The key objectives of the research project are to: 1) evaluate the efficacy of active DTMS in increasing dorsal anterior cingulate cortex (dACC) activation and reducing drinking compared to a sham condition; 2) assess the trajectory of target engagement changes after 15 versus 30 DTMS sessions; and 3) evaluate the durability of neural changes and impact on treatment outcomes, such as percent days abstinent and percent heavy drinking days. The project will enroll 100 adults with alcohol use disorder into a randomized, double-blind, sham-controlled trial to confirm target engagement and measure the impact on drinking behaviors. Neuroimaging assessments will be conducted at baseline, after half the treatment sessions, post-treatment, and 1 month after treatment.
