Section 1

SPECIFICATIONS/ SCOPE OF WORK

1.1 PROJECT SUMMARY

The Wayne County Nursing Home (WCNH) is seeking proposals for communication training and facility goal setting based on the DISC assessment. The target population will be all staff within a skilled nursing facility, across all shifts, with the majority of staff falling into direct care roles. The current staff estimate is 220. In-person training is requested and preferred for the program.

1.2 BACKGROUND

WCNH is a 192-bed skilled nursing facility that is county-owned, and serves residents of Wayne and surrounding counties for both long-term care and rehabilitative care. Facility occupancy remains based on staffing levels to meet state and/or federal mandates, and is currently less than 75 percent. The facility has launched an advertisement and recruitment campaign, along with recently increased wages, to try to attract additional nursing staff to the facility to allow for increased occupancy. Leadership changes, staff turnover, COVID, and other factors have contributed to reduced communication and employee dissatisfaction. WCNH aims to rectify communication issues, set goals, and improve staff morale by conducting communication education based on the DISC assessment.

The DISC assessment is a widely recognized tool to help teams and individuals understand their behavior and communication styles and preferences. Participation in the DISC assessment can assist with communication improvement, team development, and foster collaboration.

1.3 MINIMUM QUALIFICATIONS

The preferred candidate will possess experience in the DISC assessment for individuals and groups, including the explanation, interpretation, and insight on behavior characteristics, communication checklists/preferences, perception, natural vs. adapted style, and other modalities from the DISC assessment. The ability to assess a large number of individuals and teams is required. Background in executive/business coaching/training, social work, human resources, or other related experience required. Experience in organizational goal setting is required. DISC certification is preferred.

1.4 SCOPE OF SERVICES

The Vendor/Contractor will provide DISC communication training based on the following parameters:

1. In-person meetings prior to DISC assessments for each group, to prepare for the assessment and explore facility/team needs. It will require multiple meetings based on staff size and shifts worked.

2. DISC assessment distribution and calculation.

3. Debriefing of personal assessments as a group with the option for a 1:1 debriefing.

4. Summary of findings of the team component of assessments to administration to identify needs and implementation suggestions/goals.

5. Assist with facility goal setting, including tools, tracking mechanisms, suggestions of additional training, etc.

1.5 PRICING AND SUBMISSION REQUIREMENTS

The County is seeking to establish a project fee based on the estimated staff number of 220, with additional cost of services if assessments exceed that number, and establish contract with a contractor(s) for a term of one (1) year with the option for two (2) additional one (1) year periods. The total number of 220 staff is not guaranteed, and the facility reserves the right to delay or end the program without full staff completion.

Please provide an estimated project amount, project cost breakdown (hourly and/or rate by attendee), any additional costs that could be incurred, a project timeline, sample assessment/report, plan for accommodating

Page | 3 evening/night shift staff, and a mechanism and frequency of reporting to administration. Provide resume(s) for staff involved in proposed program, along with any certifications, affiliations, or accreditations that may apply.

EVALUATION PROCEDURES

3.1 EVALUATION SUMMARY

Proposals will be evaluated based upon their overall value to the County, which included both cost and technical factors (as outlined in this section).

3.2 CRITERIA AND WEIGHTS

Proposals that are found to be responsive, will be evaluated and assessed in accordance with the evaluation criteria below.

Criteria Points

Cost 200

Proposed work plan, timeline 350

References of similar scale and scope 50

Quality of proposed reporting abilities/ features and metrics

Firm’s capabilities and experience with Skilled Nursing Facilities staff

Ability to accommodate and provide in-person training across all shifts

Total Maximum Points Available 1,000

3.3 CALCULATIONS

The Purchasing Department will prepare a spreadsheet to input and analyze all Offeror information and evaluation scores.

The following approach will be used to calculate an Offeror’s score for cost:

(Lowest Cost / Cost of Proposal being evaluated) *(Weight Point Possible)

Lowest cost will receive maximum points for that category and subsequent proposals will receive a proportionate amount using the above formula. For example, if the Offeror provides a cost of $5, and the lowest cost was $3, and the maximum points was “30”, the Offeror would receive: ($3)/ ($5) *(30) = 18 points

3.4 RESPONSIVENESS

To be considered responsive, at a minimum, Offerors must complete and submit all of the required information (requested in this RFP and the Proposal Forms) on time and submitted as specified in this RFP.

An Evaluation Committee will be used to evaluate proposals of all responsive Proposals and participate in the Interview process (if required). The Evaluation Committee is expected to contain approximately 3-5 individuals that may, or may not, have technical expertise with this particular type of project or service. Therefore, it is important that the Offerors prepare and submit non-technical documents (to the maximum extent possible).

The Evaluation Committee will be asked to independently review and score the Submittals/Interviews comparatively to one another based on a 0 to 10 scale. The scale is based on the following criteria:

Points Awarded

Quality Criteria

10 Exceptional Exceptional; far exceeds expectations

8 Very Good Exceeds expectations

Page | 8

6 Acceptable Meets expectations and all minimum requirements

4 Below Requirements

Does not meet expectations or minimum requirements

2 Well Below Requirements

Fails to meet minimum requirements; proposes a solution that is not acceptable

0 Unacceptable Proposed solution deemed unacceptable in every aspect

The score as a percentage will then be multiplied by the weight of the category

The Purchasing Department will verify each Evaluation Committee member's score and average the scores to obtain the final average score for each offeror.

3.6 EVALUATION PROCESS

The following procedures will be followed to evaluate Proposals:

1. The Purchasing Department will review all proposals for compliance with mandatory requirements as stipulated within the RFP. Proposals deemed non-responsive will be eliminated from further consideration. The Purchasing Department may contact Offerors for clarification of the responses.
