Statement of Work
Project Sam Houston Recreation Area Mowing Contract
General Information
1.0 Scope of Work
The contractor shall provide all materials, labor, equipment, tools, transportation, supervision, expertise, and other incidentals necessary, except as otherwise provided, to successfully complete grounds maintenance at the Sam Houston RD Recreation Areas:
Cagle Rec Area, Stubblefield Rec Area, Scotts Ridge Rec Area (2 areas), and Double Lake Rec Area in accordance with attached and referenced specifications.
An organized site visit is not scheduled. Offerors are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. Contact Sam Houston Ranger District to schedule a site visit. In no event shall failure to inspect the site constitute grounds for a claim after contract award.
Nothing restricts the Government from performing any of the work with its own organization and workforce.
Hours of work shall be anytime between the hours of 8:00 am and 4:30 pm Monday through Thursday.
Services to be provided on a CALL WHEN NEEDED BASIS as shown in the specifications below.
If grounds maintenance is not required, a designated Contracting Officer Representative (COR) for the maintenance contract will notify the Contractor by 4:30 pm on Monday of the week that maintenance is not necessary. Mowing will not be performed during inclement weather. The mowing season may be extended in order to complete all specified service quantities, not to exceed the overall contract Period of Performance.
2.0 Background
Sam Houston National Forest District Office: 394 FM 1375 West, New Waverly, Texas
1. Cagle Recreation Area: From New Waverly, at intersection of I-45 and FM 1375, travel west 5 miles to entrance on left.
2. Stubblefield Recreation Area: From New Waverly, at intersection of I-45 and FM 1375 travel west 9 miles; turn right (NW) on
FSR 215 for 3 miles, entrance on right.
3. Scott’s Ridge Boat Launch Area and Scott’s Ridge Day-Use Area: From Willis, at intersection of I-45and FM 1097, travel west
8.5 miles to Scotts Ridge Road, turn right (north). Scott’s Ridge is the end of the road.
4. Double Lake Recreation Area: From New Waverly, at intersection of I-45 and FM 1375 travel east 21 miles on TX-150E; turn right on FM2025 for 0.5 miles, entrance on left.

3. Grounds Maintenance – Mowing: The Contractor is cautioned that some of the terrain to be mowed is rough and irregular. The Contractor shall be responsible for replacement of any broken windows or other damage, either of buildings, vehicles, or campers or other features, resulting from contract actions. Any litter, tree limbs, rocks, or other debris shall be picked up and disposed of properly prior to mowing.
Grass Height: All grass will be cut, edged, and trimmed to a height of two inches (2”) when measured on level ground such as grassy beaches. Lawn clippings may be left in place when mowing, but must be removed from sidewalks, individual picnic and camping sites and their spurs, roadways, parking areas, water fountains and picnic shelters, and other areas as directed by the Government.
Trimming: The contractor shall be responsible for performing trimming in conjunction with the cutting/mowing operation. The Contractor shall be responsible for any undue damage resulting from the trimming, such as cutting of tree bark, broken windows, and killing of landscaping vegetation resulting from contract actions. All grass shall be trimmed even with the edges of all curbs, sidewalks, driveways, walls, poles, tree trunks, foundations or other objects contained within or adjacent to the mowed areas.
Trimming operations shall result in a uniform appearance. Trimming debris shall be removed from sidewalks, individual picnic and camping sites and their spurs, roadways, water fountains and picnic shelters.
GENERAL: The contractor shall remove all tree limbs, branches, cuttings, etc., from the area adjacent to the trash receptacles and deposit in an area designated by the COR. The Contractor shall clean-up work areas, and restore any damaged landscape, government property, or buildings to their original state from damage resulting from contract actions. The Contractor shall supply all materials, labor, and equipment to complete all work specified in this scope of work. The contractor shall notify the government of any maintenance needs or safety concerns encountered during the performance of duties related to this contract.
PERSONAL PROTECTIVE EQUIPMENT - Contractor Responsibility Contractor shall train workers in the safe operation and use of equipment that the worker may use before the worker begins using such equipment.
Personal protective equipment (PPE), including equipment for eyes, face, head, ears, and extremities must be provided (at no cost to employee), used, and maintained in a sanitary and reliable condition. PPE must be used wherever it is necessary by reason of hazards or processes encountered which may cause injury or impairment in the function of any part of the body.
In any area where the worker is exposed to the potential for flying or falling objects, the Contractor shall provide a hard hat to employees. The Contractor shall assure that employees wear the hard hat and all required PPE. The hard hat must meet the minimum requirements of American National Standards Institute (ANSI) standard Z89.1-1997.
4. CONTRACTOR/COR REQUIREMENTS
Technical Requirements / Tasks
This contract calls for the contractor to perform the following tasks and subtasks and to perform inspections at the completion of the tasks/subtasks as a minimum and or for intermediate inspections as determined at contract award by the COR/CO.
INVOICING INSTRUCTIONS: The contract will only invoice 1 time per month on the last date of service for that month for all services completed during that month. The contractor must coordinate with the COR to make sure they will be available to approve the invoice in IPP.gov The COR will inspect services for every monthly invoice submitted and will input “Services Inspected DD/MM/YYY” in IPP.gov when they approve each invoice.

D. METHODS OF SURVEILLANCE
The government, through each COR, shall monitor the contractor's performance through a variety of methods. The methods to be used are:
1. 100% inspection of the contractor's inspection results.
2. Customer surveys.
3. Periodic inspections by COR.

Procedures
1. The COR will use one Quality Assurance (QA) Checklist form to document and evaluate the contractor's performance under the contract. The form, when complete, will document what the contractor was supposed to do, what was actually done, and the impact or consequences of what was done.
2. The COR must substantiate all tasks which he judges to be indicative of "unacceptable" performance. Performance at the "acceptable" level is expected and need not be substantiated.
3. If the COR documents unacceptable performance, they will provide the QA Checklist to the contractor. The contractor will be given the opportunity to correct the issue within 3 business days.
4. The contractor is required to respond in writing to any negative QA monitoring form within 3 working days after receipt of the form.
5. In the event the contractor is unable to correct the problem for that service, the COR must report the issue in writing to the CO within 3 business days of the issue not being corrected.
6. Upon receipt of the unacceptable service performance from the COR, the CO will contract the Contractor in writing (email) to notify the contractor of the unacceptable service performance and give them 3 days to remedy that specific problem. If the issue is not resolved within 3 business days, the contractor will not be paid for that specific service. The CO will file a memo for record in the contract file along with the QA Checklist form that documented the unacceptable service along with the email traffic notifying the contractor to correct the issue.
7. The Contracting Officer may choose to terminate the contract for repeated unsatisfactory contractor’s performance by using FAR 52.249-1 Termination for Convenience of the Government (Fixed-Price) (Short Form) (Apr 1984) or FAR 52.249-8 Default (Fixed-Price Supply and Service) (Apr 1984) depending on the circumstances.
