This Project Grant award from the Health Resources and Services Administration (HRSA) under the Community Health Centers program (CFDA 93.224) provides $600,000 in funding to University Community Health Services, Inc. (UCHS) to expand integrated behavioral health and substance use disorder services in Nashville, Tennessee. The two-year project period from September 1, 2024 to August 31, 2026 aims to improve behavioral health and substance use disorder outcomes, with a focus on maternal health patients with co-existing conditions. The proposed model will deliver integrated services through three existing community health sites and a mobile health unit, providing a minimum of 42 hours per week of care along with 24/7 phone assessment and triage. UCHS plans to serve 400 new patients with 4,000 visits over the project period. As an FQHC with a track record of delivering critical health services to underserved Nashville communities, UCHS is well-positioned to expand its integrated behavioral health programming and improve outcomes for vulnerable populations.
