of the equipment.
7.4.3. Exceptions to the prohibition of these materials must be referred to the Contracting Officer in writing, for consideration within 30 days after contract award and prior to any work being performed.
7.5. Unforeseen hazardous material. If material that may be dangerous to human health upon disturbance is encountered during contract operations the Contractor shall stop that portion of work and immediately notify the Receiving Activity’s Point of Contact. If the situation is an immediate threat to human health or the environment, call 911 on a Shipyard phone or (360) 476-3333 on an outside line or cellular phone.
8. WASTE
8.1. Handling and Disposal of Waste. PSNS & IMF is the owner of all waste (hazardous or otherwise) generated within its facilities. This includes waste generated by contractor personnel while working at PSNS & IMF. All waste that originates, or is generated, on PSNS & IMF must be designated and tracked, even if the waste has been designated as non-hazardous. Additional information may be found in P5090 (5) - Contractor’s Guide to Hazardous Waste Compliance.
8.2. Waste identification and designation. Any item or material not incorporated into the project and not reusable will be considered a waste. The Contractor is responsible to identify all waste to be generated or produced by their performance of this contract. They shall complete Section 1 of a Waste Information Sheet (WIS) for each identified waste and submit the WIS to the Receiving Activity’s Point of Contact at least ten (10) business days prior to the generation of that waste. The Government (Code 106.33) will then designate the waste listed on the Waste Information Sheet (WIS). The Government (Code106.33) shall perform any analysis required for designation of waste.
8.3. Waste Management. The Contractor shall be responsible for the following:
8.3.1. In no event shall any waste, including wastewater, be disposed of in the storm sewer system.
8.3.2. Discharge to the sanitary sewer is also prohibited without written authorization from Code 106.33 Environmental, Safety, and Health Department via the Waste Information Sheet (WIS) process.
8.3.3. Solid waste shall be containerized to prevent spills or discharges to the environment.
8.3.4. Cover and contain all solid waste to prevent it from blowing away, to control the spread of waste by rats, birds, raccoons, etc., and to prevent water run-on or run-off. The area around the solid waste collection areas shall be kept clean and free of debris.
8.3.5. Disposal of solid waste shall be in containers specified by Code 106.33
8.4. Hazardous waste (HW).
8.4.1. All waste designated by the Government as Hazardous or Dangerous shall be accumulated in a Contractor operated Satellite Accumulation Area (SAA) or a <90 day Accumulation Area (AA) that has been certified by C/106.33, or turned over to a Government operated <90 day AA (for accumulation and disposal) prior to the end of the work shift. All Hazardous or Dangerous Waste shall be accompanied by a processed WIS.
8.4.2. Under no circumstances shall the Contractor remove HW from the Government’s premises. The Government will retain ownership of all HW.
8.4.3. Containers, labels and blank WISs necessary for proper hazardous waste management will be provided by the Government (Shop 99HM).Containers and labels are available for pick-up in Bldg. 367, located in the Controlled Industrial Area (CIA), Monday through Friday between the hours of 0745-1530. Requests for bulk containers require seven (7) days advance notice.
8.4.4. Hazardous waste containers shall have an ID label (PSNS 5090/82) to identify the type of waste, HW/WSW (PSNS 5090/81 or PSNS 5090/183) label, and DOT label(s) as specified by the WIS.
8.4.5. Designated hazardous and dangerous waste shall be turned over to PSNS & IMF Shop 99 HM with a processed WIS.
8.4.6. Provide the SDS and other supporting data as requested.
8.5. Non-hazardous waste.
8.5.1. All waste designated by the Government as non-hazardous (e.g. Landfill Controlled or Solid Waste or Recyclable) shall be properly handled, managed, and disposed of by the Contractor and tracked with Solid Waste Tracking Sheets (SWTS) and Contractor Monthly Project Waste Summary Report (CMPWSR).. WAC-173-304 Minimum Functional Standards for Solid Waste Handling.
8.6. Waste Awaiting Designation (WAD).
8.6.1. Waste Awaiting Designation (WAD) (i.e. waste for which designation as hazardous, problem, or non-hazardous waste has not been completed) shall be controlled as hazardous waste per Section 8.4 until proper waste designation and direction has occurred.
8.6.2. All samples and testing for designation will be taken and performed by the Government.
8.6.3. The Contractor shall apply a completed ID label for each container of WAD. Include the letters “WAD” in the space below the “ID” indicator located on the left side of the ID label. A completed WIS shall be immediately submitted to Code 106.33 via the Receiving Activity’s Point of Contact to start the waste designation process. A copy of the submitted WIS shall be kept with the container(s) of waste to be designated.
9. SAFETY OF PERSONNEL AND EQUIPMENT
9.1. Personal safety and health. The proposed work described herein will be performed in an industrial manufacturing area under the guidelines of OSHA and the Receiving Activity’s Environmental, Safety, and Health Office.
9.1.1. The Contractor shall conduct all work in a safe manner and shall provide all necessary safety equipment during the execution of this contract.
9.1.2. The Contractor shall make the maximum use of low-noise emission equipment as certified by the Environmental Protection Agency. Applicable regulatory requirements for maximum environmental noise levels are per WAC 173-60. The Contractor shall provide hazardous noise signs and label equipment, wherever work procedures and equipment produce sound-pressure levels greater than 84 dB(A) steady state and/or 140 dB peak sound pressure level for impact or impulse noise, regardless of the duration of the exposure.
9.1.3. Contracting personnel shall perform all work in accordance with the latest OSHA rules and regulations issued by the Department of Labor, 29 CFR Parts 1910, 1915, and 1926 as applicable.
9.1.4. All contractor personnel shall have in their possession and shall properly wear OSHA approved personnel protective safety equipment (i.e. hard-hats, steel-toe safety shoes, safety glasses and hearing protection).
9.1.5. The Contractor shall provide all appropriate safety barricades, signs, and signal lights required to properly isolate the area of work.
9.1.6. The Contractor’s work space may be inspected periodically for OSHA violations.
9.1.6.1. Abatement of violations will be the responsibility of the Contractor and/or the Government as determined by the Contracting Officer.
9.1.6.2. The Contractor shall provide assistance to the Environmental, Safety, and Health escort and the federal OSHA inspector if a complaint is filed. Fines levied on the Contractor by federal OSHA offices due to safety/health violations shall be paid promptly.
9.1.6.3. Failure to comply with, or repeated violations of, OSHA safety requirements can result in the violator(s) losing their access to the Government facility or the operation being suspended until the Contractor can provide properly trained personnel. Certification of training shall be presented upon request by the Environmental, Safety, and Health office.
9.1.6.4. The Contractor shall ensure that any additional measures that the Contracting Officer or the Receiving Activity’s Point of Contact determines to be necessary for safety are taken.
9.1.7. Additional safety and health requirements shall be as specified in other paragraphs of this contract.
9.2. Fire Hazard Awareness. The Contractor and his/her employees shall know where the fire alarms are located and how to activate them. The Contractor shall handle and store all combustible supplies, materials, waste, and trash in a manner that prevents fires or hazards to persons, facilities, and materials. Contractor employees shall respond to a fire or evacuation alarm.
9.3. Accident Reporting. The Contractor shall submit to the Contracting Officer and/or the Receiving Activity’s Point of Contact a full written accident report pertaining to all accidents resulting in death, trauma, or occupational disease. Accident reports shall be submitted within 24 hours of the accident occurrence.
9.4. Damage to Government Property. The Contractor shall submit to the Contracting Officer and/or the Receiving Activity’s Point of Contact a full written report of damage to Government property or equipment by contractor employees. Damage reports shall be submitted within 24 hours of the occurrence.
9.5. Emergency Medical Care. Only emergency medical care is available in Government facilities to Contractor employees who suffer on-the-job injury or disease. Care will be rendered at the rates in effect at the time of treatment. Reimbursement shall be made by the Contractor to the Naval Regional Medical Center Collection Agent upon receipt of statement.
