FIRST ARTICLE TEST (FAT) WITNESS PROCEDURES
VIDEO DOCUMENTATION GUIDELINES (REV NC)

Purpose:
Define procedure and guidelines for alternative to “in-person” engineering witness of FAT. This procedure would only be required or invoked under adverse circumstances where “in-person” witnessing is not possible or advisable.
Guidelines:
1. All contractor proposals and procedures must be reviewed and approved by the cognizant C17 engineering department prior to performance
2. Means of information transmittal (i.e.: mail, electronic, etc.) shall be approved prior to performance
3. Results shall be recorded on a permanent media and results delivered in conjunction with FATR (CDRL A002) for formal approval via WAWF
4. Specific dates of performance and attendees, including DCMA if known, shall be documented
Procedure:
1. Facility location(s) and qualifications for performing the FAT shall be approved by cognizant C17 engineering organization prior to proceeding.
2. Formal testing procedure with data recording shall be defined, documented and submitted for approval by cognizant C17 engineering department prior to the performance.
3. The written procedure shall include, as a minimum the following:
a. Test data parameters to be measured,
b. Means of measurement, including type, accuracy and calibration status of inspection gauging equipment
c. Schematic and/or diagram/ photo of UUT set-up,
d. Personnel qualifications for test performance (i.e.: test engineering or quality certifications),
e. Test steps in order of performance sequence
4. Visual record to include as a minimum:
a. Narrative of test performance as it occurs,
b. Delineation of equipment and purpose of each as test progresses through each step, FIRST ARTICLE TEST (FAT) WITNESS PROCEDURES

c. Visual record of “step-by-step” performance of the UUT set-up, gauge views, etc.
d. Record of all gauge and/or calculated readings,
e. Record of identification plate images of sufficient view duration and location as to confirm recorded information
f. Narrative of final results and record
5. Results and all sustaining evidence summarized and documented for submittal in conjunction with formal FATR submitted for WAWF approval.
