About this file

The document is an Affirmation and Disclosure Form related to a state contract requirement, designed to ensure transparency and compliance with executive orders regarding offshore services and data location. The form requires contractors to disclose the principal location of their business, the locations where services will be performed, and the specific locations where state data will be stored, including details for any subcontractors involved in the contract. Contractors must affirm that they have read and understand prohibitions on performing offshore services, locating state data offshore, or purchasing from Russian institutions or companies.

The form mandates that contractors have a continuing obligation to disclose any changes in service or data location before, during, and after contract execution, with the state retaining the right to immediately terminate the contract if services are moved outside the United States without a signed waiver. Failure to provide complete location information may result in contract award denial. The form requires an authorized contractor representative to sign, acknowledging understanding that this disclosure is incorporated into the potential contract and that they are bound by its terms regarding service and data location transparency.
