The intervention bundle includes 1) HF-focused home palliative care, 2) visiting neighbor support for functional health, and 3) faith-based nurse-provided palliative care. The RCT, to be conducted at West Virginia University, aims to address primary outcomes for HF patients and family caregivers, as well as secondary outcomes on the helpfulness, cost, and sustainability of the intervention components. The qualitative and quantitative results will be analyzed against a social support conceptual framework to identify facilitators and barriers to sustainability. This project supports research to improve rural health equity by increasing access to services and addressing social determinants of health disparities in Appalachia.
