The New Jersey Department of Human Services was awarded a $17 million Cooperative Agreement by the Centers for Medicare and Medicaid Services (CMS) to implement the Transforming Maternal Health (TMaH) Model, a 10-year federal grant program (CFDA 93.869) designed to improve maternal health care outcomes for Medicaid and CHIP beneficiaries in New Jersey.
The grant funding will be used to: 1) Conduct deep stakeholder engagement to understand the member, provider, and community experience of care, including compensating members for their lived expertise; 2) Develop peer-to-peer technical assistance for providers participating in the TMaH payment model; and 3) Provide Medicaid staffing to support the qualitative, quantitative, and technical work needed for successful TMaH model implementation. The Department plans to leverage its existing initiatives in perinatal workforce development, person/family-centered care, and data-driven quality improvement to drive sustainable, measurable improvements in maternity care experienced by Medicaid members statewide.
