The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) awarded a $614,420 Project Grant (R01DK139152) to the Hennepin Healthcare Research Institute (HHRI) to conduct a randomized trial evaluating the "Diabetes Homeless Medication Support (D-HOMES)" program. The D-HOMES program provides behavioral counseling and connects participants experiencing homelessness and type 2 diabetes to local community resources to improve glycemic control. The 5-year project aims to test the effectiveness of the D-HOMES program versus enhanced usual care in achieving better long-term glycemic outcomes, medication adherence, psychological wellness, and reduced diabetes distress among this underserved population. The effort also includes an implementation evaluation to assess the program's reach, adoption, and future scalability through the HRSA-funded National Network of Health Care for the Homeless programs.
