The Centers for Medicare and Medicaid Services (CMS) awarded a $17,000,000 Cooperative Agreement under the Transforming Maternal Health (TMaH) Model (CFDA 93.869) to the Louisiana Department of Health (LDH) Bureau of Health Services Financing. The funding will be used over a 10-year period to implement the TMaH model in a designated test region in Louisiana in order to improve maternal health outcomes and reduce health disparities. Key activities include enhancing LDH's data infrastructure, increasing home monitoring of conditions like diabetes and hypertension, and expanding screening and referrals for perinatal depression, substance use, and social determinants of health. LDH will partner with CMS to measure the effectiveness of the TMaH model in the test region compared to a similarly situated comparison region in Louisiana. The project aims to significantly reduce maternal and infant mortality rates while increasing access to high-quality, coordinated perinatal care for Medicaid and CHIP beneficiaries.
