The National Institute on Aging (NIA), through the Aging Research Federal Grant Program (CFDA 93.866), awarded a $819,713 project grant to the University of Chicago to implement and evaluate the "CommunityRxDPeer" intervention. This IT-based, low-intensity social care navigation program is designed to connect dementia caregivers, particularly those from historically marginalized communities, with vital community resources and peer support. The 5-year project aims to assess the intervention's effectiveness in improving caregiver health and well-being, healthcare utilization, and social outcomes. Key components include caregiver education, personalized resource referrals, and ongoing support from trained peer navigators.
