The Rhode Island Department of Health (RIDOH) was awarded a $600,000 Project Grant under the Community Programs to Improve Minority Health Grant Program (CFDA 93.137) from the U.S. Department of Health and Human Services' Office of the Assistant Secretary for Health. The funding will support the development and implementation of the Providence Community Advisory Network (Providence CAN) project, which aims to improve hypertension management and diabetes prevention among Providence residents served by four safety-net clinics and three Providence-based Health Equity Zones (HEZs).

The project will establish a community-led network of trusted messengers, including community members, community-based organizations, community health workers, and clinicians, to inform and engage in the project design, implementation, and evaluation. This inclusive approach will address social determinants of health and contribute to disparities in heart disease, hypertension, and diabetes. Key project activities include: (1) forming and convening the Providence CAN on a monthly and quarterly basis, (2) implementing community health worker clinical interventions within the network, and (3) providing mini-grant opportunities to reflect the cultural and linguistic preferences of the target population. The project will run from September 1, 2024, through August 31, 2028.
