The Health Resources and Services Administration (HRSA) awarded a $3,675,000 Project Grant under the Autism Collaboration, Accountability, Research, Education, and Support (CARES) Act of 2014 program (CFDA 93.877) to the Association of Maternal and Child Health Programs (AMCHP) and its consortium partners to establish the National Coordinating Center on Transition (NCCT) program. The NCCT aims to develop comprehensive frameworks and resources to facilitate successful transitions from child to adult-serving systems for youth with autism and/or epilepsy who have complex health and social needs. Key objectives include assembling a diverse national advisory council, engaging stakeholders, assessing transition barriers, defining successful transition, establishing the NCCT as a resource hub, and providing training and technical assistance. AMCHP will serve as the backbone organization leveraging its collective impact model. The project will also include sub-awards to Child Trends to lead family and caregiver navigation strategies, and to Family Voices to lead family and youth engagement efforts.
